【摘要】：目的:本研究旨在觀察清熱化痰活血祛瘀法治療痰瘀互阻型不穩(wěn)定型心絞痛的臨床療效,并探究其作用機(jī)理。方法:本臨床研究中,所選取的病例均來自2014月9月至2015年12月山東中醫(yī)藥大學(xué)附屬醫(yī)院綜合內(nèi)科病房的住院病人,共60例,所選病例均符合納入排除標(biāo)準(zhǔn),隨機(jī)分為試驗(yàn)組與對(duì)照組,各30例,二組患者的一般情況無統(tǒng)計(jì)學(xué)差異。兩組均給予西醫(yī)常規(guī)綜合治療,試驗(yàn)組在此基礎(chǔ)上給予清熱化痰活血祛瘀方,日一劑,分早、晚兩次溫服。4周后觀察治療前后兩組的中醫(yī)癥候積分、中醫(yī)證候療效、心電圖變化、硝酸甘油停減率、血脂、血流變、纖維蛋白原等相關(guān)實(shí)驗(yàn)室檢查,通過對(duì)比分析,評(píng)價(jià)其臨床療效。結(jié)果:試驗(yàn)組與對(duì)照組療效相比,試驗(yàn)組在中醫(yī)證候積分方面改善更加明顯、改善心絞痛發(fā)作方面優(yōu)于對(duì)照組,在硝酸甘油停減率、心電圖變化、TC、TG等實(shí)驗(yàn)室指標(biāo)方面,兩組對(duì)比有統(tǒng)計(jì)學(xué)差異(P0.05或P0.01),試驗(yàn)組較對(duì)照組有更好的療效,在肝腎功等安全性指標(biāo)方面,二者無統(tǒng)計(jì)學(xué)差異,P0.05,應(yīng)用安全。結(jié)論:相比單純西醫(yī)常規(guī)治療,試驗(yàn)組在改善患者臨床癥狀及相關(guān)實(shí)驗(yàn)室指標(biāo)方面療效更加的顯著,且應(yīng)用安全。因此,其在治療痰瘀互阻型不穩(wěn)定型心絞痛方面有著廣闊的前景及優(yōu)勢(shì)。
[Abstract]:Objective: to observe the clinical effect of clearing heat, removing phlegm and activating blood circulation and removing blood stasis in the treatment of unstable angina pectoris of phlegm and blood stasis. Methods: in this clinical study, 60 inpatients were selected from the general medical wards of the affiliated Hospital of Shandong University of traditional Chinese Medicine from September, 2014 to December, 2015. All the selected cases met the exclusion criteria. The patients were randomly divided into the experimental group and the control group with 30 cases each. There was no statistical difference in the general situation between the two groups. Both groups were given routine comprehensive treatment of western medicine, and the experimental group was given the prescription of clearing heat, resolving phlegm and activating blood circulation and removing blood stasis on this basis, one dose a day, two times in the morning, two times in the evening, after 4 weeks, observe the integral of TCM symptoms and the curative effect of TCM syndrome before and after the treatment. The changes of electrocardiogram, stopping rate of nitroglycerin, blood lipid, hemorheology, fibrinogen and other related laboratory tests were compared and analyzed to evaluate the clinical effect. Results: compared with the control group, the improvement of TCM syndromes and angina pectoris in the trial group was better than that in the control group. There was statistical difference between the two groups (P0.05 or P0.01). The experimental group had better curative effect than the control group. There was no statistical difference between the two groups in terms of safety indexes such as liver and kidney function. Conclusion: compared with routine western medicine, the treatment group is more effective and safe in improving clinical symptoms and related laboratory indexes. Therefore, it has broad prospects and advantages in the treatment of unstable angina pectoris with phlegm and blood stasis.
