【摘要】：居民醫(yī)保實現(xiàn)城鄉(xiāng)統(tǒng)籌,是新一輪醫(yī)改的重要工作之一。2012年,國務(wù)院在《關(guān)于印發(fā)“十二五”期間深化醫(yī)藥衛(wèi)生體制改革規(guī)劃暨實施方案的通知》中提出“有條件的地區(qū)要探索建立城鄉(xiāng)統(tǒng)籌的居民基本醫(yī)療保險制度”。作為本市城鄉(xiāng)發(fā)展一體化的重要工作之一,市委、市政府在《關(guān)于推動新型城鎮(zhèn)化建設(shè)促進本市城鄉(xiāng)發(fā)展一體化若干意見》中也明確要求“實施城鄉(xiāng)居民統(tǒng)一的基本醫(yī)療保險制度”。上海作為在全國在醫(yī)療保險方面一直處于領(lǐng)頭地位的直轄市,在城鄉(xiāng)居民基本醫(yī)療保險制度整合方面卻由于種種原因一直未展開。近兩年來上海市政府終于下定決心準備開展整合城鄉(xiāng)居民的醫(yī)療保險制度這一項工作并展開了一系列先期調(diào)研工作。在調(diào)研和后期的分析中發(fā)現(xiàn)了在上海市整合城鄉(xiāng)居民的醫(yī)療保險制度并不是簡單地把新農(nóng)合人群直接并入現(xiàn)有城鎮(zhèn)居民基本醫(yī)療保險就能解決的問題,兩個險種存在的籌資水平差異,支付補償水平差異以及開展此項工作和今后負責(zé)此項工作的職能部門還未明確等問題都將是今后在政策制定方面必須全方面考慮統(tǒng)籌的。本文就將此次調(diào)研結(jié)合國內(nèi)外先例及上海市醫(yī)保制度的實際情況對這些問題提出解決的方案。

In recent years the Shanghai municipal government finally decided to carry out the integration of urban and rural residents In this work the medical insurance system and launched a series of preliminary research work. In the investigation and the analysis found in the integration of urban and rural residents in Shanghai City, the medical insurance system is not simply the new population directly into the current basic medical insurance for urban residents will be able to solve the problem, there are two types of different financing levels the problem of payment of compensation level and carry out the work in the future and is responsible for the work of the functional departments is not clear and so will be the future policy must consider all aspects of planning. This paper will do the research according to the actual situation of the medical insurance system at home and abroad and precedent in Shanghai city and propose solutions to these problems.
