山西省三級(jí)綜合醫(yī)院專(zhuān)科護(hù)士使用現(xiàn)狀調(diào)查與分析

【摘要】:目的:本研究旨在通過(guò)問(wèn)卷調(diào)查法,了解山西省三級(jí)綜合醫(yī)院專(zhuān)科護(hù)士使用現(xiàn)狀。探討并分析影響專(zhuān)科護(hù)士使用的主要影響因素,通過(guò)對(duì)其因素的分析討論,找出專(zhuān)科護(hù)士使用中存在的問(wèn)題并提出改進(jìn)方法和策略,完善專(zhuān)科護(hù)士主要工作和職責(zé),為促進(jìn)專(zhuān)科護(hù)理的發(fā)展,滿足社會(huì)對(duì)專(zhuān)科護(hù)士的需求提供有針對(duì)性的建議。方法:本研究采用文獻(xiàn)研究法擬定專(zhuān)科護(hù)士使用情況問(wèn)卷草案,通過(guò)德?tīng)柗品▽?duì)22名專(zhuān)家進(jìn)行兩輪函詢(xún),確定預(yù)測(cè)問(wèn)卷。采用整群抽樣法,抽取山西省汾陽(yáng)醫(yī)院在內(nèi)的三所三級(jí)綜合醫(yī)院專(zhuān)科護(hù)士300名作為預(yù)調(diào)查對(duì)象,通過(guò)預(yù)調(diào)查進(jìn)行條目再修訂與篩選,確定最終條目,形成正式問(wèn)卷。采取整群抽樣法對(duì)山西省內(nèi)十所三級(jí)綜合醫(yī)院450名專(zhuān)科護(hù)士以網(wǎng)絡(luò)調(diào)查方式進(jìn)行正式調(diào)查,采用SPSS20.0軟件對(duì)數(shù)據(jù)進(jìn)行分析、處理。結(jié)果:1.專(zhuān)科護(hù)士使用情況測(cè)評(píng)問(wèn)卷,經(jīng)探索性因子分析最終構(gòu)建了由5個(gè)維度,37個(gè)條目組成的測(cè)評(píng)工具,其信度Cronbach’a系數(shù)為0.964,分半信度是0.743,重測(cè)信度是0.936,具有良好的信度和效度。2.對(duì)山西省內(nèi)專(zhuān)科護(hù)士的使用現(xiàn)狀調(diào)查結(jié)果顯示:專(zhuān)科護(hù)士使用情況問(wèn)卷總分分值較低,專(zhuān)科護(hù)士的整體使用情況不理想。各維度使用情況中臨床實(shí)踐得分最高,研究與咨詢(xún)得分最低。3.經(jīng)回歸分析,培訓(xùn)考核方式、專(zhuān)科護(hù)士有無(wú)分層使用、有無(wú)專(zhuān)科護(hù)士職稱(chēng)晉升、績(jī)效考核的政策支持、有無(wú)培訓(xùn)考核論文答辯、有無(wú)專(zhuān)科護(hù)士繼續(xù)教育培訓(xùn)、培訓(xùn)有無(wú)護(hù)理人文課程設(shè)置對(duì)總問(wèn)卷得分有影響。其中專(zhuān)科護(hù)士有無(wú)分層使用、有無(wú)專(zhuān)科護(hù)士職稱(chēng)晉升、績(jī)效考核的政策支持對(duì)專(zhuān)科護(hù)士使用情況得分有較高的解釋力。結(jié)論:1.該測(cè)評(píng)工具具有較好的科學(xué)性和實(shí)用性,為管理部門(mén)了解專(zhuān)科護(hù)士工作現(xiàn)狀提供了依據(jù),有利于更好的管理專(zhuān)科護(hù)士,使其發(fā)揮更大的作用。2.山西省內(nèi)專(zhuān)科護(hù)士使用現(xiàn)狀不理想,其因素與培訓(xùn)考核方式、醫(yī)院管理有關(guān)。建議規(guī)范專(zhuān)科護(hù)士的培訓(xùn)與考核,盡快出臺(tái)專(zhuān)科護(hù)士使用與管理方案。
[Abstract]:Objective: the purpose of this study was to investigate the use of specialized nurses in three-level general hospitals of Shanxi province by means of questionnaire survey. This paper probes into and analyzes the main influencing factors on the use of specialized nurses, through the analysis and discussion of the factors, finds out the problems existing in the use of specialized nurses and puts forward the improving methods and strategies to perfect the main work and duties of the specialized nurses. In order to promote the development of specialist nursing and meet the needs of the community for specialist nurses to provide targeted advice. Methods: in this study, a draft questionnaire for the use of specialized nurses was drawn up by literature research, and 22 experts were interviewed twice by Delphi method to determine the predictive questionnaire. Using cluster sampling method, 300 nurses of three tertiary general hospitals in Fenyang Hospital of Shanxi Province were selected as the objects of pre-investigation. The items were revised and screened through pre-investigation, and the final items were determined and a formal questionnaire was formed. A cluster sampling method was used to investigate 450 nurses in 10 tertiary general hospitals in Shanxi Province. The data were analyzed and processed by SPSS20.0 software. The result is 1: 1. By exploratory factor analysis, an evaluation tool composed of 5 dimensions and 37 items was constructed. The reliability of the questionnaire was 0.964, the split-half reliability was 0.743, and the test-retest reliability was 0.936, which had good reliability and validity. The results of investigation on the use status of specialized nurses in Shanxi Province showed that the total score of the questionnaire on the use of specialized nurses was low and the overall use of specialized nurses was not satisfactory. The clinical practice score was the highest and the research and consultation score was the lowest. Through the regression analysis, the training examination way, whether the specialized nurse has the stratified use, whether has the specialized nurse title promotion, the performance appraisal policy support, whether has the training appraisal thesis defense, whether has the specialized nurse continuing education training, Whether the training has the nursing humanities curriculum has the influence to the total questionnaire score. The policy support of performance evaluation has a higher explanatory power to the use of specialist nurses, whether there are stratified use, whether there are professional nurses promotion, and whether the policy support of performance appraisal can explain the use of specialist nurses. Conclusion 1. The evaluation tool is scientific and practical, which provides the basis for the management department to understand the current situation of specialized nurses, is conducive to better management of specialized nurses, so that it can play a greater role. The use of specialized nurses in Shanxi Province is not ideal, and its factors are related to the training and examination methods and hospital management. It is suggested that the training and examination of specialized nurses should be standardized and the use and management scheme of specialized nurses should be introduced as soon as possible.
