【摘要】：目的:觀察針刺“環(huán)下”穴加常規(guī)取穴治療腰椎間盤突出癥的臨床療效,并與常規(guī)針刺的療效相比較,探討其作用機(jī)理,以期為針灸治療腰椎間盤突出癥提供一種新的思路。方法:篩選符合納入標(biāo)準(zhǔn)的腰椎間盤突出癥患者62例,使用Doll’s臨床病例隨機(jī)表將62例患者按1:1的比例分為治療組和觀察組,每組各31例(每組各脫落1例,實際每組30例)。治療組加刺“環(huán)下”穴,余與對照組相同,每日一次,連續(xù)六次后休息一日為一個療程,治療兩個療程。每個療程結(jié)束后進(jìn)行療效觀察,并在VAS和JOA評分方面對兩組進(jìn)行自身和組間比較。結(jié)果:1.療效結(jié)果比較:兩個療程結(jié)束后,兩組患者均取得了良好的療效,治療組的總有效率略高于對照組,但治療組的愈顯率明顯高于對照組。2.VAS評分比較:兩療程結(jié)束后,兩組患者的VAS評分均有所下降,第一、第二療程VAS評分差值比較顯示均具有顯著性差異(P0.05),提示治療組能夠持續(xù)、更大程度的減輕患者的疼痛。3.JOA評分比較:兩療程結(jié)束后,兩組患者的JOA評分均有大幅度提高(P0.05),提示兩種治療方案都能明顯改善腰椎間盤突出癥患者的臨床癥狀,第一、第二療程JOA評分差值比較顯示兩組具有顯著性差異(P0.05),而且治療組的兩療程差值均大于對照組,提示治療組相比對照組在改善患者的癥狀方面更具優(yōu)勢。結(jié)論:兩組患者經(jīng)過兩個療程的治療都取得了較好的療效,疼痛程度明顯減輕,癥狀也得到明顯緩解,且治療組優(yōu)于對照組。結(jié)果表明針刺“環(huán)下”穴加常規(guī)針刺治療腰椎間盤突出癥確有良好的臨床療效,值得推廣。
[Abstract]:Objective: to observe the clinical effect of acupuncture "Huanxia" point plus conventional acupuncture on lumbar intervertebral disc herniation, and compare the effect of acupuncture with that of routine acupuncture, and explore its mechanism, so as to provide a new way of thinking for acupuncture treatment of lumbar disc herniation. Methods: 62 patients with lumbar disc herniation were selected according to the inclusive criteria. 62 patients were randomly divided into treatment group and observation group according to the 1:1 scale by using Doll's clinical case list. Each group had 31 cases (1 case in each group and 30 cases in each group). In the treatment group, the point of "Huanxia" was pricked, and the rest was the same as the control group, once a day, six times a day for a course of treatment and two courses of treatment. The curative effect was observed at the end of each course, and the VAS and JOA scores were compared between the two groups. The result is 1: 1. The total effective rate of the treatment group was slightly higher than that of the control group, but the effective rate of the treatment group was obviously higher than that of the control group. Comparison of 2.VAS score: after the two courses of treatment, the total effective rate of the treatment group was higher than that of the control group. The VAS scores of the two groups were all decreased. First, the difference of VAS scores in the second course of treatment was significantly different (P0.05), which indicated that the treatment group could continue to alleviate the pain of the patients to a greater extent. Comparison of 3.JOA score: after the end of the two courses of treatment, The JOA scores of the two groups were significantly improved (P0.05), suggesting that the two treatment schemes can significantly improve the clinical symptoms of patients with lumbar disc herniation, first, The difference of JOA score in the second course of treatment showed significant difference between the two groups (P0.05), and the difference of two courses of treatment in the treatment group was higher than that in the control group, indicating that the treatment group had more advantages in improving the symptoms of the patients than the control group. Conclusion: after two courses of treatment, the patients in both groups achieved better curative effect, the pain degree was obviously alleviated, and the symptoms were relieved obviously, and the treatment group was superior to the control group. The results showed that acupuncture of "Huanxia" and routine acupuncture in the treatment of lumbar disc herniation has a good clinical effect and is worth popularizing.
