MTP-KPI干預(yù)模式在患兒Ⅰ類手術(shù)切口預(yù)防使用抗菌藥物中的應(yīng)用評價

【摘要】:目的觀察監(jiān)測-培訓(xùn)-計劃法(MTP)聯(lián)合關(guān)鍵績效指標法(KPI)干預(yù)模式在兒童Ⅰ類手術(shù)切口預(yù)防使用抗菌藥物中的應(yīng)用效果,進一步規(guī)范抗菌藥物在圍手術(shù)期的合理使用。方法將2014年1-12月的344例Ⅰ類手術(shù)切口患兒納入對照組,2015年1-12月的456例Ⅰ類手術(shù)切口患兒納入干預(yù)Ⅰ組,2016年1-12月的426例Ⅰ類手術(shù)切口患兒納入干預(yù)Ⅱ組,根據(jù)對照組預(yù)防用抗菌藥物的點評結(jié)果,建立清潔手術(shù)預(yù)防用抗菌藥物的合理性評價指標(預(yù)防用藥率、選用抗菌藥物合理率、給藥時機合理率、用藥療程合理率和術(shù)后感染率),干預(yù)Ⅰ組采用MTP干預(yù)模式,干預(yù)Ⅱ組采用MTP-KPI干預(yù)模式,比較干預(yù)前后合理性評價指標的改善情況。結(jié)果通過MTP-KPI模式干預(yù),干預(yù)Ⅱ組Ⅰ類手術(shù)切口的預(yù)防用藥率為9.86%,低于對照組37.79%,低于干預(yù)Ⅰ組14.25%,差異有統(tǒng)計學意義(P0.05);干預(yù)Ⅱ組的選用抗菌藥物合理率、給藥時機合理率、用藥療程合理率分別為95.24%、95.24%、88.10%,高于對照組0.8%、75.31%、72.38%,高于干預(yù)Ⅰ組58.46%,90.77%,86.15%,同時腹外疝手術(shù)的預(yù)防用藥評價指標均有顯著改善;對照組、干預(yù)Ⅰ組、干預(yù)Ⅱ組的術(shù)后感染率分別為0.29%、0.44%和0.23%,差異無統(tǒng)計學意義。結(jié)論 MTP-KPI干預(yù)模式對提高Ⅰ類手術(shù)切口預(yù)防用抗菌藥物的合理性有顯著效果,可行性高。
[Abstract]:Objective to observe the effect of monitoring, training and planning (MTP) combined with key performance index (KPI) intervention model in preventing the use of antimicrobial agents in children with type I surgical incision, and to further standardize the rational use of antibiotics in perioperative period. Methods 344 children with type I incision from January to December 2014 were included in the control group, 456 children with type I incision from January to December 2015 were included in the intervention group, 426 children with type I incision from January to December 2016 were included in the intervention group.
