【摘要】：目的：觀察電針刺激股四頭肌、股內(nèi)收肌及腓腸肌肌腹對(duì)中風(fēng)后下肢運(yùn)動(dòng)功能障礙者的作用,探討其對(duì)患側(cè)下肢肌張力、運(yùn)動(dòng)能力以及日常生活能力的影響。方法：選擇符合入選標(biāo)準(zhǔn)的中風(fēng)后下肢運(yùn)動(dòng)功能障礙患者60例,隨機(jī)分為治療組和對(duì)照組,每組各有30例。兩組患者皆給予常規(guī)針刺治療；在此基礎(chǔ)上,治療組給予電針刺激股四頭肌、股內(nèi)收肌及腓腸肌肌腹。在治療前后采用改良Ashworth量表評(píng)定患者下肢肌張力等級(jí)評(píng)分,并通過下肢簡(jiǎn)式Fugl-Meyer運(yùn)動(dòng)功能評(píng)分量表的得分情況評(píng)估患者下肢運(yùn)動(dòng)功能,以及改良Barthel量表評(píng)定患者日常生活活動(dòng)能力。結(jié)果：1.兩組患者治療前MAS的等級(jí)評(píng)分之間沒有統(tǒng)計(jì)學(xué)差異(P0.05),療后治療組等級(jí)評(píng)分較對(duì)照組下降明顯,有統(tǒng)計(jì)學(xué)差異(P0.05)2.治療后兩組患者下肢簡(jiǎn)式Fugl-Meyer運(yùn)動(dòng)功能評(píng)分較療前皆有所提高,并且進(jìn)行組間分?jǐn)?shù)比較時(shí),有顯著統(tǒng)計(jì)學(xué)差異(P0.01),治療組分?jǐn)?shù)更高；3.治療后兩組患者改良Barthel量表得分情況均較治療前有所提高,同時(shí)進(jìn)行組間比較得分情況時(shí)有統(tǒng)計(jì)學(xué)差異(P0.05),治療組優(yōu)之。結(jié)論：1.電針刺激股四頭肌、股內(nèi)收肌及腓腸肌肌腹對(duì)中風(fēng)后下肢運(yùn)動(dòng)功能障礙患者的肌張力增高有一定抑制作用,并優(yōu)于常規(guī)針刺組；2.電針刺激股四頭肌、股內(nèi)收肌及腓腸肌肌腹可以改善中風(fēng)后下肢運(yùn)動(dòng)功能障礙患者的運(yùn)動(dòng)能力,并優(yōu)于常規(guī)針刺組；3.電針刺激股四頭肌、股內(nèi)收肌及腓腸肌肌腹使中風(fēng)后下肢運(yùn)動(dòng)功能障礙患者的日常生活能力有所提高,并優(yōu)于常規(guī)針刺組。
[Abstract]:Aim: to observe the effects of electroacupuncture on quadriceps femoris, adductor femoris and gastrocnemius muscle on patients with lower extremity motor dysfunction after stroke, and to explore the effects of electroacupuncture on muscle tension, motor ability and daily living ability of affected lower extremities. Methods: sixty patients with motor dysfunction after stroke were randomly divided into treatment group and control group with 30 cases in each group. On this basis, the treatment group was treated with electroacupuncture to stimulate quadriceps femoris, adductor femoris and gastrocnemius muscle. Before and after treatment, the patients were assessed with modified Ashworth scale, and the patients' lower extremity motor function was evaluated by the simple Fugl-Meyer motor function scale. And the modified Barthel scale was used to evaluate the activity of daily living (ADL). The result is 1: 1. There was no significant difference in MAS scores between the two groups before treatment (P0.05). After treatment, the score of MAS in the treatment group was significantly lower than that in the control group (P0.05). After treatment, the lower extremity Fugl-Meyer motor function scores of the two groups were improved compared with those before treatment, and there was significant difference in scores between the two groups (P0.01), and the scores of the treatment group were higher than that of the control group (P0.01). After treatment, the scores of modified Barthel scale in the two groups were improved compared with those before treatment, and there was statistical difference between the two groups (P0.05), the treatment group was better than the treatment group. Conclusion 1. Electroacupuncture stimulation of quadriceps femoris, adductor femoris and gastrocnemius muscle could inhibit the increase of muscle tension in patients with lower extremity motor dysfunction after stroke, and was better than that of routine acupuncture group. Electroacupuncture stimulation of quadriceps femoris, adductor femoris and gastrocnemius muscle can improve the motor ability of patients with lower extremity motor dysfunction after apoplexy, and is superior to that of routine acupuncture group. Electroacupuncture stimulation of quadriceps femoris, adductor femoris and gastrocnemius improved the daily life ability of patients with motor dysfunction of lower extremities after stroke, and was superior to the routine acupuncture group.
