【摘要】：目的采用前瞻性隨機(jī)對照試驗的方法,探討中藥膜劑外敷聯(lián)合2%氫醌對于減輕燒傷后色素沉著的有效性和安全性,為燒傷后治療色素沉著提供臨床參考。方法選擇在我院接受治療的燒傷后色素沉著患者60例作為研究對象,采用隨機(jī)分組方法將其分為A組和B組,每組30例。A組接受中藥膜劑外敷治療,每周2次,8次為一療程,進(jìn)行兩個療程治療,B組患者接受常規(guī)治療,使用千百乳膏外用,1天2次,自行應(yīng)用。治療前,采集患者面部局部色素斑圖像,用特定的圖像分析系統(tǒng)逐個分析圖像采集系統(tǒng)采集的色素斑圖像,獲取色素值；治療后第1、4、8周復(fù)診,獲取色素值；肉眼判斷觀察組和對照組治療前后患處的照片,觀察療效,同時比較每位患者治療前后圖像分析系統(tǒng)分析的色素斑色素值改變的百分比,以比較兩組患者治療總有效率。結(jié)果1兩組患者的年齡、性別、患病過程以及治療前色素值等指標(biāo)比較無顯著性差異(P0.05),具有可比性。2兩組患者治療后色素值顯著低于治療前(P0.05),提示燒傷后遺留的素色沉著可以自行消退。3治療后第1、4、8周A組患者色素沉著積分均顯著低于B組患者(P0.05),A組患者治療前后色素斑色素值改變的百分比顯著高于B組患者(P0.05)。同時A組患者色素消退時間顯著短于B組患者(P0.05),提示接受中藥膜劑外敷治療的效果更加明顯,能夠明顯縮短色素的遺留時間。4治療結(jié)束后,A組患者痊愈0例,顯效6例,有效20例,無效4例,治療總有效率為86.7%；B組痊愈0例,顯效2例,有效7例,無效21例,治療總有效率為30.0%,兩組比較差異顯著(P0.05)結(jié)論輕度燒傷后遺留的色素本身可以自然消退,但使用中藥膜劑外敷聯(lián)合2%氫琨治療能夠顯著縮短患者色素消退的時間,提高治療總有效率,治療效果明顯,同時不良反應(yīng)少,患者治療依從性高,值得臨床推廣應(yīng)用。
[Abstract]:Objective to investigate the efficacy and safety of external application of traditional Chinese medicine membrane combined with 2% hydroquinone in reducing pigmentation after burn by prospective randomized controlled trial, and to provide clinical reference for the treatment of pigmentation after burn. Methods Sixty patients with postburn pigmentation in our hospital were randomly divided into group A (n = 30) and group B (n = 30). Group A was treated with external application of traditional Chinese medicine membrane twice a week. 8 times as a course of treatment, two courses of treatment, group B patients received routine treatment, using thousands of cream for external use, twice a day, self-application. Before treatment, the local pigmentation images of the patients were collected, and the pigmented images collected by the image acquisition system were analyzed one by one with a specific image analysis system to obtain the pigmented values, and the pigmented values were obtained at the 1st and 4th weeks after treatment. The images of the patients in the observation group and the control group before and after treatment were judged by the naked eye, and the curative effect was observed. at the same time, the percentage of pigmented changes analyzed by the image analysis system before and after treatment was compared to compare the total effective rate between the two groups. Results 1 there was no significant difference in age, sex, disease process and color value before treatment between the two groups (P 0.05), and it was comparable. 2 the pigment value of the two groups after treatment was significantly lower than that before treatment (P 0.05). It is suggested that the pigmentation score of group A is significantly lower than that of group B at the 1st and 4th week after treatment (P 0.05), and the pigmentation score of group A is significantly lower than that of group B at the 1st and 4th week after treatment (P 0.05). The percentage of pigmented change in group A before and after treatment was significantly higher than that in group B (P 0.05). At the same time, the pigmentation time of group A was significantly shorter than that of group B (P 0.05), suggesting that the effect of external application of traditional Chinese medicine membrane was more obvious, and the residual time of pigment could be significantly shortened. 4 after treatment, 0 patients in group A were cured. 6 cases were markedly effective, 20 cases were effective and 4 cases were ineffective. The total effective rate was 86.7%. In group B, 0 cases were cured, 2 cases were markedly effective, 7 cases were effective and 21 cases were ineffective, the total effective rate was 30.0%. There was significant difference between the two groups (P 0.05). Conclusion the pigment left behind after mild burn can fade naturally. However, the external application of traditional Chinese medicine membrane combined with 2% hydrogen Kun can significantly shorten the time of pigment regression, improve the total effective rate of treatment, the treatment effect is obvious, at the same time, the adverse reactions are few, the patients' treatment compliance is high, and it is worth popularizing and applying in clinic.
