【摘要】：目的：通過(guò)研究“神闕八陣”針刺和常規(guī)針刺治療氣虛型功能性便秘,比較兩種療法的臨床療效、安全性和依從性。方法：采用隨機(jī)、對(duì)照、單盲的研究方法,將符合納入標(biāo)準(zhǔn)的72例患者,隨機(jī)分為“神闕八陣”針刺治療組和常規(guī)針刺對(duì)照組,每組36例。兩組患者均連續(xù)治療3個(gè)療程,每個(gè)療程治療5天,間隔2天,并于治療結(jié)束后的第4周隨訪(fǎng)。觀察兩組患者治療前、治療后及治療后第4周隨訪(fǎng)的周排便次數(shù)、癥狀自評(píng)量表評(píng)分、生活質(zhì)量自評(píng)量表評(píng)分及氣虛型癥狀分級(jí)量化表評(píng)分的變化,以判定療效。結(jié)果：1.兩組在性別、年齡、年齡段分布、病程及病情嚴(yán)重程度等各方面均衡性較好(p0.05),基線(xiàn)水平可比。2.兩組治療前后比較：周排便次數(shù)、癥狀自評(píng)量表評(píng)分、生活質(zhì)量自評(píng)量表評(píng)分及氣虛型癥狀分級(jí)量化表評(píng)分均有顯著性差異(p0.01)。3.兩組3個(gè)療程結(jié)束后比較：周排便次數(shù)結(jié)果比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)；癥狀自評(píng)量表評(píng)分、生活質(zhì)量自評(píng)量表評(píng)分、氣虛型癥狀分級(jí)量化表評(píng)分結(jié)果比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。4.兩組治療結(jié)束后第4周隨訪(fǎng)比較：周排便次數(shù)結(jié)果比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)；癥狀自評(píng)量表評(píng)分、生活質(zhì)量自評(píng)量表評(píng)分、氣虛型癥狀分級(jí)量化表評(píng)分結(jié)果比較,差異有統(tǒng)計(jì)學(xué)意義(p0.05)。5.總有效率：“神闕八陣”針刺組的總有效率為85.71%,常規(guī)針刺組的總有效率為75.76%,兩組差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)。6.兩組的安全性及依從性比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05)。結(jié)論：1.“神闕八陣”針刺和常規(guī)針刺兩種療法對(duì)氣虛型功能性便秘均有較好的療效。2.“神闕八陣”針刺法改善氣虛型功能性便秘患者的臨床癥狀及生活質(zhì)量的近期、遠(yuǎn)期療效均優(yōu)于常規(guī)針刺法；“神闕八陣”針刺法改善氣虛型功能性便秘患者的周排便次數(shù)的近期、遠(yuǎn)期療效均與常規(guī)針刺法相當(dāng)。3.“神闕八陣”針刺法和常規(guī)針刺法的安全性及依從性均較好。4.“神闕八陣”針刺法較常規(guī)針刺法取穴精簡(jiǎn),操作方便,痛感低,值得臨床推廣。
[Abstract]:Objective: to compare the clinical efficacy, safety and compliance of "Shenque Bachan" acupuncture and routine acupuncture in the treatment of functional constipation with deficiency of qi. Methods: a randomized, controlled and single-blind study was conducted. 72 patients were randomly divided into "Shenque eight Array" acupuncture treatment group and routine acupuncture control group (36 cases in each group). The patients in the two groups were treated for 3 consecutive courses of treatment for 5 days, with an interval of 2 days, and followed up for 4 weeks after the end of the treatment. The changes of weekly defecation frequency, symptom checklist scale (SCL-90), quality of life (QOL) and Qi-deficiency symptom scale (QDS) were observed before, after and 4 weeks after treatment in order to evaluate the curative effect. The result is 1: 1. The gender, age distribution, course of disease and severity of the disease were well balanced between the two groups (p0.05), and the baseline level was equal to. 2. 2. There were significant differences between the two groups before and after treatment: weekly defecation frequency, symptom checklist scale score, quality of life self-rating scale score and Qi-deficiency syndrome rating scale score (p0.01) .3. There was no significant difference between the two groups after three courses of treatment: the results of weekly defecation were not significantly different (p0.05), the scores of symptom Checklist, quality of Life scale and Qi-deficiency symptom scale were compared. The difference was statistically significant (p0.05). 4. There was no significant difference in the frequency of defecation between the two groups (p0.05), the scores of symptom Checklist, quality of Life scale (QOL) and Qi-deficiency symptom scale (QQS) were compared between the two groups after the end of the treatment, and there was no significant difference between the two groups in the frequency of defecation (p0.05). The difference was statistically significant (p0.05). Total effective rate: the total effective rate of acupuncture group was 85.71, that of routine acupuncture group was 75.76.The difference between the two groups was not statistically significant (p0.05) .6. There was no significant difference in safety and compliance between the two groups (p0.05). Conclusion 1. Both "Shenque eight Array" acupuncture and routine acupuncture have good curative effect on functional constipation of qi deficiency type. "Shenque eight arrays" acupuncture method to improve the clinical symptoms and quality of life of patients with functional constipation of Qi-deficiency type in the near future, long-term results are better than conventional acupuncture; "Shenque eight arrays" acupuncture method to improve the short term defecation frequency of patients with functional constipation with deficiency of qi, the long-term effect is equal to that of routine acupuncture. 3. The safety and compliance of "Shenque eight array" acupuncture method and conventional acupuncture method are good. 4. The "Shenque eight array" acupuncture method is simpler than the conventional acupuncture method, easy to operate and low in pain, so it is worth popularizing in clinic.
