【摘要】：目的評價枳實通降顆粒對結(jié)直腸腫瘤患者術(shù)后胃腸功能恢復及術(shù)后早期炎性腸梗阻防治的有效性及安全性。方法采用多中心前瞻性隨機雙盲對照方法,將結(jié)直腸腫瘤手術(shù)患者197例分為試驗組98例,對照組99例。兩組患者圍手術(shù)期均給予常規(guī)治療,試驗組于術(shù)后6~12 h開始少量多次口服或經(jīng)胃管給予枳實通降顆粒(制成50 ml藥液),每日1劑。對照組僅口服等量安慰劑。記錄患者腸鳴音恢復時間、首次排氣時間、首次排便時間、首次進食固體食物時間、住院天數(shù)、腸梗阻發(fā)生情況,并記錄不良反應(yīng)及并發(fā)癥發(fā)生情況,觀察安全性指標變化。結(jié)果試驗組術(shù)后腸鳴音恢復時間、首次排氣時間、首次排便時間及首次進食固體食物時間均較對照組明顯縮短(P0.01)。對照組6例發(fā)生術(shù)后早期炎性腸梗阻,試驗組4例。試驗組平均住院時間(14.00±0.82)天,顯著短于對照組的(19.33±3.98)天(P0.05)。兩組并發(fā)癥及肝腎功能異常發(fā)生情況比較,差異均無統(tǒng)計學意義(P0.05)。術(shù)后第5天試驗組降鈣素原下降較對照組明顯(P0.05)。結(jié)論枳實通降顆�？纱龠M結(jié)直腸腫瘤胃腸道手術(shù)后患者胃腸功能恢復,并對術(shù)后早期炎性腸梗阻有一定的防治作用,且安全性好。
[Abstract]:Objective to evaluate the efficacy and safety of Zhishi Tongjiang granule in the prevention and treatment of postoperative gastrointestinal function and early postoperative inflammatory intestinal obstruction in patients with colorectal cancer. Methods 197 patients undergoing colorectal tumor surgery were divided into two groups (n = 98) and control group (n = 99). Patients in both groups were given routine treatment during the perioperative period. In the experimental group, a small amount of oral or gastric tube administration of Aurantium aurantii Tongjiang granules (made into 50 ml solution) was given at 6 and 12 hours after operation, once a day. The control group only took the same amount of placebo. The recovery time, the first exhaust time, the first defecation time, the first time of eating solid food, the days of hospitalization and the incidence of intestinal obstruction were recorded. The adverse reactions and complications were recorded, and the changes of safety indexes were observed. Results compared with the control group, the recovery time, the first exhaust time, the first defecation time and the first time of eating solid food in the experimental group were significantly shorter than those in the control group (P0.01). Early postoperative inflammatory intestinal obstruction occurred in 6 cases in the control group and 4 cases in the experimental group. The average hospital stay in the trial group was (14.00 鹵0.82) days, significantly shorter than that in the control group (19.33 鹵3.98) days (P0.05). There was no significant difference in the incidence of complications and abnormal liver and kidney function between the two groups (P0.05). The level of procalcitonin in the trial group was significantly lower than that in the control group on the 5th day after operation (P0.05). Conclusion Zhishitongjiang granule can promote the recovery of gastrointestinal function in patients with colorectal cancer after gastrointestinal operation, and has a certain preventive and therapeutic effect on early postoperative inflammatory intestinal obstruction, and has good safety.
