【摘要】：目的：觀察清肝燥濕解毒湯對慢性乙型病毒性肝炎(肝郁脾虛兼濕熱型)患者DNA的影響以及患者臨床癥狀、體征變化,探討中藥對慢性乙型病毒性肝炎的療效。方法：此次收入的患者均來源于2015年3月24日-2016年1月黑龍江中醫(yī)藥大學附屬第一醫(yī)院肝脾胃病科門診收治的患者,共納入45例。所有患者均經(jīng)西醫(yī)診斷標準診斷為慢性乙型病毒性肝炎,經(jīng)中醫(yī)辨證為肝郁脾虛兼濕熱型。年齡在18-45歲之間,男女不限,入組前已診為肝炎患者,且HBV-DNA含量測定在1.0E+04-1.0E+07之間。就診前半年內(nèi)未服用抗病毒藥,且轉(zhuǎn)氨酶持續(xù)升高,高于正常數(shù)值的2-3倍。連續(xù)服藥3個月,每2周復診1次,每個月復查一次DNA含量。治療期間忌食辛辣、生冷、油膩等刺激性食物,禁飲咖啡、煙酒及濃茶,注意休息,并停服其他可能影響此次臨床觀察的任何藥物。療后經(jīng)統(tǒng)計學處理比較患者治療各階段HBV-DNA含量變化,以及臨床癥候的改善情況。結(jié)果：1.治療后各階段,癥狀積分明顯低于上一治療階段,且與治療前一階段比較有差異(P0.05)。2.治療后各階段,病毒含量明顯低于上一治療階段,且與治療前一階段比較有差異(P0.05)。3.患者在治療各階段期間均無明顯不良反應(yīng)及毒副作用。治療期間有5例患者因未完成臨床觀察,中途失聯(lián),故病例脫落。結(jié)論：1.清肝燥濕解毒湯可以抑制HBV復制,降低病毒的含量。2.清肝燥濕解毒湯可改善慢性乙型病毒性肝炎患者的臨床癥候。
[Abstract]:Objective: to observe the effect of Qinggan Dishishish-jiedu decoction on DNA of patients with chronic viral hepatitis B (liver stagnation and spleen deficiency combined with damp-heat type) and the changes of clinical symptoms and signs, and to explore the curative effect of traditional Chinese medicine on chronic viral hepatitis B. Methods: all the patients were from the outpatient department of liver, spleen and stomach of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine from March 24, 2015 to January, 2016. A total of 45 patients were included. All patients were diagnosed as chronic viral hepatitis B by western medicine, and liver stagnation and spleen deficiency combined with dampness and heat were diagnosed by traditional Chinese medicine. The patients aged between 18 and 45 years old, male and female, were diagnosed with hepatitis before admission, and the HBV-DNA content was determined between 1.0E04-1.0E07. No antiviral drugs were taken in the first half of the year, and transaminase increased continuously, 2-3 times higher than the normal value. Take medicine continuously for 3 months, visit once every 2 weeks, recheck DNA content once a month. Avoid spicy, cold, greasy and other irritant foods during the treatment. Do not drink coffee, alcohol, tobacco and strong tea, take rest and stop taking any other drugs that may affect the clinical observation. After treatment, the changes of HBV-DNA and the improvement of clinical symptoms were compared. The result is 1: 1. The symptom score of each stage after treatment was significantly lower than that of the previous stage, and there was a difference compared with the previous stage (P0.05. 2). The virus content in each stage after treatment was significantly lower than that in the previous treatment stage, and there was a difference compared with the previous stage (P0.05. 3). There were no significant adverse reactions and side effects in all stages of treatment. During the course of treatment, 5 patients lost their connection because they had not completed the clinical observation. Conclusion 1. Qinggan dry dampness jiedu decoction can inhibit HBV replication and reduce virus content. 2. Qinggan Dishish-jiedu decoction can improve the clinical symptoms of patients with chronic viral hepatitis B.
