【摘要】：本研究以疏肝法作為切入點,通過進行臨床的回顧性研究,以中醫(yī)藥理論知識為指導,依次應用觀察法、比較法及數(shù)理統(tǒng)計分析法,對國醫(yī)大師段富津教授在臨床實踐中從肝論治內(nèi)科病證的方法進行歸納和分析,以期全面繼承其學術思想,為臨床治療提供指導。首先,對疏肝法進行文獻學研究,論述不同時期的醫(yī)家和醫(yī)籍對肝的認識及治法,立足古代文獻,通過中醫(yī)藏象學理論框架,整理疏肝法的立論依據(jù)。結合現(xiàn)代臨床研究進展,為整理和總結段富津教授應用疏肝法治療內(nèi)科病證的辨治規(guī)律提供了理論依據(jù)。通過回顧分析207例處方中含有疏肝作用藥物的典型病例,以方劑的配伍特點為基礎,結合證候,歸納疏肝法的治法分類和應用病種,研究分析段富津教授運用疏肝法治療內(nèi)科病證的診治思路和用藥特點：1.從肝論治,疏肝為要。段富津教授在臨床辨治不寐、心悸、呃逆、脅痛、腹痛、郁證等證,常從肝論治,故立疏肝法為基本大法。2.疏肝六法,百法備焉。疏肝六法即疏肝行氣法、疏肝養(yǎng)血法、疏肝活血法、疏肝清熱法、養(yǎng)陰疏肝法、健脾疏肝法。此六法既單獨組方運用,又依病證之變而數(shù)法合用,以圓機活法。3.味辛入肝,“配伍”疏肝。入肝經(jīng)且有辛散之性的藥物,經(jīng)配伍均可在方中具有疏肝作用,且“血中氣藥”、理氣藥、風藥,與肝經(jīng)藥配伍亦多可具有疏肝之功。本研究總結了段富津教授應用疏肝法治療內(nèi)科病證的臨床經(jīng)驗,并探索了從治法這一新角度研究名老中醫(yī)組方用藥特點和辨治疾病的思路,為進一步繼承和發(fā)揚名老中醫(yī)經(jīng)驗提供參考。
[Abstract]:This study takes the method of soothing the liver as the starting point, through the clinical retrospective study, guided by the theoretical knowledge of traditional Chinese medicine, and then applies the observation method, the comparative method and the mathematical statistical analysis method. This paper sums up and analyzes the methods of treating internal diseases and syndromes from liver in clinical practice by Professor Duan Fujin, a master of Chinese medicine, in order to inherit his academic thought and provide guidance for clinical treatment. First of all, the author studies the method of soothing the liver, discusses the understanding and treatment of liver by doctors and medical books in different periods, based on the ancient literature, collates the theoretical basis of the method of soothing the liver through the theoretical framework of traditional Chinese medicine. Combined with the progress of modern clinical research, this paper provides a theoretical basis for professor Duan Fujin's application of soothing liver method in the differentiation and treatment of internal diseases and syndromes. Based on the characteristics of prescription compatibility and syndromes, 207 cases of typical cases with soothing liver drugs in prescriptions were analyzed retrospectively. The classification and application of liver soothing methods were summarized. Research and analysis of Professor Duan Fujin's treatment of internal medicine syndrome with the method of soothing the liver, diagnosis and treatment thinking and characteristics of drug use: 1. Treat from liver, soothe liver is important. Professor Duan Fujin in clinical differentiation and treatment of insomnia, palpitations, hiccups, hypochondriac pain, abdominal pain, depression syndrome and other syndromes, often from the liver treatment, it is the basic law of soothing the liver. 2. The six methods of soothing the liver, the hundred methods of preparation. The six methods of soothing the liver are the method of soothing liver and qi, the method of soothing liver and nourishing blood, the method of soothing liver and activating blood, the method of soothing liver and clearing away heat, the method of nourishing yin and dispersing liver, and the method of strengthening spleen and soothing liver. The six methods were used separately, and combined with several methods according to the change of disease and syndrome, with round machine activity method. 3. Flavor Xin into the liver, "compatibility" soothing liver. The drugs that enter the liver meridian and have the nature of Xin San can soothe the liver in the prescription, and the combination of "qi medicine in blood", Qi regulating medicine, wind medicine, and liver meridian medicine can also have the function of soothing the liver. This study summarized the clinical experience of Professor Duan Fujin in treating internal diseases and syndromes with the method of soothing the liver, and explored the characteristics of traditional Chinese medicine prescriptions and the thinking of treating diseases from the new angle of treatment. To further inherit and carry forward the famous old Chinese medicine experience to provide reference.
