三法三穴干預(yù)坐骨神經(jīng)損傷大鼠運動功能刺激參數(shù)研究

【摘要】:[目的]通過正交試驗指導(dǎo)下比較行為學(xué)、腓腸肌功能、免疫組織化學(xué)相關(guān)指標(biāo)的改變,從行為學(xué)下研究推拿手法參數(shù)中的力量、時間、頻率對周圍神經(jīng)損傷大鼠運動功能恢復(fù)的影響。探究推拿促進坐骨神經(jīng)損傷大鼠運動功能恢復(fù)的最佳刺激參數(shù)組合。[方法]以坐骨神經(jīng)損傷為例研究周圍神經(jīng)損傷,以SD大鼠為實驗動物,采用坐骨神經(jīng)加持損傷模型。7天后使用"推拿手法模擬儀"模擬三法(撥法、揉法、點法)推拿手法在三個穴道(殷門、承山、陽陵泉)上進行干預(yù)。以力量(A)、時間(B)、頻率(C)三種因素,每個因數(shù)各有三組不同水平刺激,力量為2N、4N、6N;時間1min、3min、5min;頻率30次/min、60次/min、100次/min。用三因素三水平的正交試驗設(shè)計方案篩選推拿干預(yù)促進SNI大鼠運動功能恢復(fù)的最佳刺激參數(shù)組合。1.從行為學(xué)角度,測定斜板實驗及坐骨神經(jīng)功能指數(shù)以評價大鼠肌力功能恢復(fù)情況。2.從功能角度,測定腓腸肌濕重恢復(fù)率評價推拿對肌肉萎縮程度的改善。3.采用免疫組織化學(xué)法測定脊髓腹角內(nèi)NGF的表達情況,探尋推拿促進NGF表達的最佳刺激參數(shù)。[結(jié)果]1.斜板實驗造模7天后斜板實驗數(shù)據(jù)分析,正常組、假手術(shù)組、模型組之間存在統(tǒng)計學(xué)差異(P0.01)。推拿干預(yù)20次后,推拿組(九組)之間采用正交試驗的直觀分析法,研究結(jié)果顯示三因素的主次關(guān)系為:時間力量頻率。最佳刺激參數(shù)組合是力量4N,時間1min,頻率60次/min。推拿組(九組)之間采用方差分析法得出,三因素力量(A)、時間(B)、頻率(C)對結(jié)果的影響無統(tǒng)計學(xué)差異(P0.05)。但從F值的大小判斷(正交試驗中對應(yīng)因素的F值越大,該因素對結(jié)果的影響也越大),來看,可認(rèn)為各因素的主次關(guān)系為:時間力量頻率,而以短時間治療效果為佳。這與直觀分析法得出的結(jié)果一致。2.坐骨神經(jīng)功能指數(shù)推拿干預(yù)20次后,推拿組(九組)之間采用正交試驗的直觀分析法,研究結(jié)果顯示三因素的主次關(guān)系為:頻率時間力量。最佳刺激參數(shù)組合是力量2N,時間1min,頻率30次/mmin。推拿組(九組)之間采用方差分析法得出,三因素力量(A)、時間(B)、頻率(C)對結(jié)果的影響無統(tǒng)計學(xué)差異(P0.05)。但從F值的大小判斷(對應(yīng)因素的F值越大,該因素對結(jié)果的影響也越大),來看,可認(rèn)為各因素的主次關(guān)系為:頻率時間力量,這與直觀分析法得出的結(jié)果一致。3.腓腸肌濕重恢復(fù)率造模7天后,推拿組與模型組腓腸肌濕重恢復(fù)率無顯著性差異(P0.05),推拿干預(yù)20次后,推拿組腓腸肌濕重恢復(fù)率測定結(jié)果與模型組比較有顯著性差異(P0.05),代表腓腸肌萎縮后進行推拿干預(yù)可有預(yù)防肌肉萎縮的效果。推拿干預(yù)20次后,推拿組(九組)之間采用正交試驗的直觀分析法,研究結(jié)果顯示三因素的主次關(guān)系為力量>頻率時間,根據(jù)F值的大小,最佳刺激參數(shù)組合是力量4N,時間5mmin,頻率30次/min。推拿組(九組)之間采用方差分析法得出,三因素力量(A)、時間(B)、頻率(C)的主次關(guān)系無顯著性差異(P0.05)。但從F值的大小判斷(對應(yīng)因素的F值越大,該因素對結(jié)果的影響也越大),來看,可認(rèn)為各因素的主次關(guān)系為:力量頻率時間,這與直觀分析法得出的結(jié)果一致。4.脊髓腹角NGF光密度推拿干預(yù)20次后,推拿組(九組)之間采用正交試驗的直觀分析法,研究結(jié)果顯示三因素的主次關(guān)系為:力量時間頻率。最佳刺激參數(shù)組合是力量4N,時間1min,頻率60次/min。推拿組(九組)之間采用方差分析法得出,三因素力量(A)、時間(B)、頻率(C)對結(jié)果的影響有統(tǒng)計學(xué)差異(P0.05)。各因素的主次關(guān)系為力量時間頻率,但從單因素來看,A(力量)因素、B(時間)因素的影響顯著(P0.05),C(頻率)因素的影響不顯著(P0.05)。[結(jié)論]1.通過三法三穴干預(yù)可以促進SNI大鼠運動功能的恢復(fù),刺激參數(shù)為中等力量、短時間、中等頻率為佳。而手法力量過重、時間過長時療效反而降低。2.通過三法三穴干預(yù)可以調(diào)控SNI大鼠脊髓腹角NGF的表達。通過NGF的提高可使運動功能恢復(fù)而刺激參數(shù)以中等力量、短時間、中頻率為佳。
[Abstract]:[Objective] to compare the effect of behavior, gastrocnemius function and immuno histochemical related indexes under the guidance of orthogonal test, and study the influence of the strength, time and frequency of the manipulative parameters on the recovery of motor function in rats with peripheral nerve injury.
