【摘要】：目的:探討溫經(jīng)通絡(luò)湯結(jié)合針灸治療風(fēng)寒濕痹型關(guān)節(jié)炎的療效及對(duì)患者血清分泌型糖蛋白(Wnt)-3α,β-鏈蛋白(β-catenin)及骨形成蛋白-2(bone morphogenetic proteins-2,BMP-2)表達(dá)的影響。方法:選自天津中醫(yī)藥大學(xué)第一附屬醫(yī)院2015年2月—2016年11月收治的112例風(fēng)寒濕痹型類風(fēng)濕性關(guān)節(jié)炎(RA)患者,按照隨機(jī)數(shù)字表將其分成觀察組與對(duì)照組,每組56例。觀察組給予溫經(jīng)通絡(luò)湯聯(lián)合針灸治療,對(duì)照組給予塞來(lái)昔布聯(lián)合來(lái)氟米特治療,比較兩組臨床療效、治療前后中醫(yī)臨床證候積分、紅細(xì)胞沉降率(erythrocyte sedimentation rate,ESR),C反應(yīng)蛋白(C-reaction protein,CRP)及類風(fēng)濕因子(rheumatoid factor,RF)變化情況以及血清Wnt-3α,β-catenin及BMP-2水平變化情況。結(jié)果:治療后,觀察組總有效率為94.64%,對(duì)照組總有效率為74.00%,觀察組優(yōu)于對(duì)照組(P0.01)。兩組治療后中醫(yī)臨床證候積分均較治療前顯著下降(P0.01),與對(duì)照組治療后比較,觀察組降低更顯著(P0.01)。兩組治療后ESR,CRP,RF水平均較治療前顯著下降(P0.01),與對(duì)照組治療后比較,觀察組下降更顯著(P0.05)。與治療前比較,觀察組治療后血清Wnt-3α,β-catenin及BMP-2水平均顯著下降(P0.01),對(duì)照組治療前后比較無(wú)統(tǒng)計(jì)學(xué)差異。結(jié)論:溫經(jīng)通絡(luò)湯結(jié)合針灸治療風(fēng)寒濕痹型關(guān)節(jié)炎療效顯著,其可下調(diào)Wnt-3α,β-catenin,BMP-2表達(dá),緩解關(guān)節(jié)疼痛及改善關(guān)節(jié)功能。
[Abstract]:Objective: to investigate the therapeutic effect of Wenjing Tongluo decoction (WJT) combined with acupuncture and moxibustion on rheumatic dampness arthritic arthritis and its effect on serum secretory glycoprotein (Wnt) 3 偽, 尾 -chain protein (尾 catenin) and bone morphogenetic protein-2 (bone morphogenetic proteins-2, (BMP 2 (bone morphogenetic proteins-2,). BMP-2) expression. Methods: one hundred and twelve patients with rheumatoid arthritis (RA) were selected from the first affiliated Hospital of Tianjin University of traditional Chinese Medicine from February 2015 to November 2016. They were divided into observation group and control group according to the random number table, 56 cases in each group. The observation group was treated with Wenjing Tongluo decoction combined with acupuncture and moxibustion, while the control group was treated with celecoxib combined with leflunomide. C-reactive protein (C-reaction protein,CRP), rheumatoid factor (rheumatoid factor,RF) and serum Wnt-3 偽, 尾-catenin and BMP-2 levels were also changed. Results: after treatment, the total effective rate of the observation group was 94.64 and that of the control group was 74.00. The observation group was superior to the control group (P0.01). After treatment, the scores of TCM syndromes in both groups were significantly lower than those before treatment (P0.01). Compared with the control group, the scores of TCM syndromes in the observation group were significantly lower than those in the control group (P0.01). After treatment, ESR,CRP,RF levels in both groups were significantly lower than those before treatment (P0.01), compared with the control group after treatment, the observation group decreased more significantly (P0.05). The levels of serum Wnt-3 偽, 尾-catenin and BMP-2 in the observation group were significantly lower than those before and after treatment (P0.01), but there was no significant difference before and after treatment in the control group. Conclusion: Wenjing Tongluo decoction combined with acupuncture and moxibustion is effective in treating rheumatic dampness arthritic arthritis. It can down-regulate the expression of Wnt-3 偽, 尾-catenin,BMP-2, relieve joint pain and improve joint function.
