【摘要】：該文評價(jià)《中醫(yī)內(nèi)科常見病診療指南》的適用性和臨床應(yīng)用情況,為《指南》修訂提供依據(jù)。由中醫(yī)藥標(biāo)準(zhǔn)研究推廣基地完成,評價(jià)方法包括適用性評價(jià)和應(yīng)用性評價(jià)。適用性評價(jià)采用調(diào)查問卷方法,評價(jià)《指南》的熟悉和使用情況、質(zhì)量水平、臨床適用情況等;應(yīng)用性評價(jià)采用前瞻性病例調(diào)查分析方法,評價(jià)《指南》的臨床應(yīng)用符合度和應(yīng)用效果(包括療效、安全性、經(jīng)濟(jì)性)。結(jié)果顯示,《中醫(yī)內(nèi)科常見病診療指南》包括中醫(yī)病證部分(簡稱中醫(yī)指南)和西醫(yī)疾病部分(簡稱西醫(yī)指南)。適用性評價(jià)結(jié)果顯示,中醫(yī)、西醫(yī)《指南》整體熟悉和使用程度相當(dāng),熟悉程度分別為85.43%,84.57%,使用程度分別為52.10%,54.47%;《指南》整體質(zhì)量較好,其中適用范圍和術(shù)語使用合理性評分較高,分別為91.94%,93.35%,辨證論治相關(guān)內(nèi)容合理性均在75%以上;適用情況評價(jià)較好,其中安全性評分最高;綜合評價(jià)顯示,中醫(yī)、西醫(yī)《指南》的適用比例分別為77.73%,75.46%。應(yīng)用性評價(jià)結(jié)果顯示,《指南》與臨床實(shí)際應(yīng)用的符合度較高,除"其他治法"和中醫(yī)《指南》的"調(diào)攝預(yù)防"以外評分均達(dá)到90%以上;應(yīng)用效果評價(jià)顯示,安全性經(jīng)濟(jì)性療效;綜合評價(jià)結(jié)果,選擇較好的比例(≥7分)在75%~80%�！吨嗅t(yī)內(nèi)科常見病診療指南》在臨床實(shí)踐中獲得了較好的普及和應(yīng)用,質(zhì)量水平較高,臨床適用性較好,具有良好的安全性和經(jīng)濟(jì)性。但存在缺少循證醫(yī)學(xué)方法和創(chuàng)新性等不足,需要在修訂中不斷完善。
[Abstract]:This article evaluates the applicability and clinical application of the guidelines for the diagnosis and treatment of Common Diseases in Internal Medicine of traditional Chinese Medicine, and provides the basis for the revision of the guidelines. The evaluation method includes applicability evaluation and application evaluation. A questionnaire was used to evaluate the familiarity and use of the guidelines, the quality level, the clinical application and so on, and the prospective case investigation and analysis methods were used to evaluate the applicability of the guidelines. To evaluate the clinical consistency and efficacy (including efficacy, safety, and economy) of the guidelines. The results showed that the guidelines for the diagnosis and treatment of common diseases in internal medicine of traditional Chinese medicine included the part of disease and syndrome of traditional Chinese medicine (TCM guide for short) and the part of disease of western medicine (abbreviated as western medicine guide). The results of applicability evaluation showed that the whole familiarity and usage degree of Chinese medicine and western medicine were the same, the familiarity degree was 85.43 and the usage degree was 52.1010 and 54.47.The whole quality of the guide was better, and the scope of application and the reasonableness of terminology were higher. The rationality of the related contents based on syndrome differentiation is above 75%; the evaluation of application is better, in which the safety score is the highest; the comprehensive evaluation shows that the applicable proportion of traditional Chinese medicine and western medicine is 77.73 and 75.46 respectively. The results of application evaluation showed that the degree of conformity between the guidelines and clinical practice was high, except for "other treatment methods" and "adjustment and prevention" of traditional Chinese medicine (TCM), the scores were above 90%, and the evaluation of application effect showed that the safety and economic effect were higher than that of the traditional Chinese medicine.
