【摘要】：目的:(1)研究分析中醫(yī)肺病門診咳嗽患者的病因;(2)歸納中醫(yī)肺病門診咳嗽中醫(yī)證候分布特點;(3)用于指導臨床,提供診治疾病的依據(jù)。方法:采用回顧性流行病學調查方法,收集2014年10月至2015年12月就診于山東中醫(yī)藥大學附屬醫(yī)院中醫(yī)肺病門診,以咳嗽癥狀為主訴的80例患者病歷資料,明確咳嗽的病因診斷;運用因子分析等統(tǒng)計方法,總結中醫(yī)咳嗽證候分布特點。結果:中醫(yī)肺病門診咳嗽患者的主要病因診斷依次為:氣管-支氣管炎,急性上呼吸道感染,感染后咳嗽,咳嗽變異性咳嗽,支氣管擴張癥,支氣管哮喘,肺炎,間質性肺疾病,變應性鼻炎。少見的病因診斷依次為:變應性咳嗽,肺結核,胃食管反流性疾病,原發(fā)性支氣管肺癌,喉源性咳嗽,胸腔積液。主要為呼吸系統(tǒng)疾病,涉及耳鼻喉科、消化科等多科疾病。此次研究肺病門診咳嗽患者常見的中醫(yī)證候依次為:肺熱傷津證,痰濕蘊肺證,風燥傷肺證,肺陰虛證,肺脾氣虛證。結論:咳嗽的病因繁多,涉及除呼吸系統(tǒng)以外的其他學科,中醫(yī)證候也較多,應將咳嗽的病因診斷與證候結合起來,重視辨病論治與辨證論治相結合。
[Abstract]:Objective to study and analyze the etiology of cough patients in out-patient clinic of traditional Chinese medicine (TCM) and summarize the distribution characteristics of TCM syndromes of cough in out-patient clinic of traditional Chinese medicine (TCM) and to provide the basis for diagnosis and treatment of diseases. Methods: a retrospective epidemiological survey was conducted to collect the medical records of 80 patients with cough symptoms from October 2014 to December 2015 in the clinic of traditional Chinese Medicine (TCM) of affiliated Hospital of Shandong University of traditional Chinese Medicine (Shandong University of traditional Chinese Medicine), and the etiological diagnosis of cough was confirmed. By using factor analysis and other statistical methods, the characteristics of cough syndrome distribution in TCM were summarized. Results: the main etiological diagnoses of cough patients were tracheobronchial bronchitis, acute upper respiratory tract infection, post-infection cough, cough variant cough, bronchiectasis, bronchial asthma, pneumonia. Interstitial lung disease, allergic rhinitis. The rare etiological diagnoses were allergic cough, pulmonary tuberculosis, gastroesophageal reflux disease, primary bronchogenic carcinoma, laryngeal cough and pleural effusion. Mainly for respiratory diseases, involving otolaryngology, digestive and other multi-disciplinary diseases. The common TCM syndromes of cough patients in this study are: lung heat injury syndrome, phlegm dampness accumulation lung syndrome, wind dryness injury lung syndrome, lung yin deficiency syndrome, lung spleen qi deficiency syndrome. Conclusion: the etiology of cough is various, involving other subjects other than respiratory system, and TCM syndromes are also more. It is necessary to combine the etiological diagnosis and syndromes of cough, and pay attention to the combination of syndrome differentiation and syndrome differentiation.
