黃白合劑治療肛裂術后創(chuàng)面愈合臨床觀察

【摘要】:目的:探討濕熱下注型肛裂患者術后,使用黃白合劑坐浴熏洗外用治療,了解黃白合劑對創(chuàng)口疼痛、創(chuàng)面分泌物的PH值、創(chuàng)面面積及創(chuàng)面愈合情況等方面的影響。方法:按照隨機數(shù)表法,將符合納入標準的120例患者,分為治療組和對照組各60例。兩組在手術前后采用相同的常規(guī)治療基礎上,治療組于術后第2天予院內(nèi)制劑黃白合劑坐浴熏洗,每日1劑,早晚各坐浴1次,連續(xù)使用7天;對照組予高錳酸鉀溶液熏洗坐浴。觀察兩組患者術后第2、7、14天創(chuàng)口疼痛、創(chuàng)面分泌物的PH值等情況進行評分并記錄數(shù)據(jù)。在術后第7、14、21天記錄創(chuàng)面愈合面積,比較創(chuàng)面愈合時間以及術后第21天兩組的總療效。結果:入選的120例患者,收集時無病例脫落。治療前將兩組病例的性別、年齡、病程、創(chuàng)面疼痛、創(chuàng)面分泌物的PH值等進行比較,差異均無統(tǒng)計學意義,具有可比性。研究結果分析如下:(1)創(chuàng)面疼痛:兩組術后第7、14天創(chuàng)面疼痛情況比較均呈顯著性差異(P0.05),黃白合劑組明顯優(yōu)于高錳酸鉀組,兩組術后第2天治療前創(chuàng)面疼痛比較均無顯著性差異(P0.05)。(2)創(chuàng)面分泌物的PH值:兩組術后第7、14天創(chuàng)面疼痛情況比較均呈顯著性差異(P0.05),治療組明顯優(yōu)于對照組,兩組術后第2天治療前創(chuàng)面疼痛比較均無顯著性差異(P0.05)。(3)創(chuàng)面面積大小、創(chuàng)面愈合時間和總療效:兩組術后第7、14、21天創(chuàng)面的面積情況對比存在差異(P0.05),治療組優(yōu)于對照組。治療組平均創(chuàng)面愈合時間(14.65±1.298)天,對照組平均創(chuàng)面愈合時間(17.50±1.374)天,創(chuàng)面愈合時間對比存在顯著性差異(P0.05),治療組優(yōu)于對照組。術后第21天,治療組總療效為100%,對照組總療效為95%,兩者對比無顯著性差異(PO.05)。結論:黃白合劑可以緩解濕熱下注型肛裂患者術后創(chuàng)面疼痛,.可能與肛門局部抑菌消炎抗感染有關;黃白合劑可以降低創(chuàng)面分泌物的PH值,可能與微酸環(huán)境更益于細胞的生長有關;黃白合劑可以縮短創(chuàng)面上皮化的時間,可能與促進創(chuàng)面的肉芽組織生長有關;黃白合劑可以改善創(chuàng)面局部血供,調節(jié)機體免疫實現(xiàn)并加強組織的修復機能,進而促進創(chuàng)面愈合。且用藥過程中未出現(xiàn)不良反應,是一種有效安全的中藥湯劑,臨床值得推廣。
[Abstract]:Objective: to investigate the effect of Huangbai mixture on wound pain, PH value of wound secretion, wound area and wound healing. Methods: according to the method of random number, 120 patients were divided into treatment group (n = 60) and control group (n = 60). The two groups were treated with the same routine treatment before and after operation. The treatment group was treated with Huangbai mixture in the hospital on the second day after operation, one dose daily, one sitting bath in the morning and evening for 7 days. The control group was treated with potassium permanganate solution fumigation and sitting bath. The wound pain and PH value of wound secretion were evaluated and recorded on the 2nd day and 14th day after operation in both groups. The wound healing area was recorded on day 7, 14 and 21 after operation, and the time of wound healing and the total curative effect of the two groups were compared. Results: none of the 120 patients were abscission at the time of collection. Before treatment, the sex, age, course of disease, wound pain, PH value of wound secretion of the two groups were compared, the difference was not statistically significant and comparable. The results were as follows: (1) wound pain: there was significant difference between the two groups on the 14th day after operation (P0.05), Huangbai mixture group was significantly better than potassium permanganate group. There was no significant difference in wound pain between the two groups before treatment on the second day after operation (P0.05). (2). The PH value of wound secretion was significantly different between the two groups on the 7th and 14th day after operation (P0.05), and there was no significant difference between the two groups before treatment (P0.05). The treatment group was obviously superior to the control group. There was no significant difference in wound pain between the two groups before treatment on the second day after operation (P0.05). (3), and there was no significant difference in wound area between the two groups before treatment (P0.05). Wound healing time and total curative effect: there were differences in the area of wound between the two groups on the 7th day and the 14th day after operation (P0.05), the treatment group was better than the control group. The average wound healing time was (14.65 鹵1.298) days in the treatment group and (17.50 鹵1.374) days in the control group. There was significant difference in the wound healing time between the treatment group and the control group (P0.05). On the 21st day after operation, the total curative effect of the treatment group was 100 and that of the control group was 95. There was no significant difference between the two groups (PO.05). Conclusion: Huangbai mixture can relieve postoperative wound pain in patients with anal fissure caused by dampness and heat. Huangbai mixture can reduce the PH value of wound secretion and may be related to the microacid environment which is more beneficial to cell growth. Huangbai mixture can shorten the time of wound epithelization, which may be related to promoting the growth of granulation tissue, and Huangbai mixture can improve the local blood supply of wound, regulate the realization of body immunity and strengthen the repair function of tissue, and then promote wound healing. It is an effective and safe traditional Chinese medicine decoction, which is worth popularizing.
