【摘要】：目的探討中藥熏蒸聯(lián)合針灸對老年腎虛寒濕型強直性脊柱炎(AS)臨床癥狀及炎癥因子的影響。方法老年腎虛寒濕型AS患者120例,按隨機數(shù)字表法分為對照組、中藥組、針灸組及中藥針灸組各30例。對照組給予常規(guī)西藥治療,中藥組給予常規(guī)西藥聯(lián)合中藥熏蒸治療,針灸組給予常規(guī)西藥聯(lián)合針灸治療,中藥針灸組給予常規(guī)西藥聯(lián)合中藥熏蒸及針灸治療,連續(xù)治療3個療程。評價并比較4組治療后的臨床療效。觀察并比較4組治療前后指地距、枕墻距、晨僵時間、Sehober試驗及腰背痛視覺模擬評分(VAS)。檢測并比較4組治療前后紅細胞沉降率(ESR)以及血清C反應(yīng)蛋白(CRP)、白細胞介素(IL)-23及巨噬細胞移動抑制因子(MIF)水平。結(jié)果治療后,中藥組、針灸組及中藥針灸組總有效率均明顯高于對照組,且中藥針灸組明顯高于中藥組和針灸組(P0.05)。治療后中藥組、針灸組及中藥針灸組指地距、枕墻距、晨僵時間及腰背痛VAS均明顯低于對照組,且中藥針灸組均明顯低于中藥組和針灸組,Sehober試驗評分明顯高于對照組(P0.05);治療后且中藥針灸組評分明顯高于中藥組和針灸組(均P0.05)。治療后中藥組、針灸組及中藥針灸組ESR及血清CRP、IL-23、MIF水平均明顯低于對照組,且中藥針灸組明顯低于中藥組和針灸組(均P0.05)。結(jié)論中藥熏蒸聯(lián)合針灸治療老年腎虛寒濕型AS臨床療效顯著,能夠明顯改善患者臨床癥狀,降低炎癥因子水平。
[Abstract]:Objective to investigate the effect of Chinese medicine fumigation combined with acupuncture on the clinical symptoms and inflammatory factors of senile ankylosing spondylitis with kidney-deficiency cold and dampness type. Methods 120 elderly AS patients with kidney deficiency and cold dampness were randomly divided into control group, Chinese medicine group, acupuncture and moxibustion group and acupuncture and moxibustion group. The control group was treated with conventional western medicine, the traditional Chinese medicine group with traditional Chinese medicine fumigation, the acupuncture group with routine western medicine combined with acupuncture and moxibustion, and the traditional Chinese medicine acupuncture group with routine western medicine combined with traditional Chinese medicine fumigation and acupuncture treatment. Three consecutive courses of treatment. To evaluate and compare the clinical effect of 4 groups after treatment. Observe and compare the digital distance, occipital-wall distance, morning stiffness time and visual analogue score of low back pain (VAS).) before and after treatment in 4 groups. Erythrocyte sedimentation rate (ESR), serum C-reactive protein (CRP),) interleukin-23 (IL-23) and macrophage migration inhibitory factor (MIF) were measured and compared before and after treatment. Results after treatment, the total effective rate of TCM group, acupuncture group and acupuncture group were significantly higher than that of control group, and the total effective rate of TCM acupuncture group was significantly higher than that of traditional Chinese medicine group and acupuncture group (P0.05). After treatment, the VAS of TCM group, acupuncture group and traditional Chinese medicine acupuncture group were significantly lower than that of control group, such as the distance of ground, the distance of occipital wall, the time of morning stiffness and the VAS of low back pain. The score of Sehober test in acupuncture and moxibustion group was significantly higher than that in control group (P0.05), and the score of acupuncture and moxibustion group after treatment was significantly higher than that in traditional Chinese medicine group and acupuncture group (P0.05). After treatment, the levels of ESR and serum CRP,IL-23,MIF in TCM group, acupuncture group and TCM acupuncture group were significantly lower than those in control group, and the levels of serum CRP,IL-23,MIF in TCM acupuncture group were significantly lower than those in traditional Chinese medicine group and acupuncture group (P0.05). Conclusion Chinese herbal fumigation combined with acupuncture in the treatment of AS with kidney deficiency and cold dampness can significantly improve the clinical symptoms and reduce the level of inflammatory factors.
