【摘要】：祖國醫(yī)學(xué)典籍中無慢性胃炎這一病名。但是慢性胃炎臨床上有胃脘部疼痛或脹滿、燒心、噯氣反酸、惡心嘔吐等一系列癥狀。根據(jù)這些癥狀將其歸屬于中醫(yī)學(xué)中“胃痛”、“胃痞”等范疇。祖國醫(yī)學(xué)認為慢性胃炎的病因有六淫之氣的侵襲、飲食及藥物的損害、憂思惱怒等情志失調(diào)、五臟的虛損、先天贏弱、正氣虧虛等原因。認為本病病變部位雖在胃,但尤與肝脾相關(guān)。肝與胃是木土乘克的關(guān)系,協(xié)調(diào)機體的氣機升降,協(xié)助胃消化與吸收。脾與胃二臟同居中焦,五行中皆歸于土臟,在陰陽劃分中脾屬陰,胃屬陽,一表一里,相互為用,共調(diào)中焦氣機,脾主提升,胃主通降,為調(diào)節(jié)氣機的樞紐。脾胃相互影響,一方患病多數(shù)涉及于另一方,或者二者同時出現(xiàn)病變。孫浩老師認為慢性胃炎發(fā)病與正氣虧虛有關(guān)。祖國醫(yī)學(xué)所講的人體正氣,就是人的抗病機能,是由先天腎精與后天的脾胃之氣結(jié)合而成,因此正氣與脾胃密切相關(guān),脾氣虧虛、脾不散精,正氣不充,而發(fā)本病。正如《內(nèi)經(jīng)》所言：“邪之所湊,其氣必虛”。因此孫浩老師認為脾氣虧虛是慢性胃炎諸多證候的病理基礎(chǔ),故脾氣虧虛是慢性胃炎發(fā)病的根本。脾虛氣虧虛日久,升降失常,運化失職,制水無權(quán),脾虛不能為胃行其水谷津液,水谷津液不行,停聚于胃而為痰濕。孫浩老師認痰濕瘀滯是慢性胃炎發(fā)病之標。孫浩老師認為慢性胃炎病機本質(zhì)是脾氣虧虛�！秲�(nèi)經(jīng)》云：“虛則補之”。故臨床治療慢性胃炎以補虛為第一大法。但脾胃又有痰、濕、瘀、滯等病理產(chǎn)物,若治療上不顧其實,使脾胃更加壅滯脹滿,致使脾胃之氣停滯不運,變生他證；慢性胃炎中焦雖有實邪,但正氣早虛,已不耐攻伐,孫浩老師認為“運脾法”能夠清除脾胃中實邪,而不傷脾胃,為慢性胃炎的治療大法之二,孫浩老師在慢性胃炎這一特點,提出了“補運結(jié)合”為為治療慢性胃炎法則,二法結(jié)合,標本同治。迅速使脾胃恢復(fù)健運的功能。孫浩老師在治療本病時常用炙黃芪、太子參和炒白術(shù)等甘溫平和藥物補益脾氣。孫浩老師認為只要祛除脾胃病理產(chǎn)物的且不損傷脾胃的藥物皆是運脾藥,如用蘇梗、陳皮、枳殼等藥理脾胃氣滯；用炒山楂、神曲、炒麥芽、雞內(nèi)金等藥物消脾胃食積；用砂仁、陳皮等清中焦痰濕藥物。有瘀血者,用丹參、三七等藥化中焦瘀血。孫浩老師根據(jù)慢性胃炎的病因病機,及用藥特點,制定出益氣運脾湯(炙黃芪15g、太子參12g、茯苓10g、枳殼8g、陳皮10g、白術(shù)10g、砂仁5g、神曲10g、麥芽10g、炙甘草8g)治療慢性胃炎,也是治療脾胃虛弱證的基本方劑。此方療效較好、價格低廉,服用無不良反應(yīng),能取得滿意的結(jié)果。目的：本課題通過運用孫浩老師益氣運脾湯加減治療脾胃虛弱型慢性胃炎,觀察臨床各方面療效、主要癥狀改善情況以及幽門螺桿菌轉(zhuǎn)陰率,探討本方在治療脾胃虛弱型慢性胃炎中的作用,為本病提供新的治療方法。方法：入選2014年06月-2015年05月在江蘇省儀征市中醫(yī)院就診的門診及住院病例,參加試驗患者有60人,隨機分治療組和對照組,每組30人。對照組用西藥：胃炎沖劑,一次一包,3次/天,共28天。若上腹部飽脹、惡心欲吐,加莫沙比利5mg,3次/天。治療組用孫氏益氣運脾湯(炙黃芪15g、太子參12g、茯苓10g、枳殼8g、陳皮10g、白術(shù)10g、砂仁5g、神曲10g、麥芽10g、炙甘草8g),每日1劑,水煎,400m1,分兩次,早、中飯后服用,連續(xù)服用28天,氣滯者加香附、柴胡,惡心欲吐者加竹茹、半夏,反酸者加烏賊骨、大貝母,疼痛明顯加元胡、檀香。兩組均加用麗珠胃三聯(lián)口服7天。治療前及結(jié)束后分別通過四診合參收集癥狀、體征及快速14C呼氣試驗結(jié)果,并進行中醫(yī)證候積分,使用SPSS19.0軟件進行中醫(yī)療效統(tǒng)計分析。根據(jù)觀察指標及數(shù)據(jù)的不同,分別采用X2檢驗、t檢驗和秩和檢驗。結(jié)果：(1)在中醫(yī)證候積分變化方面,兩組治療后中醫(yī)證候積分降低均值比較有顯著差異(P0.05)。(2)兩組幽門螺桿菌轉(zhuǎn)陰情況,治療組28例,對照組20例,幽門螺桿菌轉(zhuǎn)陰率治療組93.33%明顯高于對照組66.67%,差異有統(tǒng)計學(xué)意義(P0.05)。(3)本研究中治療組有5例治愈,14例有顯著療效,9例有效,無效2例,對照組有2例治愈,14例有顯著療效,15例有效,7例無效。治療組臨床總有效93.33%,對照組總有效率為76.67%,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論：(1)孫氏益氣運脾湯治療脾胃虛弱型慢性胃炎的臨床總療效優(yōu)于西藥胃炎沖劑對照組。(2)孫氏益氣運脾湯對幽門螺桿菌有一定的殺滅或者抑制作用。(3)治療組在胃脘疼痛、痞脹等主要癥狀改善方面優(yōu)于對照組。(4)降低中醫(yī)證候積分方面,治療組更有優(yōu)勢。(5)孫氏益氣運脾湯具有益氣運脾、理氣和胃之效,切中慢性胃炎脾胃虛弱的病機,故治療脾胃虛弱型慢性胃炎的臨床療效良好,確能有效改善慢性胃炎的臨床癥狀體征。(6)本方用藥平和,治療未見不良反應(yīng)。說明孫氏益氣運脾湯能整體提高機體自身的免疫能力,提高抗病防病的能力,還有增加胃動力促進排空,抑制胃酸分泌,保護胃粘膜,保護胃黏膜屏障,改善局部微循環(huán)而促進細胞修復(fù)的作用。故孫氏益氣運脾湯方治療脾胃虛弱型慢性胃炎療效較好、且治療費用不高、長期使用無毒副作用。值得進一步研究和推廣應(yīng)用。
