【摘要】：目的:觀察運(yùn)用益腎活血降濁方治療糖尿病腎病對(duì)血清PTH水平的影響,繼而探討中西醫(yī)結(jié)合治療本病的優(yōu)勢(shì)和可行性,以及此法對(duì)因糖尿病腎病而引發(fā)的早期骨病的預(yù)防作用。方法:選擇糖尿病腎病Mogenson IV期患者60例,選用隨機(jī)數(shù)字表法將符合要求的60位患者平均分成兩組,治療組(蓋三淳+益腎活血降濁方組)30例,對(duì)照組(尿毒清+蓋三淳)30例,兩組患者均經(jīng)過(guò)一般治療控制血壓、血糖等,對(duì)照組服用尿毒清+蓋三淳,治療組服用蓋三淳外,加服以益腎活血降濁為治療原則的中藥方,服藥周期8周,分別評(píng)價(jià)治療前后患者臨床癥狀改善情況及各項(xiàng)檢查指標(biāo)(PTH、BUN、Scr、血鈣、血磷、血紅蛋白和24h尿蛋白等)。結(jié)果采用SPSS17.0進(jìn)行統(tǒng)計(jì)分析。P0.05,顯示兩組有差異,具有統(tǒng)計(jì)學(xué)意義。結(jié)果:經(jīng)臨床觀察及統(tǒng)計(jì)學(xué)分析,治療后中藥治療組在改善臨床癥狀、降低PTH、穩(wěn)定血糖、降低血肌酐、尿素氮、改善腎功、提高血紅蛋白含量改善貧血、減少尿蛋白等方面明顯優(yōu)于對(duì)照組,且P0.05,具有統(tǒng)計(jì)學(xué)意義,治療前后血、尿、便三大常規(guī),肝功,心電圖等無(wú)明顯不符合病情的變化。益腎活血降濁方治療糖尿病腎病及預(yù)防腎性骨病方面有明顯療效。結(jié)論:研究結(jié)果顯示益腎活血降濁方在降低血清PTH水平,糾正鈣磷代謝紊亂,預(yù)防腎性骨病的發(fā)生,降低尿毒素,改善腎功能,改善腎性貧血,降低尿蛋白,穩(wěn)定血糖及有效延緩病情的進(jìn)展等方面明顯優(yōu)于對(duì)照組,且安全性好。
[Abstract]:Objective: to observe the effect of Yishen Huoxue Jiangzhuo recipe on serum PTH level of diabetic nephropathy, and then to explore the advantages and feasibility of integrated traditional Chinese and western medicine in the treatment of this disease, and the preventive effect of this method on early bone disease caused by diabetic nephropathy. Methods: sixty patients with Mogenson IV stage diabetic nephropathy were randomly divided into two groups: the treatment group (Gesan Chun Yishen Huoxue Jiangzhuo) group (30 cases) and the control group (Miuduqing Qinggaicun 30 cases). The two groups were treated with general treatment to control blood pressure, blood sugar, etc. The control group was treated with UDQ and Gesan Chun, while the treatment group was treated with Gesanchun in addition to the traditional Chinese medicine prescription with the principle of tonifying kidney, activating blood circulation and lowering turbid. The treatment period was 8 weeks. To evaluate the improvement of clinical symptoms before and after treatment, and to evaluate the examination indexes (PTHU BUNScrs, blood calcium, serum phosphorus, hemoglobin and 24 hours urine protein, etc.). Results SPSS17.0 was used for statistical analysis. P 0.05, showing the difference between the two groups, with statistical significance. Results: after clinical observation and statistical analysis, the treatment group improved clinical symptoms, reduced PTH, stabilized blood glucose, reduced serum creatinine, urea nitrogen, improved renal function and hemoglobin content, and improved anemia. The decrease of urine protein was significantly better than that of the control group (P0.05). Before and after treatment, there were no significant changes in blood, urine, stool, liver function, electrocardiogram and so on. Yishen Huoxue Jiangzhuo recipe for diabetic nephropathy and the prevention of renal osteopathy has obvious effect. Conclusion: the results show that Yishen Huoxue Jiangzhuo prescription can reduce serum PTH level, correct calcium and phosphorus metabolism disorder, prevent renal osteopathy, decrease urinary toxin, improve renal function, improve renal anemia and reduce urinary protein. The stability of blood sugar and the delay of the progress of the disease were better than that of the control group, and the safety was good.
