【摘要】：目的:觀察《金匱要略》經(jīng)方澤瀉湯顆粒(以下簡稱"金匱澤瀉湯顆粒")治療良性陣發(fā)性位置性眩暈(BPPV)的臨床療效和安全性。方法:選擇中國中醫(yī)科學(xué)院廣安門醫(yī)院、河南省中醫(yī)院、北京博愛醫(yī)院門診就診的BPPV患者212例,按隨機(jī)數(shù)字表法分為治療組(92例)和對照組(120例)。對照組給予改良Epley復(fù)位法,治療組在對照組基礎(chǔ)上給予金匱澤瀉湯顆粒辨證加減,在治療后第2天、2周、3個月進(jìn)行眩暈殘障程度、臨床療效、不良事件的評定。結(jié)果:治療組在治療后2 d、2周、3個月的DHI評分較同期對照組評分改善明顯,治療組在治療后2 d、2周、3個月總有效率(76.09%、88.04%、95.65%)均優(yōu)于對照組(61.67%、73.33%、83.33%)。治療組病例變位試驗(yàn)、單側(cè)水平半規(guī)管功能參數(shù)治療后2 d、2周、3個月的陽性率低于對照組,隨訪3個月與治療后2 d比較差異有統(tǒng)計學(xué)意義,組間不良反應(yīng)發(fā)生情況差異無統(tǒng)計學(xué)意義。結(jié)論:金匱澤瀉湯顆粒能改善心下支飲型良性陣發(fā)性位置性眩暈耳石復(fù)位后的臨床療效,減輕眩暈的殘障程度及前庭功能,具有一定的遠(yuǎn)期療效,臨床安全性較好。
[Abstract]:Objective: to observe the clinical efficacy and safety of Jingzexie decoction (JKZD) in the treatment of benign paroxysmal positional vertigo (BPPVV). Methods: 212 BPPV patients were selected from Guanganmen Hospital of Chinese Academy of traditional Chinese Medicine, Henan traditional Chinese Medicine Hospital and Beijing Boai Hospital. According to the method of random digital table, 212 BPPV patients were divided into treatment group (n = 92) and control group (n = 120). The control group was treated with modified Epley reduction method, the treatment group was treated with Jinkui Zexie decoction on the basis of syndrome differentiation and subtraction, and the degree of vertigo and disability, clinical effect and adverse events were evaluated at 2 weeks and 3 months after treatment. Results: the DHI scores of the treatment group at 2 days and 3 months after treatment were significantly improved than those of the control group. The total effective rate of the treatment group was 76.09 at 2 weeks and 3 months after the treatment. The total effective rate of the treatment group was higher than that of the control group (61.6773.33 and 83.33). The positive rate of unilateral horizontal semicircular canal function parameters in the treatment group was significantly lower than that in the control group at 2 weeks and 3 months after treatment. There was a significant difference between the follow-up of 3 months and 2 days after treatment. There was no significant difference in the incidence of adverse reactions between groups. Conclusion: Jinguizexie decoction can improve the clinical effect after the reduction of the benign paroxysmal positional vertigo otolith, alleviate the degree of disability and vestibular function of vertigo, have some long-term curative effect, and the clinical safety is better.
