【摘要】：目的:評價健脾理氣法治療2型糖尿病胃輕癱的療效和安全性,并探討其可能作用機(jī)制,為健脾理氣法治療2型糖尿病胃輕癱提供循證依據(jù)。方法:1確定檢索關(guān)鍵詞,通過計(jì)算機(jī)對PubMed、Embase、The Cochrane Library等國外數(shù)據(jù)庫及萬方數(shù)據(jù)庫、中國知網(wǎng)等國內(nèi)數(shù)據(jù)庫2006年01月至2016年12月期間國內(nèi)外發(fā)表的健脾理氣法治療2型糖尿病胃輕癱的臨床研究文獻(xiàn)進(jìn)行檢索。2根據(jù)納入、排除標(biāo)準(zhǔn),選取符合納入條件的文獻(xiàn),使用RevMan 5.2軟件對健脾理氣法治療2型糖尿病胃輕癱的總有效率、痊愈率、胃排空率、糖代謝指標(biāo)、胃動素、不良反應(yīng)等指標(biāo)進(jìn)行Meta分析。結(jié)果:本研究共納入29篇符合納入條件的隨機(jī)對照試驗(yàn),結(jié)果顯示,健脾理氣法治療2型糖尿病胃輕癱在總有效率(RR=1.25,95%CI:[1.20,1.30],P0.05)、痊愈率(RR=1.83,95%CI:[1.44,2.33],P0.05)、胃排空率(MD=6.20,95%CI:[4.37,8.04],P0.05)、空腹血糖(MD=-0.69,95%CI:[-0.83,-0.54],P0.05)、餐后2h血糖(MD=-1.00,95%CI:[-1.27,-0.73],P0.05)、糖化血紅蛋白(MD=-1.04,95%CI:[-1.28,-0.80],P0.05)、胃動素(放射免疫法RIA:MD=-87.27,95%CI:[-135.92,-38.63],P0.05;酶聯(lián)免疫吸附法ELISA:MD=-62.94,95%CI:[-99.21,-26.67],P0.05)、不良反應(yīng)(RR=0.16,95%CI:[0.04,0.60],P0.05)方面優(yōu)于對照組,療效顯著。結(jié)論:1健脾理氣法治療2型糖尿病胃輕癱有顯著療效,可明顯提高胃排空率,療效優(yōu)于西藥對照組。2健脾理氣法可明顯改善患者糖代謝紊亂情況,降低患者胃動素,此可能為健脾理氣法治療2型糖尿病胃輕癱的作用機(jī)制之一。3健脾理氣法治療2型糖尿病胃輕癱有良好的安全性。4限于納入研究的文獻(xiàn)質(zhì)量不高,部分指標(biāo)樣本量少,尚需進(jìn)一步大樣本、多中心的雙盲隨機(jī)對照試驗(yàn)加以驗(yàn)證。
[Abstract]:Objective: to evaluate the efficacy and safety of invigorating spleen and regulating qi in the treatment of type 2 diabetic gastroparesis, and to explore its possible mechanism, so as to provide evidence basis for the treatment of type 2 diabetic gastroparesis by invigorating spleen and regulating qi. Methods: 1 the key words were determined, and the foreign databases such as PubMed,Embase,The Cochrane Library and Wanfang database were analyzed by computer. From January 2006 to December 2016, the domestic databases such as China knowledge Network published the literature of clinical research on the treatment of gastroparesis with type 2 diabetes mellitus by invigorating the spleen and regulating qi. 2 according to the inclusion and exclusion criteria, the literature eligible for inclusion was selected. The total effective rate, cure rate, gastric emptying rate, glucose metabolism index, motilin and adverse reactions of type 2 diabetic gastroparesis treated by invigorating spleen and regulating qi were analyzed by RevMan 5.2 software. Results: a total of 29 randomized controlled trials were included in this study. The results showed that the total effective rate of invigorating spleen and regulating qi in the treatment of type 2 diabetic gastroparesis (RR=1.25,95%CI: [1.20, 1.30], P05).
