【摘要】：女性尿道綜合征一直是個(gè)相當(dāng)敏感又尷尬的話題,主要表現(xiàn)癥狀為尿頻、尿急、排尿不暢,另外女性尿道綜合征還對(duì)女性的社會(huì)互動(dòng)、性生活、生活質(zhì)量等方面有著嚴(yán)重影響,甚至?xí)古援a(chǎn)生羞恥、抑郁等情況,從而導(dǎo)致患有女性尿道綜合征女性出現(xiàn)身體形象的改變或者是社交隔離等現(xiàn)象。女性尿道綜合征就像是一顆埋在女性身體和心里的不定時(shí)炸彈,較多女性其實(shí)很害怕在外面時(shí),因?yàn)槟蝾l、尿急,而不能及時(shí)找到找到廁所進(jìn)行排解,因此也不敢輕易的外出,出門也會(huì)盡量選擇有比較多廁所的地方,對(duì)部分人來說這樣的情形,在生活上產(chǎn)生了較大的影響。目前在國(guó)內(nèi)可查的文獻(xiàn)當(dāng)中了解到,尿道綜合征在西方醫(yī)學(xué)的治療方面,多數(shù)是服用藥物、手術(shù)治療、介入治療等,在中醫(yī)治療方面也很多元化,有中藥治療、針刺配合電針、艾灸治療、埋線治療、耳穴治療等。目的：本課題采用臨床小樣本隨機(jī)對(duì)照研究,通過觀察溫針灸組與常規(guī)針刺組治療女性尿道綜合征的患者。觀察治療前后癥狀體征的變化明確,希望透過此研究能取得較好的臨床療效。研究方法：將符合要求的56例患者隨機(jī)分為觀察組29例和對(duì)照組27例。觀察組采取溫針灸治療,對(duì)照組采取針刺治療,針刺穴位選取以腹部為主,每周2次,每10次為一個(gè)療程。2個(gè)療程之后進(jìn)行觀察評(píng)比。評(píng)分量表在治療前及治療結(jié)束后各評(píng)價(jià)1次,對(duì)比治療前后的療效及癥狀改善情況。結(jié)果：1、治療前,試驗(yàn)組與對(duì)照組在患者年齡及病程等指標(biāo),經(jīng)統(tǒng)計(jì)學(xué)檢驗(yàn),均無明顯差異(P0.05),具有可比性。試驗(yàn)組與對(duì)照組在各癥狀評(píng)分分別進(jìn)行統(tǒng)計(jì)學(xué)檢驗(yàn),結(jié)果無明顯差異(P0.05),具有可比性。2、經(jīng)過2個(gè)療程的治療,FUSS和QOL觀察患者治療后對(duì)于尿道綜合癥及生活質(zhì)量都有改善,試驗(yàn)組有效率82.76%,對(duì)照組有效率77.78%,說明兩組對(duì)尿道綜合征均有治療效果。SDS觀察患者治療后對(duì)于抑郁情況的方面也有改善。3、試驗(yàn)組和對(duì)照組在治療后尿頻、尿急、排尿不暢等癥狀,在指標(biāo)的組內(nèi)比較有統(tǒng)計(jì)學(xué)意義(P0.05),說明兩組治療方法對(duì)以上癥狀的改善有一定的療效。結(jié)論：溫針灸治療與針刺治療兩種的治療方式,對(duì)女性患者均能有效的改善尿道綜合征,生活質(zhì)量亦都有效改善。本研究經(jīng)過兩種的治療觀察,溫針灸治療效果優(yōu)于常規(guī)針刺治療。此方法為臨床研究提供了新的思路方向。
[Abstract]:Female urethral syndrome has always been a very sensitive and embarrassing topic. Its main symptoms are frequency of urination, urgency of urination, unobstructed urination. In addition, female urethral syndrome also has a serious impact on women's social interaction, sexual life, quality of life, etc. It may even lead to shame, depression and other conditions, resulting in women with female urethral syndrome, such as changes in body image or social isolation. Female urethra syndrome is like an unfixed time bomb buried in a woman's body and heart. More women are actually very afraid to be outside because of frequent urination and urination, so they cannot find the toilet in time for drainage, so they do not dare to go out easily. Go out will also try to choose more toilet places, for some people such a situation, in life has a greater impact. At present, among the available literature in China, it has been learned that the treatment of urethral syndrome in western medicine is mostly drug taking, surgical treatment, interventional therapy, and so on. There are also many kinds of Chinese medicine treatment, including traditional Chinese medicine treatment, acupuncture combined with electroacupuncture, etc. Moxibustion therapy, catgut embedding therapy, ear point therapy, etc. Objective: to observe the effect of warming acupuncture group and routine acupuncture group on female urethral syndrome. Observing the changes of symptoms and signs before and after treatment, we hope to obtain better clinical effect through this study. Methods: 56 patients were randomly divided into observation group (n = 29) and control group (n = 27). The observation group was treated with warm acupuncture and moxibustion, the control group was treated with acupuncture, and the acupuncture points were mainly selected as abdomen, twice a week, every 10 times as a course of treatment. The score scale was evaluated once before and after treatment, and the curative effect and symptom improvement before and after treatment were compared. Results: 1, before treatment, there was no significant difference between the test group and the control group in patients' age and course of disease (P0.05). There was no significant difference between the test group and the control group in each symptom score (P0.05). 2, after two courses of treatment, there was no significant difference between the test group and the control group. FUSS and QOL were used to observe the improvement of urethral syndrome and quality of life after treatment. The effective rate was 82.76 in the experimental group and 77.78 in the control group. The results showed that the two groups were effective in treating urethral syndrome. After SDS treatment, the depression was also improved. 3. The symptoms of frequency of urination, urgency of urination and dysuria after treatment were observed in the test group and control group. In the index of the group compared with statistical significance (P0.05), indicating that the two groups of treatment methods to improve the above symptoms have a certain effect. Conclusion: both warming acupuncture and acupuncture treatment can effectively improve the urethral syndrome and the quality of life in female patients. After two kinds of treatment observation, the effect of warming acupuncture is better than that of routine acupuncture. This method provides a new way of thinking for clinical research.
