抗炎干預(yù)對(duì)兔髕腱及止點(diǎn)結(jié)構(gòu)及細(xì)胞因子變化的影響

【摘要】:目的:觀察不同周數(shù)跳躍訓(xùn)練及抗炎干預(yù)對(duì)兔髕腱及腱止點(diǎn)組織學(xué),免疫組化,實(shí)時(shí)熒光定量PCR等指標(biāo)的變化,探討負(fù)荷積累量與抗炎干預(yù)對(duì)髕腱及腱止點(diǎn)結(jié)構(gòu)變化、炎癥反應(yīng)相關(guān)蛋白、細(xì)胞外基質(zhì)合成與降解相關(guān)蛋白及自噬相關(guān)蛋白的影響,觀察布洛芬抗炎藥預(yù)防腱病的效果,并揭示其作用機(jī)理。方法:68只雌性新西蘭大白兔(體重2.46±0.11kg)隨機(jī)分為4組,即空白對(duì)照組CON,跳躍訓(xùn)練組OJ,跳躍加抗炎藥組JAD以及抗炎藥組OAD,各組又按時(shí)間不同分為2周,4周,6周及8周組。其中研究一僅涉及腱止點(diǎn),研究二僅涉及髕腱,研究三涉及二者之間的關(guān)系。跳躍訓(xùn)練采用三維測(cè)力臺(tái)記錄起跳、落地峰值力及沖量,分別于對(duì)應(yīng)的時(shí)間點(diǎn)取材,腱止點(diǎn)標(biāo)本行HE、番紅及免疫組化染色,髕腱行免疫組化及RT-PCR。觀察指標(biāo)包括纖維軟骨帶厚度、細(xì)胞密度,細(xì)胞因子TNFα、HMGB1、MMP-1、MMP-2、Beclin-1、LC3B、IL-1β、COX-2、PTGES2、COL-I、COL-III;其中腱止點(diǎn)檢測(cè)所有指標(biāo)的蛋白表達(dá),髕腱樣本僅檢測(cè)后5個(gè)指標(biāo)的蛋白及mRNA表達(dá)量。結(jié)果:(1)不同周數(shù)跳躍訓(xùn)練后經(jīng)體重標(biāo)化的生物力學(xué)結(jié)果顯示,OJ4W組的起跳沖量顯著低于OJ2W組(P=0.005);(2)組織學(xué)結(jié)果顯示,與相同時(shí)間點(diǎn)的CON組相比,OJ組隨跳躍訓(xùn)練周數(shù)的變化依次呈現(xiàn)出以下變化規(guī)律:腱止點(diǎn)處細(xì)胞密度增大,纖維軟骨帶增厚,潮線模糊或消失,細(xì)胞形態(tài)改變,分布不均,骨腱結(jié)合部固有的分層結(jié)構(gòu)消失,偏振光下膠原波浪形結(jié)構(gòu)消失、排列不連續(xù);番紅染色下粘多糖蛋白異常增多維持在一定的水平上;(3)免疫組化結(jié)果顯示,與相同時(shí)間點(diǎn)的CON組相比,OJ2W、OJ4W組TNFα、IL-1β、COX-2、PTGES2、MMP2顯著升高(P0.05),OJ6W組IL-1β、COX-2、MMP2、COL-III顯著升高(P0.05),OJ8W組IL-1β、COX-2、COL-I顯著升高但MMP1、Beclin顯著降低(P0.05);與同時(shí)間點(diǎn)的OJ組相比,JAD2W組COX-2、PTGES2、MMP2顯著降低(P0.05),JAD6W組MMP1、COL-I、COL-I/COL-III、LC3B抗炎干預(yù)對(duì)兔髕腱及止點(diǎn)損傷形成的影響顯著降低(P0.05),JAD8W組HMGB1、MMP1、COL-I、LC3B顯著降低(P0.05)。結(jié)論:(1)髕腱及止點(diǎn)結(jié)構(gòu)、炎癥因子變化關(guān)系密切。TNFα、IL-1β、MMPs、COL-I、COL-III等蛋白表達(dá)具有一定的時(shí)序性規(guī)律。(2)早期存在一定程度的炎癥反應(yīng)是有益的,而持續(xù)存在的炎癥反應(yīng)是對(duì)機(jī)體不利的。(3)抗炎干預(yù)具有較好的短期效應(yīng),集中體現(xiàn)在早期有助于緩解炎細(xì)胞浸潤(rùn)及保護(hù)膠原蛋白,但長(zhǎng)期效應(yīng)來看,僅能提前機(jī)體修復(fù)反應(yīng)的時(shí)間點(diǎn),對(duì)最終的愈合質(zhì)量幫助不大。(4)髕腱對(duì)載荷的承載能力明顯大于腱止點(diǎn),可以解釋腱止點(diǎn)損傷多于肌腱的原因。
[Abstract]:Objective: to observe the changes of histological, immunohistochemical and real-time fluorescent quantitative PCR in rabbit patellar tendon and tendon after different weeks of jumping training and anti-inflammatory intervention, and to investigate the changes of load accumulation and anti-inflammatory intervention on patellar tendon and tendon insertion. The effects of ibuprofen anti-inflammatory drugs on the prevention of tendon disease were observed and the mechanism of the effects of ibuprofen anti-inflammatory drugs on the prevention of tendonitis was observed in order to investigate the effects of inflammatory response related proteins, extracellular matrix synthesis and degradation related proteins and autophagy related proteins. Methods Sixty-eight female New Zealand white rabbits (weight 2.46 鹵0.11kg) were randomly divided into four groups: control group (control group), jumping training group (OJ), jumping plus anti-inflammatory drug group (JAD) and anti-inflammatory drug group (OAD). Each group was divided into 2 weeks, 4 weeks, 6 weeks and 8 weeks groups. Among them, one involves tendon insertion, the other involves patellar tendon, and the third concerns the relationship between the two. The jump was recorded by a three dimensional force measuring table. The peak force and impulse were recorded at the corresponding time point. The specimens were collected at the corresponding time point. The specimens were stained with HEH, Phanerin and immunohistochemistry, and the patellar tendon was stained with immunohistochemistry and RT-PCR. The observed parameters included the thickness of fibrochondrocytes, cell density, cytokine TNF 偽, HMGB1, MMP-1, MMP-2, Beclin-l, LC3BU, IL-1 尾, COX-2PTGES2PTGES2, COL-ICOL-III, in which the protein expression of all the indexes was detected at the tendon insertion point, and the protein and mRNA expression in the patellar tendon samples was detected only for the last five indexes. Results: (1) after different weeks of jumping training, the biomechanical results showed that the take-off impulse of OJ4W group was significantly lower than that of OJ2W group (P0. 005); (2). Compared with the Con group at the same time point, the changes of the number of jump training weeks in the OJ group showed the following changes in turn: the cell density at the tendon stop point increased, the fibrous cartilage band thickened, the tidal line blurred or disappeared, and the cell morphology changed unevenly. The lamellar structure of the bone tendon disappeared, the collagen wavy structure disappeared under polarized light, and the abnormal increase of mucopolysaccharide protein was maintained at a certain level.
