【摘要】：糖尿病腎病(diabetic kidney disease,DKD),以下簡稱DKD.屬于糖尿病微血管病變范疇,是糖尿病嚴重并發(fā)癥之一,為臨床常見的繼發(fā)性腎臟病。富碘中藥是指含碘量豐富的中藥,其代表為海藻、昆布。常用于治療甲狀腺疾病,鮮少有人提及其在DKD中的使用和作用。導(dǎo)師高天舒教授深受呂仁和教授“微型ve瘕”觀點的影響,并在此基礎(chǔ)上,認為DKD發(fā)病的根源是火熱之邪為禍,導(dǎo)致痰濁瘀血,損害腎之絡(luò)脈,形成“微型ve瘕”。治法當以軟堅散結(jié)為基礎(chǔ)。血瘀證明顯者,需要活血化瘀,軟堅散結(jié);痰濕證突出者,應(yīng)該化痰除濕,軟堅散結(jié);氣滯證為主者,可以理氣開郁,軟堅散結(jié);水濕證為主者,就要利水祛濕,軟堅散結(jié)。兼見熱象者,佐以清熱之法。火熱為主者,直需清熱降火,軟堅散結(jié)。海藻和昆布性味都有咸寒,都歸肝、胃、腎經(jīng),功效都是消痰軟堅散結(jié),利水消腫。四氣屬寒,能發(fā)揮清熱之效;五味屬咸,歸經(jīng)入腎,可以引藥入腎,使藥力直達病所。導(dǎo)師據(jù)此將富碘中藥用于中晚期糖尿病腎病的治療,取得良好的臨床效果。
[Abstract]:Diabetic kidney disease (DKD). It belongs to the category of diabetic microangiopathy and is one of the serious complications of diabetes mellitus. It is a common secondary kidney disease. Chinese medicine rich in iodine is a Chinese medicine rich in iodine, which is represented by seaweed and kappa. It is often used in the treatment of thyroid diseases, rarely mentioned its use and role in DKD. Professor Gao Tianshu was deeply influenced by Professor Lv Renhe's idea of "mini ve mass", and on this basis, he thought that the root of the disease was the evil of fire and heat, which caused phlegm and blood stasis, damaged the collaterals of kidney, and formed "mini ve mass". The law should be based on soft consolidation. Blood stasis syndrome obvious, need to promote blood circulation to remove blood stasis, soft strong scattered knot; phlegm dampness syndrome outstanding, should dehumidify phlegm, soft strong loose knot; Qi stagnation syndrome mainly, can regulate Qi open depression, soft strong loose knot; water dampness syndrome mainly, should benefit water dispels dampness, soft strong scatter knot. Also see the heat image, accompanied by the method of clearing heat. Hot-based, the need to clear the fire, soft hard scatter. Seaweed and kelp have salty cold taste, both belong to liver, stomach, kidney meridian, efficacy is dispelling phlegm soft hard scattered knot, water swelling. Four qi is cold, can play the effect of clearing heat; five taste is salty, meridian into the kidney, can lead drugs into the kidney, so that the drug to the disease. Therefore, the Chinese medicine rich in iodine was used in the treatment of advanced diabetic nephropathy, and good clinical effect was obtained.