[Abstract]:There is no name for chronic gastritis in Chinese medical books. But chronic gastritis has a series of symptoms such as pain or fullness in the stomach, heartburn, belching, nausea and vomiting. Damage of diet and medicine, worry and anger, deficiency of five zang-organs, innate weakness of winning, deficiency of vital energy and so on. It is believed that the lesion of the disease is located in the stomach, but especially related to the liver and spleen. In the division of Yin and Yang, the spleen belongs to Yin, the stomach belongs to Yang, and the stomach belongs to one table and one interior. They are used for each other. They regulate the Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi-Qi The healthy qi of human body is the anti-disease function of human body, which is formed by the combination of congenital kidney essence and acquired Qi of spleen and stomach. Therefore, the healthy qi is closely related to spleen and stomach, deficiency of spleen-qi, non-dispersing essence of spleen, insufficiency of healthy qi, and pathogenesis. The pathogenesis of chronic gastritis is deficiency of spleen-qi for a long time, abnormal ascending and descending, irregularity of movement and dissolving, no right of water-making, spleen-deficiency can not be used for the stomach to run its valley fluid, water valley fluid can not be used, stop in the stomach and become phlegm-dampness. The essence of chronic gastritis is deficiency of spleen and stomach. Teacher Sun Hao believes that the method of "transporting the spleen" can clear away the pathogenic substances in the spleen and stomach without injuring the spleen and stomach. It is the second major method for the treatment of chronic gastritis. Teachers often use fried Astragalus root, prince ginseng and stir-fried Atractylodes macrocephala to replenish temper. Sun Hao believes that as long as the spleen and stomach pathological products and do not damage the spleen and stomach of the drugs are Spleen-activating drugs, such as stem, orange peel, orange peel and other pharmacological spleen and stomach Qi stagnation; stir-fried hawthorn, Shenqu, stir-fried malt, chicken Jin and other drugs Xiaopi stomach food. According to the etiology and pathogenesis of chronic gastritis, and the characteristics of medication, Professor Sun Hao formulated Yiqi Yunpi Decoction (roasted Astragalus 15g, Radix Pseudostellariae 12g, Poria 10g, Fructus aurantii 8g, tangerine peel 10g, Atractylodes Macrocephalae 10g, Radix Amomi 5g, Shenqu 10g, malt 10g, roasted licorice 8g). Chronic gastritis is also the basic prescription for the treatment of spleen and stomach weakness syndrome.This prescription has good curative effect,low price,no adverse reactions,and can achieve satisfactory results.Objective:To observe the clinical curative effect,main symptoms