【摘要】：目的：通過對(duì)海桐皮湯聯(lián)合體外沖擊波治療肱骨外上髁炎臨床療效的研究,探討海桐皮湯聯(lián)合體外沖擊波在緩解患者肘外側(cè)疼痛,改善患者肘部功能等方面療效,優(yōu)選臨床療效好、安全性高、簡便廉價(jià)的復(fù)合療法,并為其臨床推廣提供理論依據(jù)。方法：取2015年4月-2015年11月門診的肱骨外上髁炎病人90例,隨機(jī)分為三組：A組(中藥熏蒸組)：30例,海桐皮湯熏蒸,每日2次,連續(xù)2周；B組(沖擊波組)：30例,體外沖擊波治療,每2日1次,連續(xù)2周,C組(中藥熏蒸聯(lián)合沖擊波組)：30例,海桐皮湯熏蒸,每日2次,體外沖擊波,每2日1次,連續(xù)2周。治療周期結(jié)束后觀察臨床療效、握力、肘部疼痛綜合評(píng)定和肘關(guān)節(jié)功能評(píng)定。結(jié)果：同一時(shí)間點(diǎn)組間比較,臨床療效：A、B和C三組總有效率分別82.14%、79.31%和89.29%,三組之間比較有顯著統(tǒng)計(jì)學(xué)差異(P0.05)。握力測量：A、B、C三組冶療后握力均大于治療前握力(P0.05),并且A組優(yōu)于B組(P0.05),C組明顯優(yōu)于A組和B組(P0.05)。SF-McGill量表評(píng)分：治療后三組患者SF-McGill量表總分均較治療前顯著降低(P0.05),并且A組總分低于B組(P0.05),C組總分明顯低于A組和B組(P0.05)。肘關(guān)節(jié)功能評(píng)定：治療后三組患者肘功能評(píng)定量表總分均較治療前顯著提高(P0.05),并且A組總分高于B組,C組總分高于A組和B組,但差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論：中藥熏蒸療法、沖擊波療法和中藥熏蒸聯(lián)合沖擊波治療肱骨外上髁炎均能夠減輕局部疼痛、改善握力及肘關(guān)節(jié)功能；在臨床療效、緩解局部疼痛及改善握力方面中藥熏蒸療法療效優(yōu)于沖擊波療法,而中藥熏蒸聯(lián)合沖擊波療法療效明顯優(yōu)于單純中藥熏蒸療法和沖擊波療法。
[Abstract]:Objective: to study the clinical effect of Hetongpi decoction combined with extracorporeal shock wave in the treatment of external epicondylitis of humerus, and to explore the curative effect of Hetongpi decoction combined with extracorporeal shock wave on relieving pain and improving elbow function of patients. Excellent selection of clinical efficacy, high safety, simple and cheap combination therapy, and to provide a theoretical basis for its clinical promotion. Methods: from April 2015 to November 2015, 90 patients with external epicondylitis of humerus were randomly divided into three groups: group A (Chinese medicine fumigation group): 30 cases, Haitongpi decoction fumigation, twice a day for 2 weeks; Group B (shock wave group): 30 cases were treated with extracorporeal shock wave once every 2 days for 2 weeks, group C (Chinese medicine fumigation combined with shock wave group): 30 cases, sea Tongpi decoction fumigation, twice a day, extracorporeal shock wave once every 2 days. Two weeks in a row. The clinical effect, grip strength, elbow pain and elbow function were observed after the treatment period. Results: at the same time point, the total effective rate of group A B and C was 82.14% and 89.29%, respectively. There was significant difference among the three groups (P0.05). Grip strength measurement: after treatment, the grip strength of group A was higher than that of group B (P0.05), and group A was superior to group B (P0.05). Group C was significantly better than group A and group B (P0.05). SF-McGill scale score: after treatment, the total score of SF-McGill scale in three groups was significantly lower than that before treatment (P0.05), and the total score of group A was lower than that of group B (P0.05). The total score of group C was significantly lower than that of group A and group B (P0.05). Elbow function evaluation: after treatment, the total score of elbow function rating scale in three groups was significantly higher than that before treatment (P0.05), and the total score of group A was higher than that of group B, and the total score of group C was higher than that of group A and group B, but the difference was not statistically significant (P0.05). Conclusion: traditional Chinese medicine fumigation, shock wave therapy and combined Chinese medicine fumigation combined shock wave treatment of humeral epicondylitis can alleviate local pain, improve grip strength and elbow joint function. Traditional Chinese medicine fumigation therapy was superior to shock wave therapy in clinical efficacy, local pain relief and grip strength improvement, while traditional Chinese medicine fumigation combined with shock wave therapy was superior to simple traditional Chinese medicine fumigation therapy and shock wave therapy.
