【摘要】：研究背景:頸椎病已成為常見病、多發(fā)病,成為全球重要的公共衛(wèi)生問題,改善、緩解頸椎病患者的臨床癥狀也成為現(xiàn)代醫(yī)學(xué)的重要任務(wù),應(yīng)高度重視,予以深入研究。本研究通過進(jìn)一步系統(tǒng)觀察、對(duì)比、總結(jié)、分析研究,以探尋一種既能提高臨床療效,減輕患者疾苦,又切實(shí)可行的治療頸椎病的方法,以適應(yīng)我國(guó)構(gòu)建和諧社會(huì)、提高國(guó)民健康體質(zhì)、提高生活幸福指數(shù)的需求,為中醫(yī)針灸優(yōu)勢(shì)的拓展尋找適宜的方法和途徑,為中醫(yī)科學(xué)發(fā)展作貢獻(xiàn)。目的:泰式按摩目前在國(guó)際上研究的不是很多,其傳統(tǒng)基礎(chǔ)手法以印度草醫(yī)和瑜伽在數(shù)百年間經(jīng)無數(shù)大師代代相傳至今�，F(xiàn)代泰式按摩,是泰國(guó)社會(huì)中一種技藝,是理論性很強(qiáng)的一門科學(xué),但是國(guó)際上對(duì)其獨(dú)特的手法了解的不多,本人想通過本研究介紹泰式按摩手法特點(diǎn)并觀察其配合頸三針治療頸型頸椎病的療效,來客觀的評(píng)價(jià)其療效。方法:本臨床在研究期間嚴(yán)格按照臨床診斷標(biāo)準(zhǔn)、納入、排除標(biāo)準(zhǔn)篩選出60例病例,將60例合格頸型頸椎病患者隨機(jī)分為對(duì)照組30例(頸三針)、治療組30例(頸三針配合泰式按摩)。兩組患者均選用頸三針針刺方法,針刺完后治療組加上泰式按摩手法,治療組及對(duì)照組均隔天治療一次,1周治療3次,2周為1個(gè)療程,連續(xù)治療2個(gè)療程。2個(gè)療程后觀察療效。在治療前后對(duì)兩組患者進(jìn)行NPQ及McGill量表問卷評(píng)定,從而判定療效。結(jié)果:將納入的60例頸型頸椎病患者隨機(jī)分為兩組,本研究采用SPSS13.0進(jìn)行統(tǒng)計(jì)分析,兩組間的比性(P0.05)表示有統(tǒng)計(jì)學(xué)差異。,兩組患者一般情況分布均衡,研究觀察具有科學(xué)性和可信性。(1)McGill評(píng)分具有可比性(P0.05)。經(jīng)過2個(gè)療程治療后,治療組和對(duì)照組McGill評(píng)分均下降,臨床癥狀均有緩解,頸部功能不同程度恢復(fù)及頸部疼痛緩解而改善睡眠。治療組PRI、VAS、PPI評(píng)分均較對(duì)照組下降明顯(P0.05)。泰式按摩手法可明顯改善頸型頸椎病患者的疼痛。(2)NPQ問卷評(píng)分具有可比性(P0.05)。經(jīng)過2個(gè)療程治療后,治療組和對(duì)照組NPQ評(píng)分均下降,臨床癥狀均有緩解,治療組評(píng)分較對(duì)照組下降更為顯著(P0.05)。泰式按摩手法可緩解頸型頸椎病患者的臨床癥狀,有助于患者局部功能的改善。(3)臨床療效分析:根據(jù)臨床療效統(tǒng)計(jì),治療組患者臨床治愈為3例,顯效18例,有效7例,無效2例,治愈率為10.00%,總有效率為93.33%。對(duì)照組患者臨床治愈為1例,顯效16例,有效8例,無效5例,治愈率為3.33%,總有效率為83.33%。經(jīng)統(tǒng)計(jì)分析,治療組臨床療效優(yōu)于對(duì)照組(P0.05)。結(jié)論:本臨床課題研究經(jīng)過統(tǒng)計(jì)分析表示,頸三針及頸三針配合泰式按摩均可改善頸型頸椎病的臨床癥狀,且頸三針配合泰式按摩的療效優(yōu)于單純的頸三針療法,為頸三針和泰式按摩治療頸型頸椎病提供客觀依據(jù)。同時(shí)為泰式按摩在國(guó)內(nèi)外的頸型頸椎病治療以及其他適宜疾病的治療中發(fā)揮優(yōu)勢(shì)提供參考。
[Abstract]:Background: cervical spondylopathy has become a common disease, frequent disease, become an important global public health problems, improve the clinical symptoms of patients with cervical spondylosis has become an important task of modern medicine, should be attached great importance to, in-depth research. Through further systematic observation, comparison, summary and analysis, this study aims to explore a method that can not only improve the clinical efficacy, alleviate the suffering of patients, but also be feasible to treat cervical spondylosis, in order to adapt to the construction of a harmonious society in China. To improve the national health and physique, to improve the demand of life happiness index, to find suitable ways and means for the development of acupuncture and moxibustion in TCM, and to contribute to the development of TCM science. Objective: Thai massage has not been studied much in the world at present. Its traditional basic techniques are Indian herbalists and yoga, which have been passed down from generation to generation by countless masters over the past several hundred years. Modern Thai massage is a skill in Thai society and a very theoretical science. However, the international community does not know much about its unique techniques. I would like to introduce the characteristics of Thai massage manipulation and observe the curative effect of the treatment of cervical spondylosis. Methods: during the study, 60 cases were selected according to the clinical diagnostic criteria and exclusion criteria. 