放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用治療MGD所致干眼的臨床觀察

【摘要】：目的:通過臨床觀察,研究放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用治療MGD所致干眼的療效。方法:本課題采用隨機分組法將90例干眼患者分為3組(每組30例)。對照組1(中藥組)采用中藥清眩潤目飲內(nèi)外并用治療,對照組2(西藥配合瞼板腺按摩組)采用普拉洛芬滴眼液、玻璃酸鈉滴眼液聯(lián)合加替沙星眼用凝膠局部點眼配合瞼板腺按摩,治療組(放血拔罐聯(lián)合中藥組)放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用治療。三組均以4周為療程。對比觀察治療前后患者的主觀癥狀積分(OSDI),淚液分泌試驗(SIT),淚膜破裂時間(BUT),瞼板腺功能檢查,角膜熒光素染色(FCS),中醫(yī)主癥的指標(biāo),進行優(yōu)效性檢驗。結(jié)果:1.三組患者治療前后主觀癥狀積分(OSDI),淚液分泌試驗(SIT),淚膜破裂時間(BUT),瞼板腺功能檢查,角膜熒光素染色(FCS),中醫(yī)主癥均有改善。治療組各項指標(biāo)治療前后改善程度明顯,且有顯著性意義(p0.01)。2.在主觀癥狀積分方面,治療前后差值組間比較,治療組優(yōu)于兩對照組,治療組分別與對照組1,對照組2比較,具有統(tǒng)計學(xué)意義(p0.05)。3.在淚膜破裂時間(BUT)方面,運用LSD法對三組間治療前后差值進行兩兩比較結(jié)果顯示,治療組療效優(yōu)于兩對照組,且差異有統(tǒng)計學(xué)意義(P0.05);對照組1和對照組2差異無統(tǒng)計學(xué)意義(P=0.5270.05)。4.在淚液分泌試驗(Schirmer試驗)方面,三組治療前后差值比較,總體差異有統(tǒng)計學(xué)意義(P0.05)。治療組療效優(yōu)于對照組1和對照組2,且有統(tǒng)計學(xué)意義;對照組1優(yōu)于對照組2,但無統(tǒng)計學(xué)意義。5.在瞼板腺功能檢查方面,治療前后組間差值比較,治療組優(yōu)于兩對照組(P0.05),對照組1療效的雖高于對照組2,但兩組比較差異無統(tǒng)計學(xué)意義(P0.05)。6.在角膜熒光素染色方面,治療前后差值組間比較,治療組效果優(yōu)于兩對照組(P0.05)。治療組與對照組1和對照組2相比較,均有統(tǒng)計學(xué)意義(P0.05)。但兩對照組差異無統(tǒng)計學(xué)意義。7.在中醫(yī)主癥方面,三組治療前后差值比較,總體差異有統(tǒng)計學(xué)意義(P0.05)。運用LSD法對三組間治療前后差值進行兩兩比較結(jié)果顯示,治療組明顯高于對照組1、對照組2,差異有統(tǒng)計學(xué)意義(P=0.0000.05);對照組1和對照組2差異無統(tǒng)計學(xué)意義(P=0.1720.05)。說明治療組治療效果優(yōu)于對照組1和對照組2,且有統(tǒng)計學(xué)意義;對照組1優(yōu)于對照組2,但無統(tǒng)計學(xué)意義。結(jié)論:1.放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用、西藥聯(lián)合瞼板腺按摩、中藥清眩潤目飲內(nèi)外并用對MGD所致干眼均具有治療效果,三者均能改善患者的自覺癥狀、減輕眼表炎癥,增加淚膜的穩(wěn)定性,促進淚液分泌。2.在改善瞼板腺功能方面,放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用具有顯著優(yōu)勢。3.在改善中醫(yī)癥狀方面,放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用療效甚佳。4.放血拔罐聯(lián)合中藥清眩潤目飲內(nèi)外并用治療MGD所致干眼的療效明顯優(yōu)于西藥聯(lián)合瞼板腺按摩和中藥清眩潤目飲內(nèi)外并用。而單純中藥清眩潤目飲內(nèi)外并用優(yōu)于西藥聯(lián)合瞼板腺按摩。
