【摘要】：目的：應(yīng)用醒脾益腎方治療胡桃夾綜合征脾腎兩虛證的患者,對比患者治療前后的癥狀、體征及相關(guān)實(shí)驗(yàn)室指標(biāo)變化,觀察該方的臨床療效,為中醫(yī)藥治療胡桃夾綜合征提供臨床指導(dǎo)。方法：選取30例西醫(yī)診斷為胡桃夾綜合征,且中醫(yī)辨證為脾腎兩虛證的患者,采用治療前后自身對照的研究觀察方法,應(yīng)用醒脾益腎方治療,總療程為6個月。對比治療前與治療后患者中醫(yī)證候積分、尿紅細(xì)胞高倍鏡下計(jì)數(shù)、24小時尿蛋白定量、腹主動脈和腸系膜上動脈夾角處的左外側(cè)左腎靜脈血管內(nèi)徑(mm)與腸系膜上動脈和腹主動脈夾角處的左側(cè)腎靜脈血管內(nèi)徑(mm)的比值等臨床數(shù)據(jù)。結(jié)果：1.治療6個月后中醫(yī)證候積分的總有效率達(dá)86.7%。中醫(yī)證候積分符合正態(tài)性,采用配對樣本t檢驗(yàn),t=13.202,P0.01,有顯著差異,具有統(tǒng)計(jì)學(xué)意義。2.尿紅細(xì)胞高倍鏡下計(jì)數(shù)、24小時尿蛋白定量值均符合正態(tài)性,治療3個月、6個月后兩組尿紅細(xì)胞高倍鏡下計(jì)數(shù)、24小時尿蛋白定量較治療前均降低(P0.01),差異具有統(tǒng)計(jì)學(xué)意義。3.泌尿系彩超LRV內(nèi)徑比值符合正態(tài)性,治療6個月后LRV內(nèi)徑比值明顯低于用藥前LRV內(nèi)徑比值(P0.01),差異具有統(tǒng)計(jì)學(xué)意義。結(jié)論：1應(yīng)用醒脾益腎方可有效改善胡桃夾綜合征(脾腎兩虛)患者的中醫(yī)癥狀。2應(yīng)用醒脾益腎方可降低胡桃夾綜合征(脾腎兩虛)患者尿蛋白、尿潛血。3應(yīng)用醒脾益腎方可在一定程度緩解左腎靜脈受壓。
[Abstract]:Objective: to observe the clinical effect of Xingpi Yishen prescription in treating the patients with deficiency of spleen and kidney in Nutcracker syndrome, comparing the symptoms, signs and related laboratory indexes before and after treatment. To provide clinical guidance for the treatment of nutcracker syndrome with traditional Chinese medicine. Methods: 30 cases of Nutcracker syndrome diagnosed by western medicine and syndrome differentiation of spleen and kidney deficiency in TCM were selected. The self-control study and observation before and after treatment were carried out, and the total course of treatment was 6 months. The scores of TCM syndromes before and after treatment were compared, and the urine red blood cells were counted under high power microscope to quantify 24 hours urine protein. The ratio of the diameter of left lateral renal vein (mm) at the angle between abdominal aorta and superior mesenteric artery to that of left renal vein (mm) at the angle of superior mesenteric artery and abdominal aorta. The result is 1: 1. After 6 months of treatment, the total effective rate of TCM syndromes integral was 86.7%. The integral of TCM syndromes accorded with normality, and the matched sample t test was used to test 13.202% P0.01.The difference was statistically significant. The quantitative values of urinary protein in 24 hours under high magnification microscope were in accordance with normality. After 3 months of treatment, the 24 hour urine protein in the two groups was lower than that before treatment (P0.01), the difference was statistically significant (P 0.01). The ratio of LRV diameter in urography was normal. After 6 months of treatment, the ratio of LRV diameter was significantly lower than that of LRV before treatment (P0.01), and the difference was statistically significant. Conclusion the application of "Xianpi Yishen decoction" to improve the symptoms of traditional Chinese medicine in the patients with nutcracker syndrome (deficiency of spleen and kidney). 2. The application of Qianpi Yishen decoction can reduce urine protein in patients with nutcracker syndrome (deficiency of spleen and kidney). Urine occult blood .3 was used to relieve the pressure of left renal vein to a certain extent.
