【摘要】：目的:本研究應(yīng)用滇重樓巴布劑外敷結(jié)合雙氯芬酸鈉緩釋膠囊口服,觀察并評價對治療急性痛風性關(guān)節(jié)炎的臨床療效。以單純口服雙氯芬酸鈉緩釋膠囊作為參照,對比并評價與探討兩種方法的臨床療效差異,探討滇重樓巴布劑的療效和優(yōu)勢,嘗試為弘揚中醫(yī)藥瑰寶、發(fā)掘云南道地藥材、開發(fā)新型院內(nèi)制劑提供一些決策參考和建議。方法:將2014年12月~2015年12月就診于昆明市中醫(yī)醫(yī)院的符合納入標準、療程及隨訪觀察完整的53例急性痛風性關(guān)節(jié)炎病例,按奇偶日期分類法(奇數(shù)日期就診患者納入對照組、偶數(shù)日期就診患者納入治療組),對照組患者27例,治療組患者26例,兩組均服用雙氯芬酸鈉緩釋膠囊,治療組再外敷滇重樓巴布劑,對照組不采用任何外用藥物,僅自行局部冷敷。通過定期觀察、患者復查及隨訪方式,觀察并記錄關(guān)節(jié)紅腫、膚溫、疼痛、功能受限,血尿酸;以及局部瘙癢、紅疹,肝功能(ALT、AST)、腎功能(Scr)等各項安全性指標。對采集數(shù)據(jù)進行統(tǒng)計學分析,評價兩種不同治療方法的臨床療效。結(jié)果:在本次治療急性痛風性關(guān)節(jié)炎的臨床療效觀察中,對照組和治療組在1個療程內(nèi)對紅腫熱痛、活動功能均可有效的改善,在痛風性關(guān)節(jié)炎急性期,兩組對降低尿酸無統(tǒng)計學意義;兩組在疼痛、紅腫、關(guān)節(jié)活動功能方面,治療組的改善率明顯高于對照組,差異有統(tǒng)計學意義;在改善關(guān)節(jié)膚溫方面,兩組相當,療效的差異無統(tǒng)計學意義。結(jié)論:外敷滇重樓巴布劑配合常規(guī)口服雙氯芬酸鈉緩釋膠囊較單純口服雙氯芬酸鈉緩釋膠囊在減輕關(guān)節(jié)疼痛、紅腫、活動功能方面有明顯改善;以及未發(fā)現(xiàn)外敷藥所帶來的局部、全身的副作用等癥狀,值得臨床進一步推廣運用。
[Abstract]:Objective: to observe and evaluate the clinical effect of Dianzhonglou cataplasma combined with diclofenac sodium sustained-release capsule in the treatment of acute gouty arthritis. Taking oral diclofenac sodium sustained release capsule as reference, this paper compares and evaluates and discusses the difference of clinical efficacy between the two methods, probes into the curative effect and advantages of Dianchonglou cataplasma, and tries to provide some decision-making references and suggestions for carrying forward the treasure of traditional Chinese medicine, excavating Yunnan Daodi medicinal materials and developing new in-hospital preparations. Methods: from December 2014 to December 2015, 53 cases of acute gouty arthritis were treated in Kunming Hospital of traditional Chinese Medicine from December 2014 to December 2015. 53 cases of acute gouty arthritis were classified according to odd date classification (odd date patients included in the control group and even number date patients into the treatment group). 27 patients in the control group and 26 patients in the treatment group were treated with diclofenac sodium sustained release capsule. The treatment group was treated with Dianzhonglou cataplasma, while the control group did not use any external drugs, only local cold compress. Through regular observation, patient reexamination and follow-up, the safety indexes such as joint redness and swelling, skin temperature, pain, limited function, blood uric acid, local pruritus, rash, liver function (ALT,AST) and renal function (Scr) were observed and recorded. The collected data were statistically analyzed to evaluate the clinical efficacy of two different treatment methods. Results: in the treatment of acute gouty arthritis, the control group and the treatment group could effectively improve the redness, swelling, heat pain and activity function in one course of treatment. In the acute stage of gouty arthritis, the two groups had no significant difference in reducing uric acid, but the improvement rate of the treatment group was significantly higher than that of the control group in the aspects of pain, redness and joint movement, the difference was statistically significant. In the aspect of improving joint skin temperature, the two groups were the same, and there was no significant difference in curative effect between the two groups. Conclusion: compared with oral diclofenac sodium sustained release capsule, external application of Dianzhonglou cataplasma combined with routine oral diclofenac sodium sustained release capsule has obvious improvement in reducing joint pain, redness and swelling and motor function, and no local and systemic side effects caused by external application are found, which is worthy of further clinical application.
