【摘要】：目的:探討成人斯蒂爾病(AOSD)的中醫(yī)證型分布與患者一般情況、服用糖皮質(zhì)激素時(shí)間及C-反應(yīng)蛋白(CRP)、血清鐵蛋白(SF)的相關(guān)性。方法:回顧性納入2011年1月至2015年12月住院確診的AOSD患者108例,頻數(shù)表描述證型分布,SPSS19.0統(tǒng)計(jì)分析各影響因素與證型相關(guān)性。結(jié)果:108例AOSD患者67.59%在春夏發(fā)病,出現(xiàn)頻率大于50%的癥狀有發(fā)熱、皮疹、咽痛、關(guān)節(jié)痛、肌痛、瘰疬腫痛、乏力,證型分布為氣陰兩虛兼濕熱痹阻證(25.93%)、濕熱痹阻證(21.30%)、陰虛內(nèi)熱證(17.59%)、氣營(yíng)兩燔證(15.74%)、氣陰兩虛證(10.19%)、衛(wèi)氣同病證(9.26%);病程及服用激素時(shí)間長(zhǎng)短、CRP、SF均影響證型分布。結(jié)論:AOSD多見(jiàn)中青年女性,典型癥狀為發(fā)熱、皮疹、咽痛、關(guān)節(jié)痛,中醫(yī)證型以虛實(shí)夾雜的復(fù)合證型氣陰兩虛兼濕熱痹阻證最常見(jiàn),衛(wèi)氣同病證最少見(jiàn),病程、服用激素時(shí)間、CRP、SF與證型有明顯相關(guān)性,即病程及服用激素時(shí)間越長(zhǎng)表現(xiàn)為虛證越明顯,CRP、SF越高病程越短則實(shí)證越明顯。
[Abstract]:Objective: to investigate the relationship between the distribution of TCM syndromes of adult Steele disease (AOSD) and the general condition of patients, the time of taking glucocorticoid and the serum ferritin (SF) (SF) of C-reactive protein (CRP),). Methods: 108 AOSD patients who were hospitalized from January 2011 to December 2015 were retrospectively included. The distribution of syndromes was described by frequency table, and the correlation between the influencing factors and syndrome type was analyzed statistically by SPSS19.0. Results: 67.59% of the patients with AOSD developed in spring and summer. The symptoms with frequency greater than 50% were fever, rash, pharynx, arthralgia, myalgia, scrofula swelling and fatigue. The syndrome type was deficiency of qi and yin and dampness and heat obstruction (25.93%). Damp-heat obstruction syndrome (21.30%), Yin deficiency internal heat syndrome (17.59%), Qi Ying two burnt syndrome (15.74%), qi and yin deficiency syndrome (10.19%), Wei Qi syndromes (9.26%); Course of disease, duration of taking hormone and CRP,SF all affected the distribution of syndromes. Conclusion: the typical symptoms of AOSD are fever, rash, pharynx, arthralgia. The syndrome of deficiency and deficiency of qi and yin combined with damp-heat obstruction is the most common type of TCM syndrome, the disease of Wei Qi is the rarest, the course of disease, the time of taking hormone. There was a significant correlation between CRP,SF and syndromes, that is, the longer the course of disease and the time of taking hormone, the more obvious the deficiency syndrome, the shorter the course of CRP,SF, the more obvious the evidence was.
