【摘要】：探索中國的家庭醫(yī)生制度模式是深化醫(yī)療衛(wèi)生體制改革過程中的重要政策議題。上海市長寧區(qū)是國家首批社區(qū)衛(wèi)生服務綜合改革試點區(qū),也是最早開展家庭醫(yī)生服務制度探索和改革的示范區(qū)。自2008年起開始逐步探索家庭醫(yī)生制度至今已有10年時間,為完善家庭醫(yī)生簽約服務,全面建立成熟完善的分級診療制度,形成基層首診、雙向轉(zhuǎn)診、上下聯(lián)動、急慢分治的合理就醫(yī)秩序積累了許多可復制、可推廣的經(jīng)驗。本專題3篇文章是來自長寧區(qū)家庭醫(yī)生制度綜合改革十周年的調(diào)查,分別從服務模式、政府協(xié)同、績效考核方面,展示了長寧區(qū)家庭醫(yī)生制度綜合改革的具體做法與政策成效。伴隨著全國家庭醫(yī)生簽約服務現(xiàn)場推進會的貫徹落實,中國家庭醫(yī)生制度的長寧模式將為全國各地的家庭醫(yī)生制度探索與深化改革提供寶貴經(jīng)驗。
[Abstract]:Exploring the pattern of family doctor system in China is an important policy issue in the process of deepening the reform of medical and health system. Changning District of Shanghai is one of the first pilot areas for comprehensive reform of community health services in Shanghai. It is also the first demonstration area to explore and reform the family doctor service system. Since 2008, we have been gradually exploring the family doctor system for 10 years. In order to improve the contract service of family doctors, establish a mature and perfect hierarchical diagnosis and treatment system in an all-round way, form the first consultation at the grass-roots level, two-way referrals, and linkage between the upper and lower levels. The rational medical treatment order of urgent and slow treatment has accumulated a lot of experience that can be replicated and popularized. Three articles on this topic are from the tenth anniversary of the comprehensive reform of the family doctor system in Changning District. From the aspects of service mode, government coordination and performance appraisal, the concrete practices and policy results of the comprehensive reform of the family doctor system in Changning District are shown. The Changning model of family doctor system in China will provide valuable experience for exploring and deepening the reform of family doctor system all over the country along with the implementation of the National Family Doctor signing Service on-the-spot Promotion Association.
