【摘要】：新型農(nóng)村合作醫(yī)療制度是我國(guó)農(nóng)村醫(yī)療衛(wèi)生改革發(fā)展進(jìn)程中的一項(xiàng)重大制度創(chuàng)新,該制度已經(jīng)在全國(guó)范圍內(nèi)順利開(kāi)展,并在減輕農(nóng)民醫(yī)療負(fù)擔(dān)和提高農(nóng)民健康水平上發(fā)揮出舉足輕重的作用。但是在新農(nóng)合制度具體的運(yùn)行過(guò)程中也面臨著許多問(wèn)題,新農(nóng)合制度的實(shí)施效果是否可以滿足農(nóng)民醫(yī)療健康的需求?新農(nóng)合制度面臨的瓶頸問(wèn)題應(yīng)該采取何種措施加以克服?新農(nóng)合制度的未來(lái)發(fā)展之路應(yīng)該依賴何種策略進(jìn)行探索?這些問(wèn)題已經(jīng)成為醫(yī)療保障領(lǐng)域的專家所關(guān)注的重點(diǎn)問(wèn)題。本文圍繞新農(nóng)合制度實(shí)際運(yùn)轉(zhuǎn)過(guò)程中面臨的核心問(wèn)題進(jìn)行研究。首先,針對(duì)新農(nóng)合制度實(shí)施效果的現(xiàn)狀進(jìn)行系統(tǒng)梳理與分析,詳細(xì)論述了評(píng)價(jià)體系的各個(gè)關(guān)鍵環(huán)節(jié),并歸納總結(jié)出現(xiàn)有評(píng)價(jià)體系的不足之處。其次,設(shè)計(jì)出新農(nóng)合制度實(shí)施效果評(píng)價(jià)體系的基本框架,并利用該體系對(duì)于成都市龍泉驛區(qū)新農(nóng)合制度的運(yùn)行情況進(jìn)行評(píng)價(jià),通過(guò)對(duì)于新農(nóng)合制度相關(guān)文獻(xiàn)的梳理并結(jié)合實(shí)地調(diào)研,構(gòu)建出新農(nóng)合制度實(shí)施效果綜合評(píng)價(jià)指標(biāo)體系,并采用模糊綜合評(píng)判法和云重心評(píng)判法分別進(jìn)行評(píng)估。在比較兩種評(píng)價(jià)方法的特點(diǎn)與差異的同時(shí),進(jìn)一步對(duì)評(píng)價(jià)結(jié)果進(jìn)行深入分析,指出了現(xiàn)行新農(nóng)合制度的問(wèn)題,并給出解決困境的建議和措施。最后,從宏觀角度分析新農(nóng)合制度未來(lái)的發(fā)展路徑,借鑒復(fù)雜科學(xué)中的適應(yīng)度景觀理論及NK模型的核心思想,構(gòu)建出新農(nóng)合制度適應(yīng)度景觀模型。綜合考慮新農(nóng)合制度運(yùn)行過(guò)程中各重要影響因素之間的相互關(guān)系及影響作用,探討當(dāng)前形勢(shì)條件下我國(guó)新農(nóng)合制度的路徑選擇問(wèn)題,為相關(guān)政府部門(mén)的決策提供參考依據(jù)。本文從我國(guó)農(nóng)村社會(huì)經(jīng)濟(jì)目前所處的實(shí)際背景出發(fā),在充分認(rèn)識(shí)和了解我國(guó)新農(nóng)合制度的基礎(chǔ)上,緊密?chē)@新農(nóng)合制度的模式特點(diǎn)、績(jī)效評(píng)價(jià)以及發(fā)展戰(zhàn)略等關(guān)鍵問(wèn)題進(jìn)行研究與探討,以期有助于促進(jìn)現(xiàn)有新農(nóng)合制度的完善,使新農(nóng)合制度快速適應(yīng)我國(guó)社會(huì)轉(zhuǎn)型的發(fā)展變化,進(jìn)一步推進(jìn)我國(guó)社會(huì)保障制度的建設(shè)與發(fā)展。
[Abstract]:The new rural cooperative medical system is a major institutional innovation in the development process of rural health care reform in China, which has been carried out smoothly throughout the country. And in reducing the burden of medical care for farmers and improve the level of health of farmers play a pivotal role, but in the new agricultural cooperation system in the specific operation of the process is also facing many problems. Can the implementation effect of the new rural cooperative system meet the medical and health needs of farmers? What measures should be taken to overcome the bottleneck of the NCMS system? What strategies should be relied on to explore the future development of the new agricultural cooperation system? These problems have become the focus of experts in the field of medical security. This paper focuses on the core issues in the actual operation of the NCMS system. First of all. According to the current situation of the implementation of the new agricultural cooperation system, this paper systematically combs and analyzes the key links of the evaluation system, and summarizes the shortcomings of the evaluation system. Secondly. The basic framework of the evaluation system for the implementation effect of the new agricultural cooperation system is designed, and the system is used to evaluate the operation of the new agricultural cooperation system in Longquanyi District of Chengdu. Through combing the relevant documents of the new agricultural cooperation system and combining with the field investigation, a comprehensive evaluation index system of the implementation effect of the new agricultural cooperation system is constructed. The fuzzy comprehensive evaluation method and the cloud center of gravity evaluation method are used to evaluate, and the characteristics and differences of the two evaluation methods are compared. At the same time, the evaluation results are further analyzed. This paper points out the problems of the current NCMS system, and gives some suggestions and measures to solve the difficulties. Finally, it analyzes the future development path of the NCMS system from a macro perspective. The adaptation landscape theory in complex science and the core idea of NK model are used for reference. Construct the landscape model of the adaptability of the new agricultural cooperation system. Consider the relationship and influence of the important factors in the operation of the new agricultural cooperation system. This paper discusses the path selection of the new agricultural cooperation system in China under the current situation, and provides a reference basis for the decision-making of the relevant government departments. This article starts from the actual background of our country's rural social economy at present. On the basis of fully understanding and understanding of China's new agricultural cooperation system, closely around the new agricultural cooperation system model characteristics, performance evaluation and development strategy and other key issues to be studied and discussed. The aim is to promote the perfection of the existing new agricultural cooperation system, to make the new agricultural cooperation system adapt to the development and change of our country's social transformation, and to further promote the construction and development of our country's social security system.
