【摘要】：目的:針對腎虛血瘀型骨關節(jié)炎的病因病機特點,以補腎活血解毒為治法,觀察自擬骨痹湯治療骨關節(jié)炎的臨床療效和安全性,并對本方治療骨關節(jié)炎的作用機制進行初步探討。方法:收集2012年12月至2014年12月于日照市人民醫(yī)院風濕免疫科和五蓮縣中醫(yī)醫(yī)院內科接受診治的符合入組條件的骨關節(jié)炎患者病例100例,并隨機分成治療組和對照組,每組50例,兩組患者性別、年齡、病程均無顯著性差異。對照組給予硫酸氨基葡萄糖+雙氯芬酸鈉緩釋片治療,治療組在對照組的基礎上再給予自擬骨痹湯治療,4周為一療程。治療前對兩組患者進行病情程度評分和臨床指標量化評分,治療結束后,比較兩組患者病情程度、臨床指標及相關實驗室指標(C反應蛋白、血沉),評價骨痹湯治療骨關節(jié)炎的臨床療效。對治療組及對照組患者進行安全性評價。治療前后取血液標本,離心后取血清檢測MMP-3、TIMP-1的含量;取關節(jié)液,離心后取上清液檢測IL-1β、TNF-α的表達,對治療作用機制進行初步探討。結果:1、改善病情程度方面:治療組治療骨關節(jié)炎臨床總有效率為86.0%,明顯高于對照組(74.0%)。2、中醫(yī)癥狀:兩組治療主要中醫(yī)癥狀消失例(%)比較具有統(tǒng)計學意義上的差異(P0.05);且治療后兩組患者舌象與脈象的例數(shù)低于治療前,治療后舌象與脈象的例數(shù)與同期對照組比較也有顯著性差異(P0.05)。3、改善臨床指標方面:骨痹湯能顯著減輕骨關節(jié)炎患者主癥積分,改善患病關節(jié)疼痛、壓痛數(shù)、腫脹、活動度、晨僵時間、功能評分及行走評分。4、改善實驗室指標方面:骨痹湯能顯著降低患者血沉及C反應蛋白。5、治療作用機制研究結果:治療4周后,患者血清MMP-3、TIMP-1的含量明顯降低,關節(jié)液中的IL-1β、TNF-α含量也明顯降低,降低程度均高于對照組。骨痹湯能有效改善OA患者的關節(jié)脹痛,屈伸不利,膝腰酸軟,畏寒喜溫,關節(jié)壓痛,伴有耳鳴,頭暈目眩,關節(jié)脹痛、膝腰酸軟、關節(jié)壓痛等中醫(yī)癥狀。6、安全性評價結果:兩組患者主要不良反應為胃腸道反應反應以及肝功能異常,兩組的不良反應比較有顯著性差異(P0.05)。結論:1、骨痹湯可明顯改善骨關節(jié)炎患者的臨床癥狀及中醫(yī)證候,且無明顯不良反應,是一種安全有效的治療骨關節(jié)炎的方法,值得臨床推廣。2、骨痹湯能降低骨關節(jié)炎患者的血清MMP-3、TIMP-1的含量,降低關節(jié)液中的IL-1β、TNF-α含量,其作用機制可能與調控基質金屬蛋白酶和抑制炎癥反應有關,其他機制有待進一步探討。
[Abstract]:Objective: to observe the clinical efficacy and safety of self-made Gubi decoction in the treatment of osteoarthritis caused by kidney deficiency and blood stasis, and to explore the mechanism of the prescription in the treatment of osteoarthritis. Methods: from December 2012 to December 2014, 100 patients with osteoarthritis were randomly divided into treatment group and control group. There were no significant differences in sex, age and course of disease between the two groups. The control group was treated with glucosamine sulfate diclofenac sodium sustained-release tablets, and the treatment group was treated with self-made Gubi decoction on the basis of the control group, 4 weeks as a course of treatment. Before treatment, the severity score and clinical index quantitative score of the two groups were carried out. after the treatment, the severity, clinical index and related laboratory indexes (C-reactive protein, erythrocyte sedimentation rate) of the two groups were compared, and the clinical efficacy of Gubi decoction in the treatment of osteoarthritis was evaluated. The safety of the patients in the treatment group and the control group was evaluated. Before and after treatment, blood samples were taken, serum was taken to detect the content of MMP-3,TIMP-1, joint fluid was taken, and the expression of IL-1 尾 and TNF- 偽 was detected in the culture medium after centrifugation, and the therapeutic mechanism was discussed. Results: 1, to improve the degree of disease: the total effective rate of treatment group was 86.0%, which was significantly higher than that of the control group (74.0%). 2, TCM symptoms: there was significant difference between the two groups in the disappearance of the main TCM symptoms (%) (P 0.05), and the total effective rate of osteoarthritis in the treatment group was 86.0%, which was significantly higher than that in the control group (74.0%). After treatment, the number of cases of tongue and pulse in the two groups was lower than that before treatment, and the number of cases of tongue and pulse after treatment was also significantly different from that in the control group (P 0.05). 3, the improvement of clinical indexes: Gubi decoction could significantly reduce the main symptom score of patients with osteoarthritis, improve the number of pain, tenderness, swelling, range of motion, morning stiffness time, function score and walking score. 4, Improvement of laboratory indexes: Gubi decoction could significantly reduce erythrocyte sedimentation rate and C-reactive protein. Results: after 4 weeks of treatment, the content of serum MMP-3,TIMP-1 and the contents of IL-1 尾 and TNF- 偽 in synovial fluid were significantly decreased, and the degree of decrease was higher than that of the control group. Gubi decoction can effectively improve the symptoms of joint distension and pain, flexion and extension, weakness of knee and waist, fear of cold and warm, joint tenderness, tinnitus, dizziness, joint distension pain, knee waist weakness, joint tenderness and so on. 6, the results of safety evaluation: the main adverse reactions of the two groups were gastrointestinal reaction and abnormal liver function, and there was significant difference between the two groups (P 0.05). Conclusion: 1. Gubi decoction can obviously improve the clinical symptoms and TCM syndromes of patients with osteoarthritis, and has no obvious adverse reactions. It is a safe and effective method for the treatment of osteoarthritis, which is worthy of clinical promotion. 2, Gubi decoction can reduce the content of serum MMP-3,TIMP-1 and the content of IL-1 尾 and TNF- 偽 in synovial fluid of patients with osteoarthritis. Its mechanism may be related to the regulation of matrix metalloproteinases and the inhibition of inflammatory reaction, and other mechanisms need to be further discussed.
