【摘要】：目的:觀察針刺聯(lián)合中藥治療外傷性視神經(jīng)萎縮的臨床療效方法:采用平行隨機(jī)對(duì)照研究的方法,選擇符合納入標(biāo)準(zhǔn)的外傷性視神經(jīng)萎縮患者40例(46只眼),隨機(jī)分為治療組和對(duì)照組,對(duì)照組予以甲鈷胺口服,治療組在對(duì)照組的基礎(chǔ)上予以針刺配合逍遙四物湯口服。各組均以30天為一個(gè)療程進(jìn)行觀察,隨訪一個(gè)月。觀察入組時(shí)、療程結(jié)束后視力、視野、F-VEP、OCT及評(píng)分的改變。結(jié)果:觀察最終病例數(shù)為40人(46只眼),實(shí)驗(yàn)組及對(duì)照組各20人(23只眼)。(1)兩組基線資料比較分析:性別、年齡、眼別、病程、及各項(xiàng)檢查與評(píng)分之間均無(wú)差異(p0.05)。(2)各項(xiàng)主要指標(biāo)治療前后兩組組內(nèi)比較:治療前后治療組的視力、視野改善(p0.01),對(duì)照組無(wú)顯著差異(p0.05)。治療后治療組在頭痛、眼痛、失眠減輕(p0.01);對(duì)照組頭痛、眼痛減輕(p0.05)。治療后兩組P波潛伏期縮短,振幅升高(p0.05)。治療前后平均RNFL值,治療組無(wú)顯著性差異(p0.05),對(duì)照組下降(p0.01)。結(jié)論:1.針刺聯(lián)合中藥逍遙四物湯口服能夠改善外傷性視神經(jīng)萎縮患者視力、視野,使P波潛伏期縮短、振幅升高,減緩損傷進(jìn)展速度,并能緩解其頭痛、眼痛、失眠癥狀,改善其氣滯血瘀情況,與單純使用甲鈷胺口服相比較,療效更為理想。2.針刺聯(lián)合中藥逍遙四物湯口服安全性良好。
[Abstract]:Objective: to observe the clinical efficacy of acupuncture combined with traditional Chinese medicine in the treatment of traumatic optic atrophy. Forty patients (46 eyes) with traumatic optic atrophy were randomly divided into treatment group and control group. The control group was given megalomide orally, and the treatment group was treated with acupuncture combined with Xiaoyaowu decoction on the basis of the control group. All groups were observed 30 days as a course of treatment and followed up for one month. Visual acuity, visual field and F-VEP Oct were observed at the end of the course of treatment. Results: there were 40 cases (46 eyes), 20 cases (23 eyes,). (1) in the experimental group and 20 cases in the control group. The baseline data of the two groups were compared and analyzed: sex, age, eye type, course of disease, and so on. There was no difference between the two groups before and after treatment (p0. 05). (2). The visual acuity and visual field were improved before and after treatment (p0. 01), but there was no significant difference in the control group (p0. 05). After treatment, headache, eye pain and insomnia were alleviated in the treatment group (p0.01), and headache and eye pain in the control group (p0.05). After treatment, P wave latency was shortened and amplitude increased in both groups (p0.05). There was no significant difference in average RNFL before and after treatment in the treatment group and the control group (p 0.01). Conclusion 1. Acupuncture combined with Xiaoyao Siwu decoction can improve the visual acuity and visual field of the patients with traumatic optic atrophy, shorten the latency of P wave, increase the amplitude, slow down the progress of injury, and alleviate the symptoms of headache, eye pain and insomnia. The effect of improving qi stagnation and blood stasis was better than that of mecoamin alone. Acupuncture combined with Chinese medicine Xiaoyao Siwu decoction oral safety is good.
