【摘要】：目的觀察和評價(jià)安胃湯治療功能性消化不良寒熱錯(cuò)雜證的有效性及其對患者血漿胃動(dòng)素和血清胃泌素水平的影響,為安胃湯的臨床應(yīng)用和科研開發(fā)提供新的依據(jù)。方法選取2015年06月至2016年06月期間在廣西中醫(yī)藥大學(xué)附屬瑞康醫(yī)院消化內(nèi)科住院和門診就診被診斷為功能性消化不良寒熱錯(cuò)雜證的40例患者,按照隨機(jī)數(shù)字表法分為治療組和對照組,每組各20例,治療組予服用中藥安胃湯治療,對照組予口服多潘立酮片聯(lián)合復(fù)方阿嗪米特腸溶片治療,兩組患者均以2個(gè)月為1個(gè)療程,連續(xù)治療1個(gè)療程后,分別分析比較兩組患者的中醫(yī)證候積分、臨床療效及血漿胃動(dòng)素和血清胃泌素水平變化,并觀察藥物的安全性。結(jié)果兩組患者經(jīng)過1個(gè)療程治療后,治療組總有效率是85.00%,對照組總有效率為75.00%,兩組總有效率相比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者治療后胃脘痞滿、嘈雜、疼痛等主癥積分和納呆、噯氣等次癥積分與治療前相比均有所下降,以治療組最為顯著,兩組相比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05);治療組能夠顯著提高患者血漿胃動(dòng)素和血清胃泌素水平,與對照組相比較具有顯著性差異(P0.05);兩組患者在用藥期間均未出現(xiàn)不良反應(yīng),兩組相比較差異不具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 (1)安胃湯治療功能性消化不良寒熱錯(cuò)雜證臨床療效確切,安全性好,并且可以提高患者的血漿胃動(dòng)素和血清胃泌素水平;(2)安胃湯治療功能性消化不良寒熱錯(cuò)雜證的作用機(jī)制可能與影響患者的血漿胃動(dòng)素和血清胃泌素水平相關(guān)。
[Abstract]:Objective to observe and evaluate the efficacy of Anwei decoction in the treatment of functional dyspepsia with cold and heat disorder and its effect on plasma motilin and serum gastrin levels in order to provide a new basis for clinical application and scientific research of Anwei decoction. Methods from June 2015 to June 2016, 40 patients with functional dyspepsia, including cold and heat disorders, were diagnosed in the Department of Digestive Medicine, Ruikang Hospital affiliated to Guangxi University of traditional Chinese Medicine, from June 2015 to June 2016. According to the method of random number table, the treatment group was divided into treatment group (n = 20) and control group (n = 20). The treatment group was treated with Anwei decoction, and the control group was treated with oral domperidone tablet combined with compound azinomide enteric-coated tablet. Two months were used as a course of treatment. After one course of continuous treatment, the TCM syndromes scores, clinical efficacy, plasma motilin and serum gastrin levels were analyzed and compared between the two groups, and the safety of the drugs was observed. Results after one course of treatment, the total effective rate was 85.00 in the treatment group and 75.00 in the control group. The difference between the two groups was statistically significant (P0.05). After treatment, the scores of epigastric fullness, noise, pain, eructation and other major symptoms in the two groups were decreased, especially in the treatment group, and the difference was statistically significant (P0.05). The levels of plasma motilin and serum gastrin in the treatment group were significantly higher than those in the control group (P0.05). There were no adverse reactions between the two groups during the period of medication, the difference between the two groups was not statistically significant (P0.05). Conclusion (1) Anwei decoction is effective and safe in the treatment of patients with functional dyspepsia, cold and heat disorder, and can improve plasma motilin and serum gastrin levels. (2) the mechanism of Anwei decoction in treating functional dyspepsia with cold and heat disorder may be related to plasma motilin and serum gastrin level.
