【摘要】：目的:應(yīng)用加味二陳湯治療冠心病穩(wěn)定型心絞痛(痰濁內(nèi)阻型)患者,通過治療前后的對比,觀察加味二陳湯對患者心肌缺血總負(fù)荷及血清同型半胱氨酸的影響。方法:本次臨床觀察的42例患者來自黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院門診部,現(xiàn)代醫(yī)學(xué)診斷為冠心病穩(wěn)定型心絞痛,中醫(yī)診斷為胸痹痰濁內(nèi)阻型,對患者給以加味二陳湯,治療時間為4周,觀察治療前后患者中醫(yī)證候積分、動態(tài)心電心肌缺血總負(fù)荷、血清同型半胱氨酸(Hcy)、硝酸甘油使用量的變化,采用SPSS19.0進(jìn)行統(tǒng)計學(xué)分析。結(jié)果:1.中醫(yī)證候總積分及有效率:治療后患者的中醫(yī)證候總積分較治療前患者的總積分降低,其差異具有統(tǒng)計學(xué)意義(P0.05)。2.24小時心肌缺血總負(fù)荷:治療后患者的心肌缺血總負(fù)荷較治療前患者的心肌缺血總負(fù)荷降低,其差異具有統(tǒng)計學(xué)意義(p0.05)。3.血清同型半胱氨酸比較:治療后患者的血清同型半胱氨酸較治療前降低,其差異具有統(tǒng)計學(xué)意義(P0.05)。4.硝酸甘油使用情況:硝酸甘油停減率83.33%。5.總有效率:患者經(jīng)治療后,顯效12例,有效26例,總有效率為90.48%。結(jié)論:1.加味二陳湯可以降低冠心病心絞痛痰濁內(nèi)阻型患者的中醫(yī)證候總積分,改善其中醫(yī)證候。2.加味二陳湯可以降低冠心病心絞痛痰濁內(nèi)阻型患者的24小時動態(tài)心電圖心肌缺血總負(fù)荷。3.加味二陳湯可以降低冠心病心絞痛痰濁內(nèi)阻型患者的血清同型半胱氨酸水平。
[Abstract]:Objective: To observe the effect of Jiawei two Chen Decoction on the total load of myocardial ischemia and the serum homocysteine in patients with stable angina pectoris (turbid phlegm resistance type) of coronary heart disease (turbid phlegm resistance type) before and after treatment. Methods: 42 patients from this clinical observation came from the outpatient department of the First Affiliated Hospital of Heilongjiang University Of Chinese Medicine, Modern medicine was diagnosed as stable angina pectoris of coronary heart disease. Chinese medicine was diagnosed as internal obstruction of chest obstruction and phlegm turbidity. The treatment time was 4 weeks for patients. The score of TCM syndrome, dynamic myocardial ischemia total load, serum homocysteine (Hcy), use of nitroglycerin before and after treatment were observed before and after treatment, and SPSS19.0 was adopted. Results: total score and efficiency of 1. TCM syndromes: the total integral of TCM syndrome after treatment was lower than that of the patients before treatment. The difference was statistically significant (P0.05).2.24 hour total load of myocardial ischemia: the total load of myocardial ischemia in patients after treatment was lower than that of the patients before treatment, and the difference between the total load and the total load of myocardial ischemia was lower than that of the patients before treatment. Statistical significance (P0.05).3. serum homocysteine comparison: the serum homocysteine of patients after treatment was lower than before treatment, the difference was statistically significant (P0.05).4. nitroglycerin use situation: Nitroglycerin reduction rate 83.33%.5. total efficiency: Patients after treatment, 12 cases, effective 26 cases, the total effective rate of 90.48%. knot Theory: 1. flavored two Chen decoction can reduce the total score of TCM syndrome in patients with angina pectoris of angina pectoris, improvement of TCM syndrome.2. plus flavour two Chen Tang can reduce the total load of myocardial ischemia in 24 hour dynamic electrocardiogram of patients with coronary heart disease and angina pectoris,.3. Jiawei Er Chen decoction can reduce the phlegm resistance of angina pectoris of coronary heart disease The serum level of homocysteine in the patient.
