【摘要】：目的探討中藥灌腸聯(lián)合艾灸治療盆腔瘀血綜合征(PCS)的臨床療效。方法 140例患者分為對照組和聯(lián)合組。對照組采用中藥灌腸,聯(lián)合組采用中藥灌腸聯(lián)合艾灸。均于月經(jīng)干凈5d后開始施治,連續(xù)至下次月經(jīng)。連續(xù)治療3個(gè)月經(jīng)周期,并進(jìn)行3個(gè)月經(jīng)周期的隨訪。采用疼痛視覺模擬評分(VAS)記錄疼痛程度,測量盆腔靜脈暗帶內(nèi)徑和靜脈血流速度;進(jìn)行下腹脹痛、刺痛或墜痛,腰骼疼痛,性交痛,痛經(jīng),白帶多等癥狀評分,檢測血液流變學(xué)指標(biāo);記錄復(fù)發(fā)情況和進(jìn)行安全性評價(jià)。結(jié)果聯(lián)合組綜合臨床療效總有效率為96.97%,高于對照組的84.85%(χ~2=5.867,P0.05);治療后聯(lián)合組VAS評分、下腹脹痛、刺痛或墜痛、腰骼疼痛、性交痛、痛經(jīng)、白帶多等評分均低于對照組(P0.01);治療后聯(lián)合組盆腔靜脈暗帶內(nèi)徑小于對照組,靜脈血流速度快于對照組(P0.01);治療后聯(lián)合組全血黏度(低切、高切)、血漿黏度、纖維蛋白原和全血還原黏度均低于對照組(P0.01);在3個(gè)月經(jīng)周期的隨訪中,聯(lián)合組復(fù)發(fā)率為28.79%,低于對照組的46.97%(χ~2=4.637,P0.05);未發(fā)現(xiàn)與中藥灌腸或艾灸相關(guān)不良反應(yīng)。結(jié)論中藥灌腸聯(lián)合艾灸外治PCS患者,臨床安全、有效,是值得采用的中醫(yī)保守療法。
[Abstract]:Objective to investigate the clinical effect of traditional Chinese medicine enema combined with moxibustion on pelvic blood stasis syndrome (PCS). Methods 140 patients were divided into control group and combined group. The control group was treated with traditional Chinese medicine enema, and the combined group with Chinese medicine enema combined with moxibustion. All menstruation clean 5 days after the beginning of treatment, continuous to the next menstruation. Three menstrual cycles were continuously treated and followed up for 3 menstrual cycles. The pain degree was recorded by visual analogue score (VAS) and the diameter of the dark zone of pelvic vein and the velocity of venous blood flow were measured. The symptoms of lower abdominal distending pain, stinging pain or falling pain, waist and iliac pain, intercourse pain, dysmenorrhea, leucorrhea were evaluated, hemorheological indexes were detected, recurrence was recorded and safety was evaluated. Results the total effective rate of the combined group was 96.97, which was higher than that of the control group (84.85%) (蠂 ~ 2 ~ 5.867%, P0.05). After treatment, the scores of VAS, abdominal distending pain, stinging pain, iliac pain, dysmenorrhea, dysmenorrhea and leucorrhea in the combined group were lower than those in the control group (P0.01). After treatment, the inner diameter of the pelvic vein dark zone in the combined group was smaller than that in the control group, and the velocity of venous blood flow was faster than that in the control group (P0.01). After treatment, the whole blood viscosity (low shear, high shear), plasma viscosity, fibrinogen and whole blood reduction viscosity in the combined group were lower than those in the control group (P0.01). In the follow-up of three menstrual cycles, the recurrence rate in the combined group was 28.79, which was lower than that in the control group (46.97%) (蠂 ~ 2, 4.637p. 05), and no adverse reactions related to traditional Chinese medicine enema or moxibustion were found. Conclusion traditional Chinese medicine enema combined with moxibustion is a safe and effective therapy for PCS patients.
