【摘要】：背景:功能性胃腸病是臨床常見的慢性功能性疾病,其中的功能性消化不良具有較高發(fā)病率,是臨床常見病、多發(fā)病。隨著社會進(jìn)步和生活水平的提高,本病發(fā)病率具有明顯升高趨勢,且其發(fā)病與精神心理因素密切相關(guān),因此治療效果欠佳,反復(fù)發(fā)作,遷延難愈。臨床治療過程中給予對癥處理有時很難達(dá)到預(yù)期效果,而同時給予精神心理方面的干預(yù)治療,可能會有明顯效果。中醫(yī)藥在本病治療過程中有明顯優(yōu)勢,中醫(yī)藥對情志的調(diào)節(jié)歷史悠久,療效顯著,應(yīng)當(dāng)在本病治療中充分發(fā)揮優(yōu)勢,總結(jié)處方用藥規(guī)律,提高臨床療效。目的:通過對功能性消化不良伴焦慮狀態(tài)的病因病機(jī)及癥狀的研究和總結(jié),對病例進(jìn)行對照實驗設(shè)計,并進(jìn)行數(shù)據(jù)研究分析,驗證柴胡類方對功能性消化不良伴焦慮狀態(tài)的療效,總結(jié)臨床有效治療方法。方法:臨床收集符合標(biāo)準(zhǔn)的功能性消化不良伴有焦慮狀態(tài)的患者65例,根據(jù)臨床調(diào)查表,調(diào)查記錄患者的臨床癥狀積分及嚴(yán)重程度,對所選患者進(jìn)行隨機(jī)分組,分為對照組和實驗組;對照組給予常見西藥對癥處理,無情緒方面的治療,實驗組在對照組基礎(chǔ)上再給予柴胡類方進(jìn)行情緒的調(diào)節(jié)及臨床癥狀的控制;記錄實驗前后及治療組與實驗組之間的區(qū)別,通過對實驗數(shù)據(jù)及實驗結(jié)果進(jìn)行統(tǒng)計分析,得出相關(guān)結(jié)論。相關(guān)數(shù)據(jù)采用SPSS17.0統(tǒng)計軟件處理分析。結(jié)果:在本研究中,經(jīng)柴胡類方治療后,經(jīng)檢驗,單項證候積分如大便不暢、身重困倦、小便短黃、大便稀溏等較治療前無顯著性差異(P0.05),而對于胃脘疼痛、脹滿、噯氣反酸、飲食減少等癥狀的改善治療組與對照組之間有差異性,說明柴胡類方對于改善胃脘脹滿疼痛、噯氣反酸等代表性癥狀有良好效果;對于睡眠的影響,經(jīng)檢驗P0.05,治療前后對于改善睡眠情況差異具有統(tǒng)計學(xué)意義;對于癥狀積分的改善,中醫(yī)癥狀積分和SAS焦慮積分均顯示實驗組治療前后的差異性有統(tǒng)計學(xué)意義;最終本實驗結(jié)果顯示,實驗組經(jīng)柴胡類方加減治療后,臨床癥狀明顯減輕,其有效率達(dá)到93.33%,經(jīng)檢驗有統(tǒng)計學(xué)意義。結(jié)論:柴胡類方對于改善中醫(yī)臨床癥狀及焦慮狀態(tài)有良好療效,相比單純西藥對癥治療有明顯優(yōu)勢,其中既包括癥狀的減輕又包括情緒的調(diào)控。
[Abstract]:Background: functional gastrointestinal disease is a common chronic functional disease in clinic, and functional dyspepsia has a high incidence. With the progress of society and the improvement of living standard, the incidence of this disease is obviously increasing, and its incidence is closely related to mental and psychological factors, so the therapeutic effect is not good, repeated attacks are difficult to cure. In the course of clinical treatment, it is difficult to achieve the desired effect by giving symptomatic treatment, but at the same time, the psycho-psychological intervention may have obvious effect. Chinese medicine has obvious advantages in the course of treatment of this disease. Chinese medicine has a long history of regulating emotion and remarkable curative effect. We should give full play to the advantages in the treatment of this disease, sum up the law of prescribing drugs, and improve the clinical curative effect. Objective: to study and summarize the etiology, pathogenesis and symptoms of functional dyspepsia with anxiety. To verify the efficacy of Bupleurum decoction in the treatment of functional dyspepsia with anxiety, summarize the effective clinical treatment methods. Methods: 65 patients with functional dyspepsia accompanied with anxiety were collected. According to the clinical questionnaire, the clinical symptom score and severity of the patients were recorded, and the selected patients were randomly divided into two groups. The control group was divided into control group and experimental group, the control group was treated with common western medicine without emotional treatment, and the experimental group was treated with Bupleurum recipe on the basis of control group for emotional regulation and clinical symptom control. The differences between the experimental group and the experimental group before and after the experiment were recorded and the relevant conclusions were obtained by statistical analysis of the experimental data and the experimental results. The related data were analyzed by SPSS17.0 statistical software. Results: in this study, after treatment with Bupleurum recipe, after examination, the single syndrome score, such as stool unobstructed, heavy sleepiness, short yellow urine, loose stool and so on, had no significant difference (P0.05), but for epigastric pain, fullness, belching regurgitation, and so on, there was no significant difference between before treatment and so on (P0.05), but for stomachache, bloating, belching and regurgitating acid, and so on, There were differences between the treatment group and the control group in improving the symptoms such as diet reduction, which indicated that Chaihu recipe had a good effect on improving the typical symptoms such as stomachache, belching and regurgitation, and the effect on sleep. After testing P0.05, before and after treatment for the improvement of sleep has statistical significance, for the improvement of symptom score, TCM symptom score and SAS anxiety score show the difference between the experimental group before and after treatment has statistical significance. Finally, the experimental group was treated with Bupleurum decoction, the clinical symptoms were obviously alleviated, and the effective rate reached 93.33, which was statistically significant. Conclusion: Bupleurum prescription has a good effect on improving the clinical symptoms and anxiety state of traditional Chinese medicine, which has obvious advantages over western medicine alone in the treatment of symptoms, including both symptom relief and emotional regulation.
