【摘要】：目的:探討參附養(yǎng)榮湯加味治療病毒性心肌炎(VMC)慢性期的療效及對(duì)患者血清中白細(xì)胞介素(IL)-17,IL-27,IL-6,IL-9和核轉(zhuǎn)錄因子(NF)-κB水平的影響。方法:將140例VMC患者隨機(jī)按數(shù)字表法分為對(duì)照組和觀察組各70例。對(duì)照組給予西醫(yī)綜合干預(yù)措施。觀察組在對(duì)照組治療的基礎(chǔ)上給予參附養(yǎng)榮湯加味,1劑/d,早晚2次內(nèi)服;兩組患者連續(xù)治療8周。比較兩組患者陰陽(yáng)兩虛證癥狀評(píng)分、血清中心型游離脂肪酸結(jié)合蛋白(H-FABP)和肌鈣蛋白I(c Tn I)水平、總體療效及不良反應(yīng)發(fā)生。檢測(cè)兩組患者血清中IL-17,IL-27,IL-6,IL-9和NF-κB水平。結(jié)果:治療后觀察組陰陽(yáng)兩虛證癥狀(心悸、氣短、肢冷畏寒、煩躁、喘促、汗多、浮腫)評(píng)分、血清中H-FABP和c Tn I水平均明顯低于對(duì)照組(P0.01);觀察組的臨床總有效率為91.18%,明顯高于對(duì)照組76.12%(P0.05);觀察組患者的頭痛等不良反應(yīng)發(fā)生率均少于對(duì)照組,其中惡心嘔吐和腹瀉明顯少于對(duì)照組(P0.05);觀察組治療后血清中IL-17,IL-6,IL-9和NF-κB水平均明顯低于對(duì)照組,IL-27高于對(duì)照組(P0.01)。結(jié)論:在西醫(yī)常規(guī)治療的基礎(chǔ)上,參附養(yǎng)榮湯加味對(duì)VMC慢性期患者發(fā)揮了良好療效,且安全性好,調(diào)節(jié)血清IL-17,IL-27,IL-6,IL-9和NF-κB水平變化可能是其作用機(jī)制之一。
[Abstract]:Objective: to investigate the therapeutic effect of Shenfu Yangrong decoction on chronic phase of viral myocarditis (VMC) and its effect on the levels of interleukin-17 (IL) -27 (IL-27) IL-6 (IL-6) IL-9 and nuclear transcription factor (NF)-魏 B in serum of patients with viral myocarditis. Methods: 140 patients with VMC were randomly divided into control group (n = 70) and observation group (n = 70). The control group was given western comprehensive intervention. The observation group was treated with Shenfu Yangrong decoction on the basis of the treatment in the control group, 1 dose per day, 2 times in the morning and evening, and the patients in the two groups were treated continuously for 8 weeks. The scores of symptoms of yin and yang deficiency syndrome, the levels of serum free fatty acid binding protein (H-FABP) and troponin I (c Tn I), the overall curative effect and the occurrence of adverse reactions were compared between the two groups. The serum levels of IL-17,IL-27,IL-6,IL-9 and NF- 魏 B were measured in both groups. Results: after treatment, the symptoms of Yin and Yang deficiency syndrome (palpitation, shortness of breath, cold and cold limbs, irritability, shortness of breath, sweating and swelling) in the observation group were significantly lower than those in the control group (P0.01). The serum levels of H-FABP and c Tn I in the observation group were significantly lower than those in the control group (P0.01). The total effective rate of the observation group was 91.18%, which was significantly higher than that of the control group (76.12%) (P0.05). The incidence of headache and other adverse reactions in the observation group was lower than that in the control group, and the incidence of nausea, vomiting and diarrhea was significantly lower than that in the control group (P0.05). The serum levels of IL-17,IL-6,IL-9 and NF- 魏 B in the observation group were significantly lower than those in the control group, and the levels of IL-27 were higher than those in the control group (P0.01). Conclusion: on the basis of routine western medicine treatment, Shenfu Yangrong decoction has a good curative effect on chronic VMC patients, and is safe and adjusts serum IL-17,IL-27,IL-6,. The changes of IL-9 and NF- 魏 B levels may be one of its mechanisms.
