黃連降糖片改善2型糖尿病胰島素抵抗的臨床研究

【摘要】：目的:本課題研究擬通過臨床觀察“黃連降糖片”治療2型糖尿病胰島素抵抗的臨床療效和安全性,初步評價(jià)“黃連降糖片”的作用機(jī)理及其安全性,為中醫(yī)藥改善胰島素抵抗提供新的思路與方法。方法:本課題隨機(jī)選取開封市中醫(yī)院內(nèi)分泌科門診符合2型糖尿病(T2DM)胰島素抵抗的診斷標(biāo)準(zhǔn),符合中醫(yī)濕熱互結(jié)辨證標(biāo)準(zhǔn)的T2DM患者66例,探討“黃連降糖片”改善糖尿病胰島素抵抗的臨床療效。同時(shí)觀察,受試者用黃連降糖片后的臨床癥狀,空腹血糖(FPG),餐后2小時(shí)血糖(2h PG),糖化血紅蛋白(Hb A1C),空腹胰島素(FINS),胰島素抵抗指數(shù)(HOMA-IR),總膽固醇(TC),甘油三脂(TG),高密度脂蛋白(HDL-C),低密度脂蛋白(LDL-C)等指標(biāo)以及中醫(yī)臨床癥狀積分治療前后變化,為臨床用藥提供科學(xué)依據(jù)。結(jié)果:1.治療組的中醫(yī)癥候有效率優(yōu)于對照組,差異有顯著意義(P0.05);2.兩組治療后中醫(yī)癥狀積分均減少,同組內(nèi)治療前后相比有統(tǒng)計(jì)學(xué)意義(P0.05);組間比較治療組優(yōu)于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.兩組均可降低FPG、2h PG、Hb A1C,同組內(nèi)治療前后相比有統(tǒng)計(jì)學(xué)意義(P0.05);治療后組間FPG、2h PG比較,治療組優(yōu)于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療后兩治療后組間比較,Hb A1C無明顯差異(P0.05)。4.兩組均可降低FINS、HOMA-IR,同組內(nèi)治療前后相比有統(tǒng)計(jì)學(xué)意義(P0.05);組間比較治療組優(yōu)于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。5.兩組均能降低TC、TG、LDL-C,治療組可升高HDL-C,差異有顯著意義(P0.05)。6.兩組試驗(yàn)過程中三大常規(guī)和肝腎功能等未出現(xiàn)異常。結(jié)論:“黃連降糖片”能夠很好的改善患者的癥狀和體征,可以降低血糖、減輕胰島素抵抗,還能夠有效調(diào)節(jié)脂代謝紊亂,初步判定其降糖作用可能與改善胰島素抵抗相關(guān)。
[Abstract]:Objective: to observe the clinical efficacy and safety of Huanglian Jiangtang tablet in the treatment of insulin resistance in type 2 diabetes mellitus, and to evaluate the mechanism and safety of Huanglian Jiangtang tablet. To provide new ideas and methods for traditional Chinese medicine to improve insulin resistance. Methods: this study randomly selected 66 T2DM patients who met the diagnostic criteria of insulin resistance of type 2 diabetes mellitus (T2DM) in the Department of Endocrinology, Kaifeng Hospital of traditional Chinese Medicine. To investigate the clinical effect of Huanglian Jiangtang tablet on improving insulin resistance of diabetes mellitus. At the same time, the clinical symptoms of Huanglian Jiangtang tablet, 2 hours postprandial blood glucose of (FPG), (2 h PG), glycosylated hemoglobin (Hb A1C), fasting insulin (FINS), insulin resistance index (HOMA-IR) were observed. The changes of total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL-C), low density lipoprotein (LDL-C) and clinical symptom integral of traditional Chinese medicine (TCM) before and after treatment provide scientific basis for clinical use of drugs. The result is 1: 1. The effective rate of TCM symptom in the treatment group was better than that in the control group, the difference was significant (P0.05); 2. The scores of TCM symptoms in the two groups were decreased after treatment, there was statistical significance before and after treatment in the same group (P0.05); the comparison between the treatment group and the control group was better than that in the control group, the difference was statistically significant (P0.05). Both groups can reduce FPG,2h PG,Hb A1C, the same group before and after treatment compared with the statistical significance (P0.05); after treatment, the treatment group was better than the control group, the difference was statistically significant (P0.05). There was no significant difference in Hb A1C between the two groups after treatment (P0.05). Both groups can reduce FINS,HOMA-IR, in the same group before and after treatment compared with statistical significance (P0.05), the treatment group was better than the control group, the difference was statistically significant (P0.05). Both groups can reduce the TC,TG,LDL-C, treatment group can increase HDL-C, there was significant difference (P0.05). The three major routine and liver and kidney function were not abnormal in the two groups. Conclusion: Huanglian Jiangtang tablet can improve the symptoms and signs of the patients, reduce blood sugar, reduce insulin resistance, and regulate lipid metabolism disorder effectively. It is preliminarily concluded that its hypoglycemic effect may be related to the improvement of insulin resistance.
