用肌骨超聲技術探索針刺足三里穴“得氣”相關的局部組織特征

【摘要】:目的:采用高頻肌骨超聲技術實時動態(tài)觀察針刺人體足三里穴"得氣"的過程,對針刺"得氣"時針體所觸及的經(jīng)穴局部組織結構特征及周圍組織進行相關性研究,為臨床和教學提供針刺"得氣"現(xiàn)象的客觀證據(jù)。方法:選取健康志愿者60名(男、女各30名),針刺雙側足三里穴至"得氣"狀態(tài),在肌骨超聲技術實時、動態(tài)觀察下,記錄最強"得氣"感時針尖與皮膚表面、脛骨的距離,在各解剖層次位置分布特點以及受性別、左右側的影響。結果:針刺60名受試者雙側足三里穴共120個穴位點,肌骨超聲圖像顯示"得氣"最強時針尖與皮膚表面的垂直距離為(25.32±5.82)mm,與脛骨的距離為(5.45±0.55)mm;針尖所處局部組織結構層次主要分布為:脛骨前肌肌層靠近深筋膜處46個,脛骨前肌與趾長伸肌相交處筋膜上62個,共108個穴點(108/120,90%);針尖位于脛骨前肌與趾長伸肌相交處筋膜上或脛骨前肌肌層靠近筋膜處獲得最強"得氣"感,其中52個為男性,56個為女性,53個屬左側經(jīng)穴,55個屬右側經(jīng)穴。結論:當針尖位于肌筋膜旁及筋膜集中處"得氣"感最強,提示針刺足三里穴"得氣"的主要組織結構是肌筋膜,這種相關性與性別、部位(左右側)的差異不明顯。
[Abstract]:Objective: to observe the process of acupuncture at Zusanli (Zusanli) by using high frequency musculoskeletal ultrasound, and to study the correlation between the local tissue structure and the surrounding tissues of the acupoints touched by the acupuncture of "deqi". To provide objective evidence for clinical and teaching of acupuncture "qi" phenomenon. Methods: 60 healthy volunteers (30 male and 30 female) were selected to acupuncture bilateral Zusanli (Zusanli) to "deqi" state. The distance between the tip of the needle and the skin surface, tibia and tibia was recorded under the real-time and dynamic observation of the technique of musculoskeletal ultrasound.
