【摘要】：目的研究冠心病痰瘀互結(jié)證病機(jī)轉(zhuǎn)化相關(guān)證候診斷指標(biāo)的權(quán)重,為冠心病痰瘀互結(jié)證轉(zhuǎn)化相關(guān)證候的診斷標(biāo)準(zhǔn)研究提供依據(jù)。方法邀請(qǐng)全國(guó)各地具有豐富臨床經(jīng)驗(yàn)的中醫(yī)心腦血管病專家,采用現(xiàn)場(chǎng)問(wèn)卷調(diào)查的方法,對(duì)冠心病痰瘀互結(jié)病機(jī)轉(zhuǎn)化3個(gè)證候(阻絡(luò)、化火、生毒)診斷指標(biāo)的重要程度進(jìn)行問(wèn)卷調(diào)查,采用主成分分析法利用綜合評(píng)價(jià)函數(shù)確定其權(quán)重。結(jié)果 (1)阻絡(luò)證候診斷指標(biāo)權(quán)重順序:心悸、脈澀、胸痛反復(fù)發(fā)作,日久不愈、斑塊不穩(wěn)定、炎性活動(dòng)增強(qiáng)、肢體麻木、煩躁、胸悶、脈弦、斑塊形成;(2)化火證候診斷指標(biāo)權(quán)重順序:斑塊形成、斑塊不穩(wěn)定、炎性活動(dòng)增強(qiáng)、心痛如灼、煩躁、脈數(shù)、口苦、苔黃、舌紅、咽干、溲赤、便秘;(3)生毒證候診斷指標(biāo)權(quán)重順序:咽痛甚、口瘡、重度口苦、病情兇險(xiǎn)、舌下絡(luò)脈紫紅或絳紫、老舌、剝苔(不含類剝苔)、中重度心絞痛、脈澀、斑塊形成、斑塊不穩(wěn)定、舌青或青紫、炎性活動(dòng)增強(qiáng)、脈結(jié)代。結(jié)論冠心病痰瘀互結(jié)轉(zhuǎn)化證候的宏觀診斷指標(biāo):阻絡(luò)證候以心悸、脈澀、胸痛反復(fù)發(fā)作、日久不愈、肢體麻木、煩躁為重要參考指標(biāo);化火證候以心痛如灼、煩躁、脈數(shù)、口苦、苔黃、舌紅;為重要參考指標(biāo),生毒證候以咽痛甚、口瘡、重度口苦、病情兇險(xiǎn)、舌下絡(luò)脈紫紅或絳紫、老舌為重要參考指標(biāo)。斑塊和炎癥活動(dòng)在轉(zhuǎn)化證候中占較大權(quán)重,在臨床辨證中有重要參考價(jià)值。
[Abstract]:Objective to study the weight of diagnosis index of pathogenesis transformation of phlegm and blood stasis syndromes in coronary heart disease (CHD), and to provide basis for the study of diagnostic criteria of syndrome transformation of phlegm and blood stasis syndromes in coronary heart disease (CHD). Methods the experts of cardiovascular and cerebrovascular diseases with rich clinical experience from all over the country were invited to transform the pathogenesis of phlegm and blood stasis of coronary heart disease into three syndromes (blocking collaterals and removing fire) by using the method of on-the-spot questionnaire. The importance of the diagnostic index was investigated by questionnaire, and the weight was determined by the comprehensive evaluation function by principal component analysis (PCA). Results (1) weight order of diagnosis index of blocking collateral syndrome: palpitation, pulse acerbity, chest pain repeated attack, long unhealed, unstable plaque, increased inflammatory activity, limb numbness, irritability, chest tightness, pulse string, (2) weight order of diagnosis index of fire syndrome: plaque formation, plaque instability, inflammatory activity, heart pain such as burning, irritability, pulse number, bitter mouth, yellow moss, tongue red, dry pharynx, Deutzia, etc. Constipation; (3) weight order of diagnostic indexes of toxic syndromes: sore throat, mouth sore, severe oral pain, dangerous condition, purple or purple sublingual collaterals, old tongue, exfoliating fur (excluding exfoliation), moderate and severe angina pectoris, acerbity of veins, plaque formation, plaque instability, The tongue is green or blue, inflammatory activity is increased, pulse knot generation. Conclusion the macroscopical diagnostic indexes of syndrome transformation of phlegm and blood stasis in coronary heart disease are palpitation, pulse acerbity, chest pain repeated attack, long days without healing, limb numbness and irritability, and fire syndrome with heart pain such as cauterization, irritability, pulse number, etc. Bitter mouth, yellow fur, red tongue; as an important reference index, toxic syndromes are very sore throat, mouth sore, severe bitter mouth, dangerous condition, purple or crimson of the sublingual collaterals, and the old tongue is an important reference index. Plaque and inflammation play an important role in transforming syndromes and have important reference value in clinical syndrome differentiation.
