【摘要】：目的:探討城鄉(xiāng)居民基本醫(yī)療保險(xiǎn)基金平衡的測(cè)算方法,為醫(yī)保管理部門(mén)和研究者提供科學(xué)參考。方法:通過(guò)現(xiàn)場(chǎng)調(diào)查獲得參保人群的就醫(yī)參數(shù),采用蒙特卡羅隨機(jī)模擬的方法,模擬測(cè)算在不同補(bǔ)償方案下基金的平衡情況。結(jié)果:參保人群的年住院率為8.76%,縣內(nèi)住院比例為40%,實(shí)際補(bǔ)償率為52%;將鄉(xiāng)鎮(zhèn)衛(wèi)生院、縣級(jí)醫(yī)院、市級(jí)醫(yī)院和市外醫(yī)院住院的實(shí)際補(bǔ)償比分別提高到82%、75%、56%和51%時(shí),基金使用率分別為91%、95%、99%和96%。結(jié)論:同等比例提高縣級(jí)醫(yī)院和市級(jí)醫(yī)院的補(bǔ)償比時(shí),基金的使用率的提高相對(duì)較快,建議補(bǔ)償政策繼續(xù)向基層醫(yī)療機(jī)構(gòu)傾斜;隨機(jī)模擬技術(shù)可以為醫(yī)保基金的平衡測(cè)算提供新方法。
[Abstract]:Objective: to study the method of measuring the balance of basic medical insurance fund for urban and rural residents, and to provide a scientific reference for medical insurance management departments and researchers. Monte Carlo random simulation method was used to simulate and calculate the fund balance under different compensation schemes. Results: the annual hospitalization rate of insured population was 8.76, the proportion of county residents was 40 and the actual compensation rate was 52. When the actual compensation ratio of municipal hospitals and outside hospitals increases to 827575 / 56% and 51% respectively, the utilization rate of funds is 91 / 95 / 99% and 96 / 96 respectively. Conclusion: when the compensation ratio of county-level hospitals and municipal hospitals is increased in the same proportion, the utilization rate of funds increases relatively quickly. It is suggested that the compensation policy should continue to favor the primary medical institutions, and the stochastic simulation technology can provide a new method for the balance calculation of the health insurance fund.
