【摘要】：目的：通過觀察舒筋外洗顆粒配合功能康復(fù)訓(xùn)練對(duì)前交叉韌帶(Anterior Cruciate Ligament, ACL)重建術(shù)后膝關(guān)節(jié)功能康復(fù)的影響,探討分析此制劑的臨床療效及其作用機(jī)理,為ACL重建術(shù)后膝關(guān)節(jié)功能的快速康復(fù)找到一種簡(jiǎn)便廉驗(yàn)的方法。方法：于2015年4月至2016年1月期間,從廣州中醫(yī)藥大學(xué)第三附屬醫(yī)院選取符合納入標(biāo)準(zhǔn)的60例ACL斷裂患者。采用隨機(jī)數(shù)字表法,將60例患者隨機(jī)分為治療組和對(duì)照組,每組各30例。治療組在傷口愈合后接受為期4周的舒筋外洗顆粒熏洗療法配合康復(fù)訓(xùn)練,而對(duì)照組僅給予相同的康復(fù)訓(xùn)練。觀察記錄兩組患者在ACL重建術(shù)后2周、4周、6周及12周時(shí)患膝腫脹程度、VAS疼痛評(píng)分、膝關(guān)節(jié)活動(dòng)度(ROM)、Lysholm膝關(guān)節(jié)評(píng)分,記錄患者手術(shù)后重返工作崗位所需時(shí)間,收集數(shù)據(jù)并進(jìn)行統(tǒng)計(jì)分析。結(jié)果：1、兩組患者的一般資料及術(shù)后2周時(shí)患膝腫脹程度、VAS疼痛評(píng)分、ROM、Lysholm膝關(guān)節(jié)評(píng)分差異均無統(tǒng)計(jì)學(xué)意義(P0.05),治療組和對(duì)照組具有可比性；2、術(shù)后4周和6周,兩組患者之間的患膝腫脹程度、VAS疼痛評(píng)分、ROM、Lysholm膝關(guān)節(jié)評(píng)分比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05),治療組優(yōu)于對(duì)照組；3、術(shù)后12周兩組間各項(xiàng)觀察指標(biāo)經(jīng)檢驗(yàn)差異均無統(tǒng)計(jì)學(xué)意義(P0.05)；4、治療組術(shù)后重返工作崗位所需時(shí)間少于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)；5、兩組患者在整個(gè)研究過程中均無不良反應(yīng)。結(jié)論：舒筋外洗顆粒熏洗療法配合功能康復(fù)訓(xùn)練能減輕ACL重建術(shù)后的膝關(guān)節(jié)腫脹、疼痛,增加膝關(guān)節(jié)活動(dòng)度,改善膝關(guān)節(jié)功能,進(jìn)而縮短術(shù)后康復(fù)時(shí)間。此康復(fù)方案實(shí)用安全、療效確切,值得在臨床上推廣。
[Abstract]:Objective: to observe the effect of Shujin Ouxi granule combined with functional rehabilitation training on the functional rehabilitation of knee joint after anterior cruciate ligament (ACL) (Anterior Cruciate Ligament, ACL) reconstruction, and to explore the clinical effect and mechanism of this preparation. To find a simple and inexpensive method for rapid rehabilitation of knee joint function after ACL reconstruction. Methods: from April 2015 to January 2016, 60 patients with ACL breakage were selected from the third affiliated Hospital of Guangzhou University of traditional Chinese Medicine. 60 patients were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group received 4 weeks of fumigation therapy combined with rehabilitation training after wound healing, while the control group only received the same rehabilitation training. To observe and record the degree of swelling of knee and the knee joint score of (ROM) and Lysholm after ACL reconstruction, and to record the time needed to return to work after operation, and to observe and record the degree of swelling of knee and the score of knee joint in two groups at 2 weeks, 4 weeks, 6 weeks and 12 weeks after ACL reconstruction. Collect data and make statistical analysis. Results there was no significant difference in general data and VAS pain score and Lysholm knee score between the two groups at 2 weeks after operation (P0.05). There was no significant difference between the treatment group and the control group at 4 and 6 weeks after operation. There were significant differences in VAS pain score and Lysholm knee score between the two groups (P0.05). The treatment group was better than the control group (P < 0.05). There was no significant difference in the observation indexes between the two groups 12 weeks after operation (P0.05). 4. The time needed to return to work after operation in the treatment group was less than that in the control group, the difference was statistically significant (P0.05). There were no adverse reactions in the whole course of the study between the two groups. Conclusion: the fumigation and washing therapy combined with functional rehabilitation training can reduce the swelling and pain of knee joint after ACL reconstruction, increase the range of knee motion, improve the function of knee joint, and then shorten the time of postoperative rehabilitation. This rehabilitation program is practical, safe and effective. It is worth popularizing in clinic.
