針?biāo)幗Y(jié)合治療腎虛肝郁型黃體功能不足月經(jīng)先期的臨床觀察

【摘要】：目的:本課題評(píng)估天癸湯聯(lián)合針灸對(duì)腎虛肝郁型黃體功能不足(luteal phasedefect,LPD)月經(jīng)先期的療效,并與地屈孕酮進(jìn)行對(duì)比觀察,評(píng)定臨床有效性,闡明針?biāo)幗Y(jié)合治療LPD月經(jīng)先期的作用機(jī)制,旨在為針?biāo)幗Y(jié)合治療該病的優(yōu)勢(shì)性提供可靠的臨床及實(shí)驗(yàn)根據(jù),展現(xiàn)中醫(yī)藥未病預(yù)防的觀點(diǎn),冀此降低日久LPD引發(fā)的不孕、流產(chǎn)等。研究方法:采取隨機(jī)對(duì)照法實(shí)行前瞻性方案觀察,病例納入62例受試者隨機(jī)分為治療組31例,對(duì)照組31例,其中對(duì)照組1例脫落,最終入組治療組31例,對(duì)照組30例,治療組采納口服天癸湯結(jié)合穴位針刺,對(duì)照組予以口服地屈孕酮,治療時(shí)間均始于BBT上升第一天,連續(xù)10日,若經(jīng)血下溢停止診療。3個(gè)月經(jīng)周期后對(duì)比評(píng)價(jià)兩組受試者療前、療后疾病療效、黃體中期P、E2水平、中醫(yī)癥狀及BBT變化情形。結(jié)果:1.當(dāng)療期結(jié)束后,據(jù)統(tǒng)計(jì)學(xué)解析,治療組疾病總療效、中醫(yī)總癥狀改觀情況優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2.治療組在受試者月經(jīng)提前天數(shù),BBT高溫相持續(xù)天數(shù),高低溫差及血P值改善情況與對(duì)照組相當(dāng),差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。治療組上升E2水平高于對(duì)照組,差異具顯著性(P0.01)。3.觀察預(yù)后的穩(wěn)定性,隨訪顯示治療組復(fù)發(fā)率小于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:天癸湯結(jié)合針灸治療腎虛肝郁型黃體功能不足月經(jīng)先期臨床療效肯定,且優(yōu)于對(duì)照組,其療效機(jī)理以調(diào)治腎一天癸一沖任一胞宮軸的穩(wěn)態(tài),從而有效調(diào)節(jié)月經(jīng)周期,改善受試者癥狀,升高血清激素水平,恢復(fù)黃體功能,且遠(yuǎn)期療效好,安全性高。
[Abstract]:Objective: to evaluate the effect of Tiangui decoction combined with acupuncture and moxibustion on menstrual premenstruation in patients with deficiency of kidney and liver depression (luteal phasedefect,LPD), and to evaluate the clinical efficacy and the mechanism of acupuncture combined with medicine in the treatment of menstrual premenstruation of LPD. The purpose of this study was to provide reliable clinical and experimental basis for the combination of acupuncture and medicine in the treatment of the disease, to show the viewpoint of prevention of non-disease in traditional Chinese medicine, and to reduce infertility and abortion caused by LPD for a long time. Methods: 62 patients were randomly divided into treatment group (n = 31), control group (n = 31), final treatment group (n = 31) and control group (n = 30). The treatment group was treated with oral Tiangui decoction combined with acupuncture at acupoint, and the control group was treated with diproprogesterone. The treatment time began on the first day of BBT rise and lasted for 10 consecutive days, if the diagnosis and treatment were stopped through blood overflow. After 3 menstrual cycles, the subjects of both groups were compared and evaluated before treatment. After treatment, the curative effect of disease, the level of E _ 2 in luteal phase, the symptoms of TCM and the change of BBT. The result is 1: 1. After the end of the treatment period, according to statistical analysis, the total curative effect of the treatment group, the total symptoms of traditional Chinese medicine improved better than the control group, the difference was statistically significant (P0.05). 2. In the treatment group, the days of menstrual advance and the duration of BBT hyperthermia, the difference of high and low temperature and the improvement of blood P value were similar to those of the control group, and the difference was not statistically significant (P0.05). The level of E _ 2 in the treatment group was higher than that in the control group, and the difference was significant (P 0.01). 3. The stability of prognosis was observed and the recurrence rate of the treatment group was lower than that of the control group (P0.05). Conclusion: Tiangui decoction combined with acupuncture in the treatment of luteal insufficiency of kidney deficiency and liver-depression type has definite clinical effect in early stage and is superior to control group. The mechanism of its curative effect is to regulate the homeostasis of any axon of the uterus of one day a day of kidney, so as to regulate menstrual cycle effectively. Improve the symptoms, increase serum hormone levels, restore luteal function, and long-term efficacy, high safety.
