【摘要】：目的:本研究以中重度原發(fā)性痛經(jīng)患者為研究對象,以隔藥灸臍法為治療方法,西藥布洛芬膠囊為對照,觀察隔藥灸臍法治療原發(fā)性痛經(jīng)的臨床療效,并評價二者的療效差異,為隔藥灸臍法的臨床推廣提供證據(jù)。方法:將符合納入標(biāo)準(zhǔn)的66例原發(fā)性痛經(jīng)患者采用中醫(yī)癥狀積分表分為中度和重度,采用隨機(jī)、雙盲雙模擬、陽性對照的研究方法,將患者分為治療組隔藥灸臍+西藥模擬劑膠囊和對照組隔模擬劑灸臍+布洛芬膠囊,以中醫(yī)癥狀評分為觀察指標(biāo),觀察治療前后及隨訪期兩組中醫(yī)癥狀的改善情況,并比較兩組差異。結(jié)果:1.隨著治療周期的延長,治療組原發(fā)性痛經(jīng)患者中醫(yī)癥狀逐步改善,尤其是疼痛程度、疼痛持續(xù)時間、有血塊、畏寒或手足欠溫、經(jīng)血量少方面療效顯著,且遠(yuǎn)期效果好;對照組對原發(fā)性痛經(jīng)患者疼痛程度、疼痛持續(xù)時間、乳房脹痛、腰部酸痛即時效果明顯,但遠(yuǎn)期效果差。2.對照組中度原發(fā)性痛經(jīng)患者在第一周期腰酸癥狀改善較治療組明顯;第二周期兩組無明顯差異;第三周期和隨訪期,治療組在疼痛程度、疼痛持續(xù)時間、經(jīng)血量少、畏寒方面的改善明顯優(yōu)于對照組。3.在第一、二、三周期,兩組重度原發(fā)性痛經(jīng)患者的疼痛程度和持續(xù)時間均隨治療周期延長而改善,在隨訪期治療組患者疼痛程度和持續(xù)時間較治療前持續(xù)降低,對照組疼痛程度和持續(xù)時間較一、二、三周期明顯升高。結(jié)論:隔藥灸臍法改善中重度原發(fā)性痛經(jīng)患者中醫(yī)癥狀療效優(yōu)于布洛芬膠囊,且遠(yuǎn)期療效顯著。
[Abstract]:Objective: to observe the clinical effect of moxibustion on primary dysmenorrhea and evaluate the difference between them. To provide evidence for clinical popularization of moxibustion with separated medicine. Methods: 66 patients with primary dysmenorrhea who met the inclusion criteria were divided into moderate and severe by TCM symptom score table. The patients were divided into two groups: the treatment group was treated with moxibustion omphalicum analogue capsule and the control group was treated with moxibustion ibuprofen capsule. The improvement of TCM symptoms was observed before and after treatment and during the follow-up period. The differences between the two groups were compared. Results: 1. With the prolongation of the treatment cycle, the TCM symptoms of primary dysmenorrhea patients in the treatment group were gradually improved, especially the degree of pain, duration of pain, blood clot, chills or undertemperature of the hand and foot, less menstrual blood volume, and good long-term effect. In the control group, the pain degree, duration of pain, breast pain, waist pain were obvious, but the long-term effect was poor. In the control group, the improvement of lumbar acid symptoms in the first cycle was more obvious than that in the treatment group, but there was no significant difference between the two groups in the second cycle. In the third cycle and follow-up period, the improvement in pain degree, pain duration, menstrual blood volume and chills in the treatment group was significantly better than that in the control group. In the first, second, and third cycles, the pain degree and duration of severe primary dysmenorrhea patients in both groups improved with the prolongation of the treatment cycle, and the pain degree and duration of the patients in the treatment group continued to decrease during the follow-up period compared with those before treatment. The pain degree and duration in the control group were significantly higher than that in the first, second and third cycles. Conclusion: the curative effect of herbal moxibustion on the symptoms of moderate and severe primary dysmenorrhea is better than ibuprofen capsule, and the long-term curative effect is remarkable.
