當(dāng)歸芍藥散加減聯(lián)合兒黃散膜劑治療宮頸HPV感染的臨床觀察

【摘要】：目的:觀察當(dāng)歸芍藥散加減聯(lián)合兒黃散膜劑治療宮頸HPV感染的臨床療效,為臨床治療宮頸HPV感染提供方法和思路。方法:本課題收集就診于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院婦科門診自2015年10月至2016年10月期間符合診斷標(biāo)準(zhǔn)的60例宮頸HPV感染的患者,將這些患者隨機(jī)分為治療組和對照組。治療組30例,予中藥湯劑當(dāng)歸芍藥散加減口服聯(lián)合兒黃散膜劑陰道置藥,3個(gè)月經(jīng)周期(一個(gè)療程);對照組30例,予辛復(fù)寧陰道置藥,3個(gè)月經(jīng)周期(一個(gè)療程)。觀察兩組病例中醫(yī)臨床證候積分的改變、HPVDNA病毒載量的改變,并通過治療前后自身對比和兩組間的對照對其進(jìn)行療效分析。結(jié)果:1、中藥湯劑當(dāng)歸芍藥散加減聯(lián)合兒黃散膜劑能明顯改善患者中醫(yī)臨床癥候積分(p0.01),總有效率為90%,共30例,痊愈率36.7%,顯效率43.3%,有效3%,無效3%,優(yōu)于辛復(fù)寧的對照組:總有效率為56.7%,痊愈率6.7%,顯效率40%,有效率3%,無效率43.3%。2、中藥湯劑當(dāng)歸芍藥散加減聯(lián)合兒黃散膜劑在降低患者HPVDNA病毒載量方面有顯著統(tǒng)計(jì)學(xué)差異(P0.01),明顯優(yōu)于對照組。3、治療后治療組30例中有20例轉(zhuǎn)陰,轉(zhuǎn)陰率為66.7%,對照組30例中12例轉(zhuǎn)陰,HPV轉(zhuǎn)陰率為40%,兩組HPV轉(zhuǎn)陰率的比較有顯著統(tǒng)計(jì)學(xué)差異(P0.05)。4、治療后對照組總有效率56.7%;治療組總有效率90.0%。兩組總有效率的比較有顯著統(tǒng)計(jì)學(xué)差異(P0.01)。結(jié)論:1、當(dāng)歸芍藥散加減聯(lián)合兒黃散膜劑治療宮頸HPV感染,能明顯改善患者的中醫(yī)臨床癥候,降低HPVDNA病毒載量,提高HPV的轉(zhuǎn)陰率,臨床療效確切,且安全可靠,值得推廣應(yīng)用。
[Abstract]:Objective: to observe the clinical effect of Danggui Shaoyao Powder (Danggui Shaoyao Powder) combined with er Huang San (er Huang San) in the treatment of cervical HPV infection, and to provide methods and ideas for clinical treatment of cervix HPV infection. Methods: from October 2015 to October 2016, 60 patients with cervical HPV infection in gynecological outpatient department of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine were collected and randomly divided into treatment group and control group. The treatment group (30 cases) was treated with traditional Chinese medicine decoction Danggui Shaoyao Powder plus or minus orally combined with er Huang San film vaginal placement for 3 menstrual cycles (one course of treatment) while the control group (30 cases) were given Xinfuning vaginal insertion medicine for 3 menstrual cycles (one course of treatment). The changes of TCM syndromes integral and HPVDNA viral load were observed in the two groups, and the therapeutic effects were analyzed by self-comparison before and after treatment and control between the two groups. Results: 1. Traditional Chinese medicine decoction Danggui Shaoyao Powder plus or minus combined with er Huang San could obviously improve the clinical symptom score of TCM (p0.01), the total effective rate was 90 cases (30 cases), the cure rate was 36.7%, the effective rate was 43.3%, and the effective rate was 33.3%. The total effective rate was 56.7, the recovery rate was 6.7, the effective rate was 40, the effective rate was 3percent, and the inefficiency was 43.33.2, the total effective rate was 56.7, the recovery rate was 6.7 percent, the effective rate was 40, the effective rate was 3percent, and the inefficiency was 43.3percent. Traditional Chinese medicine decoction Danggui Shaoyao Powder plus or minus combined with er Huang San film had significant difference in reducing the viral load of HPVDNA in patients (P0.01), which was obviously superior to the control group. 3. After treatment, 20 of 30 cases in the treatment group turned negative. The negative rate was 66.7%, 12 cases turned negative and 40% HPV turned negative in 30 cases in the control group. There was significant difference between the two groups in the negative conversion rate of HPV (P0.05). 4. The total effective rate of the control group was 56.7% after treatment. The total effective rate of the treatment group was 90.0%. There was a significant difference in the total effective rate between the two groups (P 0.01). Conclusion: 1.Danggui Shaoyao Powder plus or minus combined with er Huang San in the treatment of cervical HPV infection can significantly improve the clinical symptoms of patients with HPVDNA, reduce the load of HPVDNA virus, increase the negative rate of HPV, the clinical efficacy is accurate, and safe and reliable. It is worth popularizing and applying.
