【摘要】：目的:探討通竅活血湯對外傷后水瘀互結型腦積水患者血清神經(jīng)生化標志物及腦脊液p73,p38表達和認知功能的影響。方法:選取新鄉(xiāng)醫(yī)學院第一附屬醫(yī)院就診并診斷為外傷后腦積水患者74例,按照隨機數(shù)字表法分為兩組,對照組(37例)給予常規(guī)對癥及支持治療,觀察組(37例)在對照組基礎上予通竅活血湯治療,觀察并記錄兩組患者治療前后人腦髓鞘堿性蛋白(myelin basic protein,MBP),中樞神經(jīng)特異蛋白(Protein S100β),神經(jīng)元特異性烯醇化酶(neuron-specificenolase,NSE)水平,腦脊液p73,p38水平,術前簡易精神狀態(tài)評價量表(minimum mental state examination,MMSE),格拉斯哥昏迷評分(Glasgow coma scale,GCS),同時對比臨床療效及不良反應發(fā)生情況。結果:對照組治療有效率低于觀察組(P0.05);與治療前比較,兩組治療后血清MBP,S100β,NSE水平均較治療前明顯降低(P0.05),且觀察組顯著低于對照組(P0.05);兩組治療后患者的腦脊液p73,p38蛋白表達均升高(P0.05),且觀察組高于對照組(P0.05);兩組治療后患者的MMSE,GCS評分均升高(P0.05),且觀察組高于對照組(P0.05)。結論:通竅活血湯能有效改善患者臨床癥狀,可能與其降低外傷后水瘀互結型腦積水患者血清MBP,S-100β,NSE水平,提高腦脊液p73,p38蛋白表達水平有關。
[Abstract]:Objective: to investigate the effect of Tongqiao Huoxue decoction on serum neurobiochemical markers, expression of p73 p38 and cognitive function in patients with hydrocephalus. Methods: a total of 74 patients with post-traumatic hydrocephalus were selected from the first affiliated Hospital of Xinxiang Medical College and were randomly divided into two groups: the control group (37 cases) received routine treatment and supportive therapy. The observation group (37 cases) was treated with Tongqiao Huoxue decoction on the basis of the control group. The myelin basic protein (myelin basic protein,MBP) and central nervous specific protein (Protein S100 尾) were observed and recorded before and after treatment in both groups. The levels of neuron-specific enolase (neuron-specificenolase,NSE), cerebrospinal fluid (CSF) p73 and p38, preoperative mini-mental state assessment scale (minimum mental state examination,MMSE), Glasgow coma score (Glasgow coma scale,GCS), etc. At the same time, the clinical efficacy and adverse reactions were compared. Results: the effective rate of treatment in the control group was lower than that in the observation group (P0.05), the levels of serum MBP,S100 尾 and NSE in the two groups were significantly lower than those before treatment (P0.05), and the levels of serum MBP,S100 尾 and NSE in the observation group were significantly lower than those in the control group (P0.05). After treatment, the expression of p73 p38 protein in cerebrospinal fluid increased in both groups (P0.05), and was higher in observation group than that in control group (P0.05). The MMSE,GCS scores of the two groups were increased after treatment (P0.05), and the observation group was higher than the control group (P0.05). Conclusion: Tongqiao Huoxue decoction can effectively improve the clinical symptoms of patients with hydrocephalus, which may be related to the decrease of serum MBP,S-100 尾 and NSE levels in patients with hydrocephalus and increase the expression level of p73 p38 protein in cerebrospinal fluid.
