【摘要】：目的:本臨床研究為進(jìn)一步探討補(bǔ)腎活血方對(duì)不明原因復(fù)發(fā)性流產(chǎn)(unexplain recurrent spontaneous abortion,URSA)的調(diào)節(jié)機(jī)制及臨床療效,選取白細(xì)胞介素 13(interleukin-13,IL-13),白細(xì)胞介素 15(interleukin-15,IL-15)及白細(xì)胞介素35(interleukin-35,IL-35)三個(gè)因子,通過觀察以上三個(gè)因子的受體IL-13Rα1、IL-15Rα及IL-12Rβ2、gp130在URSA患者及正常早孕婦女蛻膜組織中的表達(dá),以及臨床60例URSA腎虛血瘀證患者服用補(bǔ)腎活血方前后外周血中IL-13、IL-15和IL-35含量變化和用藥后療效分析,探討補(bǔ)腎活血方對(duì)IL-13、IL-15、IL-35的調(diào)節(jié)機(jī)制,探索IL-13、IL-15及IL-35與URSA的相關(guān)性,從而為進(jìn)一步研究補(bǔ)腎活血方對(duì)URSA的臨床療效提供理論依據(jù)。方法:本實(shí)驗(yàn)研究共分為兩部分,第一部分收集2016年1月-2017年3月來黑龍江中醫(yī)藥大學(xué)附屬醫(yī)院婦科、產(chǎn)科門診及住院妊娠6-10周的15例不明原因復(fù)發(fā)性流產(chǎn)妊娠失敗患者的蛻膜組織作為研究組(URSA組),選取妊娠6-10周的15例正常妊娠行人工流產(chǎn)婦女的蛻膜組織作為對(duì)照組;第二部分收集60例URSA妊娠患者用藥前采集外周血,分為30例服用補(bǔ)腎活血方+地屈孕酮實(shí)驗(yàn)組,及30例服用地屈孕酮對(duì)照組,分別采集兩組用藥前及用藥四周后的外周血。采用RT-PCR方法檢測(cè)15例URSA患者和15例正常早孕婦女蛻膜組織中IL-13受體、IL-15受體及IL-35受體的表達(dá)差異;采用ELISA法測(cè)定外周血中IL-13、IL-15、IL-35水平,并監(jiān)測(cè)兩組患者用藥前后血清孕激素、雌激素和β-HCG的水平變化。結(jié)果:(1)在URSA患者蛻膜組織中IL-13Rα1和gp130的表達(dá)明顯低于正常人流組,而IL-15Rα和IL-12Rβ2的表達(dá)顯著高于正常人流組,兩組比較均具有統(tǒng)計(jì)學(xué)意義(P0.05)。(2)IL-13在URSA實(shí)驗(yàn)組患者的外周血中用藥后含量明顯高于用藥前;IL-13在對(duì)照組患者的外周血中含量用藥后明顯高于用藥前;IL-13在URSA實(shí)驗(yàn)組用藥后外周血含量高于對(duì)照組用藥后,以上均有統(tǒng)計(jì)學(xué)差異(P0.01)。(3)IL-15在URSA實(shí)驗(yàn)組患者的外周血中用藥后含量明顯低于用藥;IL-15在對(duì)照組患者的外周血中含量用藥后明顯低于用藥前;IL-15在URSA實(shí)驗(yàn)組用藥后外周血含量低于對(duì)照組用藥后,以上有統(tǒng)計(jì)學(xué)差異(P0.01)。(4)IL-35在URSA實(shí)驗(yàn)組患者的外周血中用藥后含量明顯高于用藥前;IL-35在對(duì)照組患者的外周血中含量用藥后明顯高于用藥前;IL-35在URSA實(shí)驗(yàn)組用藥后外周血含量高于對(duì)照組用藥后,有統(tǒng)計(jì)學(xué)差異(P0.01)。(5)補(bǔ)腎活血方+地屈孕酮實(shí)驗(yàn)組治療后外周血中孕酮、雌二醇、血β-HCG的含量明顯高于治療前,兩組比較具有明顯統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論:(1)URSA患者蛻膜組織中IL-13Rα1和gp130的表達(dá)相對(duì)于正常妊娠蛻膜組織減少,而IL-15Rα和IL-12Rβ2的表達(dá)增多,表明IL-13、IL-15、IL-35細(xì)胞因子在URSA患者和正常早孕患者蛻膜組織中發(fā)揮作用有差異,提示IL-13、IL-15、IL-35可能參與了 URSA的發(fā)病過程。(2)IL-13、IL-35在URSA實(shí)驗(yàn)組和對(duì)照組患者的外周血中用藥后含量均明顯高于用藥前,說明IL-13、IL-35水平過低可能影響母胎免疫平衡,從而導(dǎo)致流產(chǎn);IL-15的含量在URSA實(shí)驗(yàn)組和對(duì)照組患者的外周血中用藥后含量均明顯低于用藥前,說明適量的IL-15有利于維持妊娠,濃度過高則不利于妊娠,這可能是流產(chǎn)的原因之一。(3)補(bǔ)腎活血方可能通過適當(dāng)提高IL-13、IL-35在URSA實(shí)驗(yàn)組外周血中因子水平,以及降低IL-15在URSA實(shí)驗(yàn)組外周血中因子水平來改善不明原因的復(fù)發(fā)性流產(chǎn)患者的臨床療效。(4)補(bǔ)腎活血方能夠明顯提高URSA患者外周血中孕酮、雌二醇、血β-HCG的含量,以維持妊娠。
[Abstract]:Objective: To study the regulation mechanism and clinical effect of Bushen Huoxue recipe for unexplained recurrent spontaneous abortion (URSA), and to select the three factors of interleukin-13 (IL-13), interleukin-15 (IL-15) and interleukin-35 (IL-35). The expression of IL-13R-1, IL-15R-1, IL-12R-2 and gp130 in the decidua tissues of the patients with URSA and normal early pregnancy were observed by observing the expression of IL-13R-1, IL-15R-1, IL-12R-2 and gp130 in the decidua tissues of the patients with URSA and normal early pregnancy. To study the effects of Bushen Huoxue Recipe on IL-13, IL-15 and IL-35, the relationship between IL-13, IL-15 and IL-35 and URSA was explored. Methods: The experimental study was divided into two parts. The first part collected 15 cases of non-unexplained recurrent spontaneous abortion with unexplained recurrent spontaneous abortion during the period from January to March 2017 in the Affiliated Hospital of Heilongjiang University of Traditional Medicine, Obstetrics and Gynecology and in the hospital for 6-10 weeks as the study group (URSA group). In the second part, 60 patients with URSA were collected, and the peripheral blood was collected and