60 eligible patients with cervical spondylosis were randomly divided into control group (30 cases) and treatment group (30 cases). The treatment group and the control group were treated once every other day, 3 times a week, 2 weeks as a course of treatment, 2 consecutive courses of treatment. After 2 courses of treatment, the treatment group was treated with Thai massage manipulation, and the treatment group and the control group were treated every other day for 3 times and 2 weeks as a course of treatment, and the curative effect was observed after 2 courses of treatment. NPQ and McGill questionnaire were used before and after treatment to evaluate the curative effect. Results: sixty patients with cervical spondylosis were randomly divided into two groups. SPSS 13.0 was used for statistical analysis. The observation was scientific and credible. McGill score was comparable (P 0.05). After two courses of treatment, the McGill scores of the treatment group and the control group were decreased, the clinical symptoms were relieved, the neck function was recovered and the neck pain was relieved, and the sleep was improved. Compared with the control group, the PPI scores in the treatment group were significantly lower than those in the control group (P 0.05). Thai massage manipulation can significantly improve the pain of patients with cervical spondylosis. After two courses of treatment, the NPQ scores of the treatment group and the control group were all decreased, and the clinical symptoms were relieved. The scores of the treatment group were significantly lower than that of the control group (P 0.05). Thai massage technique can relieve the clinical symptoms of cervical spondylopathy, which is helpful to the improvement of local function of patients. Analysis of clinical curative effect: according to the statistics of clinical curative effect, the treatment group has 3 cases of clinical cure, 18 cases of remarkable effect and 7 cases of effective. The cure rate was 10.00 and the total effective rate was 93.33. In the control group, 1 case was clinically cured, 16 cases were markedly effective, 8 cases were effective, 5 cases were ineffective, the cure rate was 3.33 and the total effective rate was 83.33. The clinical effect of the treatment group was better than that of the control group (P 0.05). Conclusion: the statistical analysis of this clinical study shows that the clinical symptoms of cervical spondylopathy can be improved by three cervical acupuncture and Thai massage, and the curative effect of three-needle combined with Thai massage is better than that of simple three-needle therapy. To provide the objective basis for the treatment of cervical spondylosis with three needles and Thai massage. At the same time, it provides a reference for Thai massage in the treatment of cervical spondylosis at home and abroad, as well as in the treatment of other suitable diseases.