improvement and Helicobacter pylori(Hp) by using the modified decoction of replenishing qi and transporting spleen of Professor Sun Hao in the treatment of spleen and stomach weakness chronic gastritis. Methods: From June 2014 to May 2015 in Yizheng Hospital of Traditional Chinese Medicine in Jiangsu Province, 60 patients participated in the trial were randomly divided into treatment group and control group, 30 in each group. Medicines: gastritis granule, a package, three times a day, a total of 28 days. If the upper abdomen is full, nausea, plus Mosapride 5mg, three times a day. The treatment group with Sun's Yiqi Yunpi Decoction (fried Astragalus 15g, prince's ginseng 12g, Poria cocos 10g, orange shell 8g, orange peel 10g, Atractylodes macrocephala 10g, Shenqu 10g, malt 10g, fried licorice 8g), one dose a day, water decoction, 400m1, twice a day, morning, after lunch. Two groups were given Lizhu stomach triple orally for 7 days. Symptoms, signs and rapid 14C breath test results were collected by four diagnostic methods before and after treatment, and TCM syndromes were carried out. Results: (1) In the aspect of the change of TCM syndrome integral, there was a significant difference between the two groups (P 0.05). (2) Helicobacter pylori negative conversion between the two groups. The negative rate of Helicobacter pylori was 93.33% in the treatment group and 66.67% in the control group. The difference was statistically significant (P 0.05). (3) In this study, 5 cases were cured, 14 cases were significantly effective, 9 cases were effective, 2 cases were ineffective, 2 cases were cured in the control group, 14 cases were effective, 7 cases were ineffective. The total effective rate was 93.33%. The total effective rate was 76.67%. The difference was statistically significant (P (5) Sun's Yiqi Yunpi Decoction has the effect of Invigorating Qi and transporting spleen, regulating qi and stomach. It can cut the pathogenesis of spleen and stomach weakness in chronic gastritis, so it has good clinical effect in treating spleen and stomach weakness chronic gastritis and can effectively improve chronic gastritis. The results showed that Sun's Yiqi Yunpi Decoction could improve the immunity of the body as a whole, enhance the ability of disease resistance and prevention, increase gastric motility to promote emptying, inhibit gastric acid secretion, protect gastric mucosa, protect gastric mucosal barrier, improve local microcirculation and promote cell repair. Therefore, Sun's Yiqi Yunpi Decoction is effective in treating chronic gastritis of weak spleen and stomach, and the treatment cost is not high, long-term use of non-toxic side effects.
