【摘要】：目的:觀察加減易黃湯治療脾虛濕熱型復(fù)發(fā)性外陰陰道假絲酵母菌病的臨床療效,初步探討其作用機(jī)制。方法:選取脾虛濕熱型復(fù)發(fā)性外陰陰道假絲酵母菌病患者60例,隨機(jī)分為治療組和對(duì)照組,各30例。將治療過程分為強(qiáng)化階段、鞏固階段:強(qiáng)化階段治療組口服中藥湯劑加減易黃湯+克霉唑陰道片(陰道放置),對(duì)照組克霉唑陰道片(陰道放置),停藥3天后,真菌轉(zhuǎn)陰的患者進(jìn)入鞏固階段;鞏固階段治療組中藥湯劑口服+克霉唑陰道片(陰道放置),對(duì)照組克霉唑陰道片(陰道放置),每月月經(jīng)干凈3天后開始用藥7天,連續(xù)治療3個(gè)月經(jīng)周期。觀察兩組患者治療前后臨床癥狀療效、各陰道微生態(tài)指標(biāo)恢復(fù)情況及復(fù)發(fā)率。結(jié)果:臨床癥狀、體征療效情況比較,兩組患者治療后,治療組總有效率90%,對(duì)照組總有效率83%,差異有統(tǒng)計(jì)學(xué)意義(P0.05);臨床癥狀、體征積分情況比較,兩組臨床癥狀體征積分差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療組在改善臨床癥狀體征方面由于對(duì)照組;陰道菌群的密集度、陰道環(huán)境清潔度、陰道分泌物PH、H2O2比較分析,差異有統(tǒng)計(jì)學(xué)意義(P0.01或者P0.05);累計(jì)治療療程對(duì)比治療組在維持陰道微生態(tài)方面優(yōu)于對(duì)照組。結(jié)論:加減易黃湯治療脾虛濕熱型復(fù)發(fā)性外陰陰道假絲酵母菌病療效確切,改善臨床癥狀同時(shí)在恢復(fù)及維持陰道微生態(tài)環(huán)境方面具有明顯優(yōu)勢(shì)。
[Abstract]:Objective: to observe the clinical effect of Jiajianyihuang decoction on recurrent vulvovaginal candidiasis of spleen deficiency damp-heat type, and to explore its mechanism. Methods: 60 patients with recurrent vulvovaginal candidiasis were randomly divided into treatment group and control group. The treatment process was divided into strengthening stage, consolidation stage: the treatment group took oral Chinese traditional medicine decoction plus Yihuangtang clotrimazole vaginal tablet (vaginal placement), the control group clotrimazole vaginal tablet (vaginal placement), 3 days after the withdrawal of drugs, In the treatment group, clotrimazole vaginal tablet (vaginal placement) was taken orally, while in the control group clotrimazole vaginal tablet (vaginal placement) began to be used for 7 days after 3 days of menstruation. Continuous treatment for 3 menstrual cycles. The clinical symptoms, recovery and recurrence rate of each vaginal microecological index were observed before and after treatment. Results: the clinical symptoms and signs were compared. After treatment, the total effective rate of the treatment group was 90%, the total effective rate of the control group was 833%, the difference was statistically significant (P0.05), the scores of clinical symptoms and signs were compared. Two groups of clinical symptoms and signs score difference was statistically significant (P0.05), the treatment group in the improvement of clinical symptoms and signs due to the control group, vaginal flora density, vaginal environment cleanliness, vaginal secretion PH,H2O2 comparative analysis, The difference was statistically significant (P0.01 or P0.05); the treatment group was superior to the control group in maintaining vaginal microecology. Conclusion: Yi-Huang decoction is effective in treating recurrent vulvovaginal candidiasis of spleen deficiency and damp-heat type, and the improvement of clinical symptoms has obvious advantages in recovering and maintaining the vaginal microecological environment at the same time.