divided into 30 cases, the experimental group with the Bushen Huoxue and the dydrogesterone group, and the control group of the dydrogesterone in 30 cases. The peripheral blood was collected before and after the administration of the two groups. The expression of IL-13, IL-15 and IL-35 in 15 patients with URSA and 15 normal early pregnancy women were detected by RT-PCR, and the levels of IL-13, IL-15 and IL-35 in peripheral blood were determined by ELISA. The level of estrogen and HCO3-HCG was changed. Results: (1) The expression of IL-13R-1 and gp130 in decidua tissues of URSA was significantly lower than that of normal people, while the expression of IL-15R-1 and IL-12R-1 was significantly higher than that of normal control group (P <0.05). (2) The content of IL-13 in the peripheral blood of the URSA experimental group was significantly higher than that of the control group. The content of IL-13 in the peripheral blood of the control group was significantly higher than that of the control group. The content of IL-13 in the peripheral blood of the control group was higher than that of the control group (P0.01). (3) The content of IL-15 in the peripheral blood of the patients with URSA was significantly lower than that in the control group. The content of IL-15 in the peripheral blood of the control group was significantly lower than that of the control group. The content of IL-15 in the peripheral blood of the control group was lower than that of the control group (P0.01). (4) The content of IL-35 in the peripheral blood of the URSA experimental group was significantly higher than that of the control group, and the level of IL-35 in the peripheral blood of the control group was significantly higher than that of the control group (P0.01). (5) The content of progesterone, estradiol and blood-HCG in the peripheral blood of the experimental group of the Bushen Huoxue and the dydrogesterone group was significantly higher than that before treatment (P0.01). Conclusion: (1) The expression of IL-13R-1 and gp130 in the decidua tissue of the patients with URSA is decreased compared with that of the normal pregnancy and the expression of IL-15R-1 and IL-12R-2 is increased, indicating that the IL-13, IL-15 and IL-35 cytokines play a role in the decidua tissues of the patients with URSA and the normal early pregnancy, and it is suggested that IL-13 and IL-15, IL-35 may be involved in the pathogenesis of the URSA. (2) The content of IL-13 and IL-35 in the peripheral blood of both the URSA and the control group was significantly higher than that of the control group. The content of IL-15 in the peripheral blood of the URSA experimental group and the control group was significantly lower than that in the control group, indicating that the appropriate amount of IL-15 was beneficial to the maintenance of pregnancy, and the high concentration of IL-15 was not conducive to the pregnancy, which may be one of the causes of the abortion. (3) Bushen Huoxue Recipe may improve the level of IL-13 and IL-35 in the peripheral blood of the URSA experimental group, and decrease the level of the factor of IL-15 in the peripheral blood of the URSA experimental group to improve the clinical curative effect of the patients with recurrent spontaneous abortion. (4) Bushen Huoxue Decoction can obviously improve the content of progesterone, estradiol and blood-HCG in the peripheral blood of the patients with URSA, so as to maintain the pregnancy.
